Comparative prevalence rates of sexually transmitted diseases in heterosexual and homosexual men.

This study compared prevalence rates of most common sexually transmitted diseases (STD) in heterosexual and homosexual men who made respectively 12,201 and 5324 visits to an STD clinic over 18 months. Overall, homosexual men were significantly (p < 0.001) more likely than heterosexual men to have gonorrhea (30.31% vs. 19.83%), early syphilis (1.08% vs. 0.34%) and anal warts (2.90% vs. 0.26%) but less likely to have nongonococcal urethritis (NGU) (14.63% vs. 36.40%, p < 0.001), herpes genitalis (0.93% vs. 3.65%, p < 0.001), pediculosis pubis (4.30% vs. 5.35%, p < 0.005), scabies (0.42% vs. 0.76

Malakoplakia. Two unusual cases which presented diagnostic problems.

Two unusual cases of malakoplakia are presented. Both cases occurred outside the urinary tract (retroperitoneum and anus) and were diagnostic problems due to the paucity of Michaelis-Guttman bodies. One case mimicked a retroperitoneal malignant fibrous histiocytoma and the other a common perianal abcess.

Long-term results achieved by guideline-based stage-dependent management of anal cancer in a non-HIV population.

PURPOSE: Therapy of anal cancer follows national and international guidelines that are mainly derived from randomized trials. This study aimed to analyze long-term results of stage-dependent treatment of anal cancer in a non-selected patient cohort. PATIENTS AND METHOD: All consecutive patients treated for anal cancer between 2000 and 2015 were retrieved from a prospective database. Risk-dependent screening for human immunodeficiency virus showed no infection. Main outcome measure was overall survival with respect to tumor site and treatment. Secondary endpoints were cause-specific survival,

Inguinal lymph node recurrence in the untreated groin of patients with anal carcinoma.

BACKGROUND: Inguinal lymph node metastasis is predictive of locoregional recurrence and poor overall survival in anal carcinoma. Metachronous lymph node metastasis occurs in 10% of all anal cancer patients, but multiple studies have shown that the benefit of elective irradiation of the groin depends on T-stage, and the toxicity of groin irradiation must not be underestimated. OBJECTIVE: To analyze the inguinal recurrence rates among patients with anal carcinoma (T1-4, N0-1) who did not receive elective irradiation therapy to the groin and to determine predictors of inguinal recurrence. DESIG

Ablation of neoplasia by direct current.

The application of low-voltage direct electrical current (DEC) has been studied in animals and humans for the ablation of anal condylomata, oesophageal cancer and Kaposi's sarcoma. Twenty milliamps of DEC passed through multiple 6 cm x 1 cm, flat-plate longitudinal electrodes into the squamous mucosa of the oesophagus of healthy dogs for periods ranging from 10 min to 2 h resulted in denudation and necrosis of the oesophageal mucosa at the site of application of the current. In humans, the application of DEC to two patients with benign anal condyloma acuminata, three patients with inoperable o

Incidences and risk factors of metachronous vulvar, vaginal, and anal cancers after cervical cancer diagnosis.

OBJECTIVE: To examine incidences and risk factors for metachronous vulvar, vaginal, and anal malignancies after a cervical cancer diagnosis. METHODS: This is a retrospective study examining data from the Surveillance, Epidemiology, and End Result Program between 1973 and 2013. Cumulative incidences of vulvar, vaginal, and anal cancers after the diagnosis of cervical cancer were assessed (n = 79,050). Multivariable analysis was performed to determine independent risk factors for these metachronous cancers. RESULTS: Vaginal cancer (20-year cumulative incidence, 0.57%) was the most common type

Interstitial Brachytherapy for the Treatment of Locally Recurrent Anorectal Cancer.

BACKGROUND: Local tumor control (LC), overall survival (OS), symptom palliation, and late toxicity for patients with locally recurrent anorectal cancer treated with a computed tomography (CT)-guided interstitial brachytherapy implant were examined. METHODS: The medical records of 20 consecutive patients who had received interstitial brachytherapy for locally recurrent anorectal cancer from 2000 through 2012 were reviewed. Seventeen patients (85 %) had rectal cancer and three had anal cancer [median follow-up time for living patients, 23 months (range 13-132)]. Brachytherapy was used most comm

Long-term oncological outcomes after salvage surgery for anal squamous cell carcinoma - a national cohort study.

