Supraventricular tachycardia: mechanisms and management.

The phrase paroxysmal supraventricular tachycardia describes a group of arrhythmias with similar electrocardiographic features but different mechanisms that have been clarified in recent years with specialized intracardiac recording and pacing techniques. Reentry accounts for most cases and has been localized to the A-V node and less frequently to the sinus node, the atria themselves, and A-V nodal bypass tracts (Wolff-Parkinson-White syndrome). These forms of supraventricular tachycardia are initiated by premature beats that dissociate conduction between two pathways and permit the establishm

Supraventricular tachycardia in Lown-Ganong-Levine syndrome: atrionodal versus intranodal reentry.

The mechanism of the abbreviated atrioventricular (A-V) nodal conduction time and paroxysmal supraventricular tachycardia in the Lown-Ganong-Levine syndrome was evaluated in six patients. In each the A-H interval increased in response to rapid atrial pacing and atrial extrastimuli; typical dual A-V nodal pathways were demonstrated. In five patients studied at two cycle lengths prolongation of conduction and refractoriness of the "fast" pathway was noted at the shorter basic cycle length. Propranolol prolonged conduction and refractoriness of the "fast" pathway in three patients and in one prod

Pacing delivered rate and rhythm control for atrial fibrillation.

PURPOSE OF REVIEW: Device therapy for atrial fibrillation remains contentious despite the recognized benefit of atrial pacing in sinus node dysfunction. There are various new specialized pacing algorithms that aim to provide rhythm or rate control in atrial fibrillation. We review the various options for device therapy and the evidence available concerning their effectiveness. RECENT FINDINGS: Randomized trials on preventative algorithms for atrial fibrillation have not shown consistent benefit. Anti-tachycardia pacing for atrial fibrillation has inherent problems illustrated in this review a

Atrioventricular Wenckebach point and progression to atrioventricular block in sinoatrial disease.

The value of measurement of the atrioventricular (AV) Wenckebach point at rest as a predictor of progression to AV block was investigated prospectively. Twenty-four patients with sinoatrial disease without evidence of conduction disturbance on 12-lead ECG or 24-hour ambulatory monitoring were paced with Medtronic Activitrax II, Medtronic Legend, or Telectronics Meta MV systems in AAI or AAIR modes. Patients were monitored for symptoms and evidence of AV block on 24-hour tapes. The mean age of the patients was 67 years (range: 42-88). There were 11 males and 13 females. The mean follow-up time

Histopathology of the conduction system in sudden cardiac death.

In order to clarify the importance of the conduction system involvement in cases of sudden cardiac death, 35 autopsied hearts, obtained from the patients who died within one hour after the onset of a critical attack, were examined histopathologically and compared with 27 both age- and disease-matched and 30 only age-matched control hearts from individuals who had not died suddenly. The conduction system was serially sectioned using Lev's method and observed under a light microscope. Purkinje cell lesions, which made a structural maze compatible to the electrophysiological re-entry mechanism, a

Limitation of the surface electrocardiogram in diagnosis of atrial arrhythmias. Further observations on dissimilar atrial rhythms.

Electrophysiologic studies with recordings of multiple intracavitary electrograms were performed in two patients with atrial dysrhythmias. In Case 1 the arrhythmic pattern in the surface electrocardiogram resembled atrial flutter. Electrophysiologic studies revealed the arrhythmia to be paroxysmal left atrial tachycardia, with separation of left and right atrial components of the P wave by an isoelectric period secondary to marked interatrial conduction delay. In Case 2 the surface electrocardiogram indicated paroxysmal atrial tachycardia with block. Electrophysiologic studies revealed right a

Nonpharmacologic strategies for treating atrial fibrillation.

Nonpharmacologic tools to treat atrial fibrillation (AF) are direct current cardioversion, radiofrequency (RF) current catheter ablation, antiarrhythmic surgery, pacing, and atrial defibrillation. In patients with sustained AF, when no cause can be found for AF or when the associated disease is mild, an attempt should be made to restore sinus rhythm. Electrical cardioversion by synchronized direct current shock can be attempted when drugs have failed and is the first choice in acutely ill patients. Virtually all patients should be anticoagulated. Temporary pacing should be available in patient

Electrophysiological properties in patients undergoing atrial compartment operation for chronic atrial fibrillation with mitral valve disease.

AIMS: Surgical treatment for atrial fibrillation is now feasible in selective cases. The aim of this study was to assess the electrophysiological properties of patients undergoing atrial compartment operation for chronic atrial fibrillation. METHODS AND RESULTS: Electrophysiological studies were performed in 20 mitral valve patients with atrial fibrillation who had been maintained in sinus rhythm for more than 1 year after atrial compartment operation. Intra-cardiac recording and programmed electrical stimulation were performed in various atrial compartments. The parameters studied included s

High-resolution mapping of the triangle of Koch: Spatial heterogeneity of fast pathway atrionodal connections.

