While in most cases, the operation for cataract gives full satisfaction to the patient, it is nevertheless a fact that in some cases--perhaps only a few but certainly not to be neglected--the long term effects of such an operation are not well tolerated. The cause is not faculty performance of the operation, but most often unsuitable operational timing or technics or else defective or illchosen visual aids. These, therefore are the problems dealt with in this paper by the authors who made thorough study of the aphaque eye and its corrective methods.

Prophylaxis of selected retinal breaks is effective in preventing detachment. Most retinal breaks, however, will not cause detachment and can be safely followed without treatment. This paper reviews the evidence for and against prophylactic treatment of commonly encountered retinal breaks in an effort to arrive at typological guidelines for treatment. Education of patients with high risk of detachment is probably of equal value to cryotherapy or photocoagulation in preserving visual acuity.

Methods of intraocular lens (IOL) calculation in non-standard clinical situations have been developed and introduced. These include a formula that improves the calculation accuracy in patients with relatively short anterior-posterior axis and another method for eyes after previous radial keratotomy. A comparative clinical evaluation of the proposed methods has been performed. The results indicate good perspectives for their use in ocular surgery.

BACKGROUND: Posterior capsule opacification (PCO) is the most common complication of lens extraction. Although intraocular lenses (IOLs) are thought to inhibit capsule opacification, the mechanisms by which they do this are poorly understood. The efficacy of epidermal growth factor (EGF) in reducing the healing time of the cornea after an epikeratoplasty has been evaluated in an open study in two groups of patients. The time required for complete reepithelialization of the cornea was recorded, and the data obtained were analyzed statistically. In the EGF group the reepithelialization was significantly faster than in the control group. These results indicate that EGF is effective in reducing the reepithelialization time of the cornea after an epikeratoplasty.

We present the cases of four patients in whom juvenile aphakic glaucoma developed. An excessive loss of hyperopia was the initial clinical sign that alerted us to the diagnosis of glaucoma. At the time of diagnosis, the mean refractive error of the six glaucomatous eyes in the four patients was +4.75 diopters (D) (range, -0.25 to +6.75 D). The mean change in refraction from time of cataract extraction to diagnosis of juvenile aphakic glaucoma was 17.00 D (range, 9.25 to 21.00 D). The author applied several methods of closing the operative wound using an incision at the limbus, corneal incision, an incision with a conjunctival or scleral flap and using various stitches.
