Effects of diabetes mellitus during pregnancy on pregnancy outcomes and growth and development of children aged 0-6 years: a retrospective cohort study.

BACKGROUND: Gestational diabetes mellitus (GDM) and pre-gestational diabetes mellitus (PGDM) are jointly termed as diabetes mellitus during … logical jaundice (OR 7.264, 95% CI 5.923-8.910), neonatal infection (OR 2.876, 95% CI 2.415-3.424), asphyxia neonatorum (OR 2.435, 95% CI 1.802-3.291), macrosomia (OR 1.355, 95% CI 1.217-1.508), and cesarean section (OR 1.207, 95% CI 1.131-1.288) (all p < 0.001). This study statistically analyzed 18, 590 pairs of mothers and children who had completed the national st …

Feasibility and reproducibility of resting-state functional magnetic resonance imaging in asphyxiated neonates.

BACKGROUND: In the ongoing search for prognostic tools in neonates suffering perinatal asphyxia, we assessed the feasibility of resting-state functional magnetic resonance imaging (RS-fMRI). The aim of our study was to determine both the intra-session and inter-subject reproducibility of resting-state networks (RSNs), as well as factors affecting these. NEW METHOD: We conducted a prospective multicenter study in 21 asphyxiated term newborns (mean gestational age = 39.2 weeks, mean 5-minute Apgar score = 3) after treatment with controlled hypothermia at a level III neonatal intensive care unit

Study protocol of the Speech of children After Neonatal Encephalopathy (SANE): a single-centre prospective cohort study.

INTRODUCTION: Language development is a key determinant of academic achievement and psychosocial outcome. Due to the therapeutic hypothermia procedure, 75% of children diagnosed with moderate to severe neonatal encephalopathy (E) survive without serious neurological sequelae. Several studies have already revealed the overall cognitive deficits of these infants; however, their long-term language developmental specificities and their attendant neonatal risk markers remain under-investigated. METHODS AND ANALYSES: In our single-centre prospective cohort study (n≈128), we will include only those

A global cross-sectional survey on neonatal analgosedation: unveiling global trends and challenges through latent class analysis.

PURPOSE: This study aims to analyze global prescribing patterns for analgosedation in neonates during four critical care scenarios. The research explores existing patterns, their association with geographic and sociodemographic index (SDI), and adherence to evidence-based practices. METHODS: Data from a 2024 global survey of 924 responses to 28 questions were analyzed, focusing on four items for their high variability: premedication in intubation (Q17), sedation in preterm (Q19) and full-term newborns (Q23), and perinatal asphyxia (Q26). Latent class analysis (LCA) classified neonatal intensi

Adherence to Perinatal Asphyxia or Sepsis Management Guidelines in Low- and Middle-Income Countries.

IMPORTANCE: Most of the 2.3 million annual neonatal deaths occur in sub-Saharan Africa and South Asia, with perinatal asphyxia and neonatal sepsis being the leading causes of neonatal mortality. Most neonatal deaths are considered preventable through high-quality clinical care, which includes adherence to clinical care guidelines. OBJECTIVE: To assess adherence to World Health Organization clinical care guidelines for management of perinatal asphyxia and neonatal sepsis and to identify patient-level factors in adherence among neonates who died from these conditions. DESIGN, SETTING, AND PART

Inhibition of the HIF-1α Signaling Pathway Promotes Oligodendrocyte Maturation in the In Vitro Model of Perinatal Asphyxia.

Preterm or complicated labor is often accompanied by the asphyxia of the newborn. Temporal limitation of oxygen and nutrient supply can be especially harmful to the developing brain and lead to hypoxic-ischemic encephalopathy. This injury is mainly caused by the reduction in white matter volume and may be triggered by delayed maturation of oligodendrocytes and affected myelination. One of the key factors involved in the pathophysiology of neural tissue after asphyxia is the activation of hypoxia-inducible factor 1α (HIF-1α) signaling. The objective of the study was to verify whether this facto

Hypoxic-ischemic encephalopathy following intrapartum asphyxia: is it avoidable?

BACKGROUND: The proportion of term hypoxic-ischemic encephalopathy arising during intrapartum fetal surveillance remains unclear. Moreover, recent Cochrane review and other studies emphasized the need for research on the impact of admission cardiotocography and highlighted the necessity for a definition of "avoidable perinatal brain injury". OBJECTIVE: To assess the impact of intrapartum asphyxia on neonatal hypoxic-ischemic encephalopathy occurrence and identify the proportion of cases that benefit from preventive measures. STUDY DESIGN: This retrospective 20-year birth cohort study include

Population pharmacokinetics of vancomycin in term neonates with perinatal asphyxia treated with therapeutic hypothermia.

AIMS: Little is known about the population pharmacokinetics (PPK) of vancomycin in neonates with perinatal asphyxia treated with therapeutic hypothermia (TH). We aimed to describe the PPK of vancomycin and propose an initial dosing regimen for the first 48 h of treatment with pharmacokinetic/pharmacodynamic target attainment. METHODS: Neonates with perinatal asphyxia treated with TH were included from birth until Day 6 in a multicentre prospective cohort study. A vancomycin PPK model was constructed using nonlinear mixed-effects modelling. The model was used to evaluate published dosing guide

The value of umbilical artery blood gas analysis in the rapid diagnosis of fetomaternal hemorrhage.

As a rare obstetric disease, fetomaternal hemorrhage (FMH) often causes severe fetal anemia, edema and even death, easily to be confused with severe neonatal asphyxia. Currently, there are several ways to detect or predict FMH, however, most of them are flawed and time-consuming, as well as unsuitable for rapid diagnosis and timely intervention of FMH. To explore the values of umbilical artery blood gas analysis in the rapid diagnosis of FMH, providing basis for rapid guidance of newborn rescue. Five cases of neonates with FMH from the First Affiliated Hospital of Army Military Medical Univers

A systematic review of noninflammatory cerebrospinal fluid biomarkers for clinical outcome in neonates with perinatal hypoxic brain injury that could be biologically significant.

Neonatal encephalopathy (NE) that purportedly arises from hypoxia-ischemia is labeled hypoxic-ischemic encephalopathy (HIE). Perinatal asphyxia is a clinical syndrome involving acidosis, a low Apgar score and the need for resuscitation in the delivery room; asphyxia alerts one to the possibility of NE. In the present systematic review, we focused on the noninflammatory biomarkers in cerebrospinal fluid (CSF) that are involved in the development of possible brain injury in asphyxia or HIE. A literature search in PubMed and EMBASE for case-control studies was conducted and 17 studies were found

Rigor of Neurovascular Coupling (NVC) Assessment in Newborns Using Different Amplitude EEG Algorithms.

Birth asphyxia constitutes a major global public health burden for millions of infants with a critical need for real time physiological biomarkers. This proof of concept study targets the translational rigor of such biomarkers and aims to examine whether the variability in the amplitude-integrated EEG (aEEG) outputs impact the determination of neurovascular coupling (NVC) in newborns with encephalopathy. A convenience sample with neonatal asphyxia were monitored for twenty hours in the first day of life with EEG and near infrared spectroscopy (NIRS)-based cerebral tissue oxygen saturation (Sct
