Clinical outcomes of lower cranial nerve palsies caused by vertebral artery-posteroinferior cerebellar artery aneurysms after endovascular embolization.

OBJECTIVE: To report clinical outcomes of lower cranial nerve palsy (LCNP) caused by vertebral artery (VA) aneurysms. METHODS: Seventy-two patients with VA aneurysms, who were treated endovascularly between 2001 and 2008, were retrospectively studied to identify patients presenting LCNP without hemorrhage. RESULTS: LCNP occurred in 12 patients (16·7%) initially caused by VA aneurysms: four located at vertebral artery-posteroinferior cerebellar artery (PICA) complex, seven at distal to PICA segment and one at proximal to PICA segment. Among these patients, ten patients were diagnosed as havin

Endovascular treatment of bilateral intracranial vertebral artery aneurysms: an algorithm based on a 10-year neurointerventional experience.

BACKGROUND: The management of bilateral intracranial vertebral artery dissecting aneurysms (IVADAs) is controversial, and requires the development of endovascular treatment modalities and principles. We aim to investigate the endovascular treatment strategy and outcomes of bilateral IVADAs. METHODS: We identified all bilateral IVADAs at a high-volume neurointerventional centre over a 10-year period (from January 2009 to December 2018). Radiographic and clinical data were recorded, and a treatment algorithm was derived. RESULTS: Twenty-seven patients with bilateral IVADAs (54 IVADAs in total,

Topographic location and branching pattern of the superior mesenteric artery with its clinical relevance: a cadaveric study.

BACKGROUND: The topographic location of the superior mesenteric artery (SMA) and its branching pattern are usually arbitrary in textbooks. This study, therefore, aims to provide topographic information of SMA with reference to the vertebral bodies, ventral branches of aorta and branching pattern of SMA. MATERIALS AND METHODS: The study was conducted on 35 embalmed adult human cadavers. We performed detailed dissection of the SMA to topographically locate its origin in respect to vertebral level and other ventral branches of the abdominal aorta. We have categorised the branching pattern of SMA

Segmental arterial mediolysis.

This article reports the case of a 51-year-old woman in whom brain MRI to follow up multiple sclerosis incidentally discovered an intramural hematoma in the extracranial internal carotid artery. MR angiography of the supra-aortic trunks and CT angiography of the aorta showed arterial dilations, aneurysms, dissections, and intramural hematomas in the internal carotid arteries, vertebral arteries, and arteries in the splanchnic territory. These findings raised suspicion of segmental arterial mediolysis. After 6 months of treatment with antiplatelet drugs, the arterial involvement resolved. Segme

Correlations between the semiologic changes and the imaging aspects in the lateral bulbar infarction.

The study aimed to evaluate the correlations between the clinical and paraclinical data in the lateral bulbar infarction, benefiting from the access to the semiologic characteristics of a group studied and the MRI angiography, without a contrast agent, through the 3D TOF technique combined with MIP, as an imaging technique for the evaluation of the arterial lesion. The study group included 20 patients with lateral bulbar infarction, 14 men, and 6 women aged between 21 and 80 years, the mean age being 56, 9 years, who were enrolled in the study in the period 2012 and 2014, following the admissi

Medial medullary stroke: report of seven patients and review of the literature.

Medial medullary infarctions (MMI) were reported in less than 40 patients with satisfactory clinicotopographic documentation. We studied seven patients with MRI-proven acute MMI seen in two neurologic departments over a 5-year period (1990-1994). MMI represented less than 1% of ischemic strokes in the posterior circulation. Five patients had an infarction above the pyramidal decussation. All patients had contralateral hemiparesis and lemniscal sensory loss, accompanied by ipsilateral lingual palsy (Dejerine's syndrome) in three. Two patients had infarction below the pyramidal decussation, with

Anatomic considerations for uncovertebral involvement in cervical spondylosis.

