Bilateral Vestibular Atelectasis as a Fluctuating Bilateral Vestibulopathy: A Case Report Care.

The vestibular atelectasis (VA) is a very rarely reported clinical and radiological condition characterized by the collapse of the labyrinthine membrane. Clinical features include symptoms which mimic endolymphatic hydrops; bilateral forms are rarely reported in the literature. Tullio's sign was reported in a few cases. In this paper, we report an original case of bilateral fluctuating VA associated with a rare posterior semicircular canal (SSC) dehiscence variant. A 54-year-old patient presented with evolving and fluctuating bilateral vestibulopathy associated with a pressureinduced nystagmus

The importance of the bow and lean test as an initial positional test for diagnosing BPPV.

PURPOSE: This study aimed to evaluate the diagnostic potential of nystagmus direction observed during bow and lean tests (BLTs) in patients with posterior canal canalolithiasis BPPV (P-BPPV), lateral canal canalolithiasis BPPV (Lca-BPPV), and lateral canal cupulolithiasis BPPV (Lcu-BPPV). METHODS: A total of 62 patients (40 women, 22 men; aged 24-70 years) with clinically suspected BPPV were enrolled. Diagnoses included 39 cases of P-BPPV, 15 of Lca-BPPV, and 8 of Lcu-BPPV. Each participant underwent the Dix-Hallpike Test (DH), Head Roll Test (HRT), and bow and lean tests for diagnostic asses

Meniere's Disease.

This article reviews 3 aspects of Meniere's disease (MD), which have been recently revisited: namely, the pathologic mechanism causing the attacks of vertigo, the clinical diagnosis, and the medical and surgical treatments. The characteristic attacks of vertigo are unlikely to be due to membrane ruptures, so a hypothesis that the vertigo is caused by a volume of endolymph shifting suddenly from the cochlea into the pars superior is suggested. The definite diagnosis according to the American Academy of Otolaryngology HNS 1995 criteria [<citeref rid="ref13">13</citeref>] failed to exclude vestib

The impaired subjective perception of verticality independent of peripheral vestibular function in dizzy elderly with orthostatic hypotension.

BACKGROUND: The elderly often complain of faintness, lightheadedness, dizziness, and unsteadiness, which may be associated with the orthostatic hypotension (OH), but most of them are subclinical. The pathogenic mechanism of subjective symptoms in the elderly with OH is controversial. AIMS: The aim of this study was to assess the involvement of the irregularity of the vertical perception in the dizzy elderly with OH. METHODS: This study consisted of 403 patients seeking treatment for dizziness in our hospital. The neurotological examinations include the subjective visual vertical (SVV) test a

Static and dynamic body balance following provocation of the visual and vestibular systems in females with and without joint hypermobility syndrome.

OBJECTIVES: Joint hypermobility syndrome (JHS) is a heritable disorder of the connective tissue characterized by excessive joint movement, musculoskeletal pain and neurophysiological deficits (i.e. decreased proprioceptive acuity, altered neuromuscular reflexes). Such deficits may affect body balance thus increasing the risk of injury. The present study aimed at examining static and dynamic body balance following challenge of the visual and vestibular systems in individuals with JHS. METHODS: The sample consisted of 21 females with JHS and 20 controls without signs of JHS. Static body balance

A new model for utricular function testing using a sinusoidal translation profile during unilateral centrifugation.

The utricle plays an important role in orientation with respect to gravity. The unilateral centrifugation test allows a side-by-side investigation of both utricles. During this test, the subject is rotated about an earth-vertical axis at high rotation speeds (e.g. 400°/s) and translated along an interaural axis to consecutively align the axis of rotation with the left and the right utricle. A simple sinusoidal translation profile (0.013 Hz; amplitude = 4 cm) was chosen. The combined rotation and translation induces ocular counter rolling (OCR), which is measured using 3-D video-oculography. Th

Hearing loss and vestibular dysfunction in Menière's disease.