BACKGROUND: Dedicated mapping studies of the triangle of Koch to characterize retrograde fast pathway activation have not been previously performed using high-resolution, 3-dimensional, multielectrode mapping technology. OBJECTIVE: To delineate the activation pattern and spatial distribution of the retrograde fast pathway within the triangle of Koch during typical atrioventricular nodal reentrant tachycardia (AVNRT) and right ventricular pacing in a consecutive series of patients using the Rhythmia mapping system (Boston Scientific, Natick, MA). METHODS: A total of 18 patients with symptomat

The mechanisms of spontaneous termination of reentrant supraventricular tachycardias.

BACKGROUND: Atrioventricular nodal reentrant tachycardia (AVNRT) and atrioventricular reentrant tachycardia (AVRT) often terminate spontaneously, presumably due to changes in the electrophysiological properties of the reentrant circuit. However, the mechanism of spontaneous termination of these arrhythmias is incompletely understood. METHODS: We included 70 consecutive patients with reentrant supraventricular tachycardias (35 AVNRT, 35 AVRT) in whom the arrhythmia ended spontaneously during the electrophysiologic study. We determined in each patient the duration of the induced arrhythmia, sit

A new method for estimating preexcitation index without extrastimulus technique and its usefulness in determining the mechanism of supraventricular tachycardia.

The preexcitation index has been shown to be useful in determining the mechanism of paroxysmal supraventricular tachycardia (SVT) and the site of the accessory pathway in atrioventricular (AV) reentrant tachycardia. To test whether a preexcitation index could be computed analytically instead of by scanning the whole SVT cycle with extrastimuli, 19 patients with SVT were studied. The new index was computed using the following formula: (AV conduction time during SVT) + (ventriculoatrial conduction time during ventricular pacing at the SVT cycle length) - (SVT cycle length). There was a strong co

Unexpected Case of Cardiac Sarcoidosis in a Caucasian Male.

Cardiac sarcoidosis (CS) is an underappreciated diagnosis in healthy patients presenting with recurrent syncope. This may be particularly limited in patients who do not meet common epidemiology and manifestations of sarcoidosis, which are typically African American women and pulmonary, respectively. In our case, we have a previously healthy middle-aged Caucasian American male who presented with recurrent syncope for one week. Initial electrocardiogram showed a right bundle branch block with a normal P-R interval. A few days into the admission, the patient suffered another episode of syncope pr

[Effect of somatostatin on the conduction system of the heart].

Somatostatin (S), a naturally occurring peptide, was first discovered more than 20 years ago. This peptide was later found in many tissues, including human cardiac tissue. High concentration of S was found in the sinus and atrioventricular (AV) nodes. Negative chronotropic activity of S and depressive effects of this substance on AF junction have also been described. The purpose of the present study was to determine the effect of S on the cardiac conduction system in man during antegrade and retrograde conduction. The study group consisted of 12 pts (5 women and 7 men, mean age 43 years). They

Chronic wireless communication between dual-chamber leadless pacemaker devices.

BACKGROUND: Aveir DR (Abbott, Abbott Park, IL) is a dual-chamber leadless pacemaker (LP) system with distinct atrial and ventricular LPs (ALP, VLP) that communicate wirelessly to deliver atrioventricular synchronous pacing. Success rates of these implant-to-implant (i2i) transmissions have not been systematically evaluated. OBJECTIVE: This study aims to evaluate chronic i2i success rates in a clinical setting. METHODS: Patients meeting standard dual-chamber pacing indications were enrolled and implanted with dual-chamber LP systems as part of a prospective international clinical trial (Aveir

Complete Heart Block in a Patient Undergoing Combination Immune Checkpoint Inhibitor Therapy.

Combination immune checkpoint inhibitor (ICI) therapy is an emerging chemotherapy strategy for patients with solid tumor malignancies. Cardiotoxicity is a rare adverse effect of ICI therapy, most commonly presenting as acute myocarditis and, less frequently, as significant conduction abnormalities. We present a unique case of a 68-year-old female with urothelial cancer who developed shortness of breath and chest pain one week after receiving combination ICI therapy with ipilimumab and nivolumab. Biomarkers were elevated, including high-sensitivity troponin to 14,000 ng/L and creatine phosphoki

Bradyarrhythmias in Repaired Atrioventricular Septal Defects: Single-Center Experience Based on 34 Years of Follow-Up of 522 Patients.

Atrioventricular Septal Defect (AVSD) is a rare congenital heart defect (CHD) often associated with genetic syndromes, most commonly Down syndrome (DS). Over the last four decades, surgical repair has increased survival and improved quality of life in these patients. The prevalence of bradyarrhythmias namely, atrioventricular block (AVB) and sinus node dysfunction (SND) in AVSD is partially known. 522 cases with both partial and complete AVSD (38.7% with DS), undergoing intracardiac repair from 1982 to 2016 at our institution, were reviewed from our system database. 38 (7.3%) patients received