Morphometric analysis of 54 dry cervical spines from C3 to C7 (a total of 270 cervical vertebrae) and bilateral dissection of 10 anatomic specimen cervical spines were performed. The uncinate processes were significantly higher at C4 to C6 (5.8 +/- 1.1 mm to 6.1 +/- 1.3 mm) levels than at C3 or C7 levels. The anteroposterior diameter of the intervertebral foramina was smaller at the C4, C5, and C6 levels compared with that at the C3 or C7 levels. The length of nerve root between the lateral border of dural tube and medial border of vertebral artery gradually increased from C3 (3.3 +/- 1.1 mm)

Blood supply to the corticospinal tract: A pictorial review with application to cranial surgery and stroke.

The corticospinal tract (CST) is the main neural pathway responsible for conducting voluntary motor function in the central nervous system. The CST condenses into fiber bundles as it descends from the frontoparietal cortex, traveling down to terminate at the anterior horn of the spinal cord. The CST is at risk of injury from vascular insult from strokes and during neurosurgical procedures. The aim of this article is to identify and describe the vasculature associated with the CST from the cortex to the medulla. Dissection of cadaveric specimens was carried out in a manner, which exposed and pr

Differential diagnosis between traumatic and nontraumatic rupture of the intracranial vertebral artery in medicolegal autopsy.

We attempted to establish histopathological identification between traumatic rupture and nontraumatic arterial dissection of the intracranial vertebral artery (IVA) resulting in subarachnoid hemorrhage (SAH). Step-serial observations of ruptured IVAs among four traumatic and 44 nontraumatic SAH patients were investigated. We found that the most specific characteristic for differentiation was the shape of the ruptured adventitia. Extension of the adventitia was clearly observed in nontraumatic cases. In contrast, traumatic cases showed transmural ruptures. Other specific characteristics were al

Occipital Artery -Posterior Inferior Cerebellar Artery Bypass Combined with Aneurysm Trapping for a Recurrent Ruptured Vertebral Artery-Posterior Inferior Cerebellar Artery Aneurysm: Two-Dimensional Operative Video.

Vertebral artery-posterior inferior cerebellar artery (VA-PICA) aneurysm is challenging owing to its variable anatomy and interference of lower cranial nerves. A recurrent ruptured VA-PICA aneurysm was successfully treated by occipital (OA)-PICA bypass with aneurysm trapping (Video 1). Informed consent was obtained from the patient. A ruptured right VA aneurysm was found in a 47-year-old man and treated by endovascular treatment. Follow-up angiography at 1 month revealed aneurysm recurrence with migration of endovascular materials. The patient was placed in a park-bench position for the far-la

Cervicocranial arterial dissection.

Cervicocranial arterial dissection (CCAD) occurs when there is a tear in the intimal layer of the carotid or vertebral arteries with subsequent extravasation of blood into the subintimal layers. The dissection may be extradural, intradural, or extend over both segments. The contents of the subintimal layers are highly thrombogenic, and thus, embolism, vessel stenosis, or occlusion may follow. Symptoms of dissection may be caused by local injury to the blood vessel or by ischemia to the retina or brain. Thus, dissection should always be considered in patients who present with Horner syndrome as

Dissection and Plastination of the Nervous System of a Dog: A Teaching Tool.

Dissection and plastination of the nervous system have been used to teach anatomy; however, there is no record of a dissection and plastination of a dog's complete nervous system. The objective of this study was to dissect and preserve the whole nervous system of a dog, using the plastination technique, as a tool for teaching veterinary neuroanatomy. A 7-month-old dog was perfused through the common carotid artery with an ethanol-based soft embalming solution, and 10% formalin was injected through the cisterna magna. An incision was made at the level of the dorsal midline, and the muscle tissu

Atherosclerosis in patients with cervical artery dissection.

INTRODUCTION: Cervical artery dissection (CeAD) is considered a non-atherosclerotic arteriopathy, but atherosclerosis of the cervical arteries may co-exist. We explored the frequency and clinical importance of co-existent atherosclerosis in patients with CeAD. PATIENTS AND METHODS: Single-center exploratory study from the Stroke Center Basel, Switzerland. We re-reviewed duplex ultrasound images at (i) baseline and (ii) last follow-up visit for the presence versus absence of the following atherosclerotic manifestations in the carotid arteries: (i) abnormal carotid intima-media thickness, (ii)