In a retrospective study of 475 patients suffering from Meniere's disease, the hearing loss, both in the low and high frequencies, and the loss of vestibular function were studied over time. There were large variations of hearing loss from case to case but, overall, the low were more affected than the high frequencies, regardless of time elapsed since onset of disease. Hearing deteriorated over time and mean values of hearing loss correlated well with the elapsed time since onset of disease. There were also large variations of post-caloric labyrinthine preponderance (lp) from case to case but,

[Anxiety syndrome in vertigo patients. A comparison of neuro-otologic findings in patients with and without anxiety].

Anxiety is a common symptom is vertigo patients. ACute anxiety is known in neurology and psychiatry as panic disorder. The cardiovascular and the gastrointestinal subtypes have been used in the classification of panic disorders up to now. Frommberger and co-workers (1993) have also defined a vestibular subtype. In this study, we have compared the neurotologic test results of 53 patients with vertigo in combination with panic disorder and of 53 patients with balance disorders without anxiety to clarify, whether disturbances in the vestibular systems can be objectivated or not. Differences of bo

[Value of Head-Shaking's test in the pre- and the postoperative vestibular assessment].

The value of Head-Shaking maneuver has brought scientific controversies regarding its role as a clinical tool. In the following publication, we explain the discrepancies between the different studies and find out the role of such a maneuver for clinical evaluation during the pre and post operative period. We showed the influence of the diagnosis and the importance of time interval in the post Head-Shaking responses.

Vestibulo-ocular abnormalities in spasmodic torticollis before and after botulinum toxin injections.

In order to establish whether vestibular abnormalities often found in spasmodic torticollis are secondary to the abnormal head posture, the vestibulo-ocular reflex (VOR) was studied in eight patients before and after correction of head posture with botulinum toxin. Eye movements were recorded in the dark during sinusoidal and velocity step rotation. Four patients showed a significantly asymmetric response, with the slow phase of the VOR more active ipsilateral to the torticollis (chin). Despite significant improvement of the head posture in all patients for up to 10 weeks following treatment,

Persistent mal de debarquement syndrome: a motion-induced subjective disorder of balance.

Six patients with a distinct syndrome of persistent sensations of imbalance are presented. Common features included onset following a period of motion exposure, symptoms lasting months to years, mild unsteadiness and anxiety, minimal relief from antivertiginous medication, and normal neurologic and quantitative vestibulo-ocular examinations. One patient experienced recurrent episodes. Mal de debarquement refers to sensations of motion experienced on return to stable land after adaptation to motion lasting from hours to days in normal individuals. The presented patients exemplify a syndrome of

[New aspects of differential vertigo diagnosis in childhood].

Eight cases of vertigo in childhood are reported. Four of them suffered from benign paroxysmal vertigo, and the other four from atypical migraine. History of a head injury and attacks of vertigo lasting only a few seconds are typical of benign paroxysmal vertigo. These children are older than 4 years and the vertigo is of peripheral origin. Vertigo due to atypical migraine is frequent in young children up to 4 years of age, shows a longer duration of vertigo (more than 1 min) and is of central origin.

The mechanism of physiological height vertigo. II. Posturography.

In order to validate the hypothesis that height vertigo is based on visual destabilization of free stance when the distance between eye and object becomes critically large, several of its consequences were demonstrated in posturographic experiments: (1) Visual signals conflicting with simultaneous vestibular and somatosensory inputs provided by sinusoidally tilting rooms may destabilize postural sway in the fore-aft as well as in the lateral direction. (2) In natural surrounding sway amplitudes increase with increasing eye-object distance up to 5 meters. Thus, teleologically, subjective height

[Ataxia and deafness secondary to meningitis in children: the role of labyrinth diseases].

A study is made of post meningitic ataxia from 9 purulent meningitis observations in children. This ataxia is accompanied by a bilateral vestibular destruction syndrome in 6 of the 7 patients examined. The labyrinthic origin of the ataxia accounts for its pure static aspect, its regressive evolution and the frequency of associated deafness. The study of sightless walking on unstable ground (mattress) gives a clinical sight of the vestibular impairment long after the acute phase. In the same way, the study of post-meningitis deafness shows that, most of the time, it is accompanied by an impairm