INTRODUCTION: Anal squamous cell carcinoma (ASCC) is an HPV-related tumor primarily treated with concurrent chemoradiotherapy (CRT). Salvage surgery is reserved for patients with residual or recurrent disease following CRT, representing a highly selected, high-risk subgroup. Population-based data on outcomes and prognostic factors after salvage surgery are limited. This study aimed to assess overall survival (OS) and identify prognostic factors in a centralized, national cohort to inform clinical decision-making. METHODS: Data were retrospectively collected from the Swedish anal cancer regist

Talking about human papillomavirus and cancer: development of consultation guides through lay and professional stakeholder coproduction using qualitative, quantitative and secondary data.

BACKGROUND: High-risk human papillomaviruses (HPVs) cause all cervical cancer and the majority of vulvar, vaginal, anal, penile and oropharyngeal cancers. Although HPV is the most common sexually transmitted infection, public awareness of this is poor. In addition, many clinicians lack adequate knowledge or confidence to discuss sexual transmission and related sensitive issues. Complex science needs to be communicated in a clear, digestible, honest and salient way. Therefore, the aim of this study was to coproduce with patients who have cancer appropriate resources to guide these highly sensit

Acute interstitial nephritis after sequential ipilumumab - nivolumab therapy of metastatic melanoma.

BACKGROUND: The anti-Programmed Death receptor 1 (anti-PD-1) antibodies nivolumab and pembrolizumab are new treatments in metastatic melanoma. Immunotherapies are best known to be responsible for thrombotic microangiopathy. However, immune interstitial nephritis has been described in a patient treated by nivolumab and ipilimumab concomitantly, and three cases of granulomatous interstitial nephritis have been reported with ipilimumab monotherapy. We report herein a case of acute interstitial immune nephritis in a patient treated with nivolumab after ipilimumab for pulmonary metastatic melanoma.

Second primary cancers following anal and cervical carcinoma: evidence of shared etiologic factors.

The authors examined the incidence of second primary cancers occurring after cervical and anal cancer. Data from the Connecticut Tumor Registry for 1935-1988 and eight other US tumor registries for 1973-1988 were used. Women with primary invasive cervical cancer had a relative risk of 4.6 (95% confidence interval (CI) 2.4-8.1) for subsequent invasive anal cancer. Increased relative risks after cervical cancer were also found for cancers of the oral cavity (relative risk (RR) = 2.2), stomach (RR = 1.5), rectum (RR = 1.4), larynx (RR = 3.4), lung (RR = 3.0), vagina (RR = 5.6), bladder (RR = 2.7)

[Diagnostic problems of virus-induced atypical squamous cell proliferations in the area of the anus and genitalia. A case report on Bowenoid papulosis].

Papillomatous proliferations of squamous epithelium of the genital and anal region termed Bowenoid Papulosis (BP) macroscopically resemble condylomata acuminata, whereas histologically they show in part severe dysplasias. Despite the similarity of these lesions to Bowen's disease they are - like condylomas - induced by papilloma viruses and may regress spontaneously. Condyloma-like but predominantly flat proliferations of squamous epithelium of the transitional zone between exocervix and endocervix, which are likewise caused by viruses and have a prognosis similar to that of BP, are even more

Anorectal melanoma: experience from a tertiary cancer care centre in South India.

Introduction Mucosal malignant melanoma of the anorectum is a rare and aggressive disease, in which early diagnosis is difficult. The prognosis remains extremely poor, irrespective of the treatment. We share our experience in treating this malignancy at our centre in South India. Methods This study describes a retrospective analysis of 31 cases of anorectal melanoma presented to our centre between January 2001 and December 2013. Results Twenty-two patients (71%) presented with metastasis and had a median overall survival of nine months. None of the 22 patients survived for two years. Nine pati
