Chronic diseases, depressive symptoms and functional limitation amongst older people in rural Malaysia, a middle income developing country.

OBJECTIVES: To determine prevalence and prevalence ratio of functional limitation amongst older people with combined chronic diseases and co-morbid depressive symptoms compared with older people with either chronic disease or depressive symptoms alone. METHODS: Data were analysed from a cross-sectional study of 765 people aged 60 years and over, conducted from 2007 to 2008 in Malaysia. Chronic diseases were self-reported, depressive symptoms were measured using the Geriatric Depression Scale and functional limitation was assessed using the Tinetti Performance Oriented Mobility Assessment Tool

Deep hypothermic total circulatory arrest for internal carotid artery aneurysm extending into the cranium: experience from a developing country.

Cardiopulmonary bypass is commonplace for acquired and congenital cardiac procedures. It has also stretched to facilitate complicated non-cardiac operations. Carotid artery aneurysms are treated both with surgical repair without cardiopulmonary bypass (CPB) and, occasionally, by utilizing CPB perfusion techniques. We have successfully repaired an internal carotid artery aneurysm, extending into cranium in a 30-year-old woman, by establishing deep hypothermic circulatory arrest on cardiopulmonary bypass.

Cutaneous phenotype and MC1R variants as modifying factors for the development of melanoma in CDKN2A G101W mutation carriers from 4 countries.

The G101W founder mutation is the most common CDKN2A mutation in Italy, Spain, and France. As the background of modifying genes, environmental exposures, and sun behavior vary across countries, studying G101W carriers from distinct countries offers a unique opportunity to evaluate possible modifying factors in melanoma development. We evaluated 76 G101W cases and 59 carrier controls from France, Italy, Spain, and the United States. Hair color and dysplastic nevi distributions differed significantly in cases and controls across the 4 study groups. Cases also varied significantly for eye color,

Cross-country comparisons suggest shared risk factors for carcinomas, including male lung adenocarcinoma and colon cancer development.

Smoking is clearly the major risk factor for both squamous cell carcinomas (SCCs) and adenocarcinomas (ACs) of the lung, although less so for the latter, where other influences appear to be important. In order to determine whether cross-country comparisons might provide evidence of shared risk factors with cancers in other organs of males, an examination of IARC/WHO data for cancer incidence was made for countries/registries in Europe, North America, Australia and Asia. Significant simple correlations, which persisted on partial analysis, were observed between lung SCCs and tumors of the laryn

Serum anti-gliadin antibody profile in childhood protracted diarrhoea due to coeliac disease and other causes in a developing country.

Serum anti-gliadin antibody (AGA) titres were estimated by diffusion in a gel enzyme-linked immunosorbent assay in children with coeliac disease (n = 11), protracted diarrhoea of non-coeliac causes (n = 110), acute gastroenteritis (n = 20), protein energy malnutrition (n = 20), and asymptomatic, well-nourished children (n = 66). The mean IgG and IgA AGA titres were significantly higher (p less than 0.001) in children with coeliac disease than in any other groups. There was no significant difference (p greater than 0.01) in AGA titres in relation to age, nutritional status, or severity of villo

Good clinical laboratory practice (GCLP): the need for a hybrid of good laboratory practice and good clinical practice guidelines/standards for medical testing laboratories conducting clinical trials in developing countries.

There has been a significant increase in the number of clinical drug trials (particularly phase III) being conducted in developing countries for infectious diseases such as HIV, malaria, and tuberculosis. Laboratory results provided by medical testing laboratories in the region are critical to ensuring the safety of patients and the generation of good quality data. A number of well accepted Good Clinical Practice (GCP) and Good Laboratory Practice (GLP) guidelines govern the conduct of clinical trials internationally. Good Clinical Practice guidelines remain too vague with respect to sample an…

[Ilisarov callus distraction and callus compression in treatment of defect gunshot fractures, pseudarthroses and post-traumatic deformities in developing countries. A report of experiences in Cambodian and Ethiopian provincial hospitals].

Between March 1989 and May 1993, a total of 22 patients were treated by Ilisarov's method of callus distraction and compression in provincial hospitals in Cambodia and Ethiopia. In both countries were times of civil war and unrest. The indications were high-velocity gunshot and explosive injuries with defect fractures (9 patients), infected and non-infected non-unions after gunshot injuries (6 patients) and civil accidents (3 patients), and deformities after civil accidents and gunshot injuries (4 patients). The deformities were leg shortening by between 5 and 7 cm and in 1 case ankylosis of t

Utility of QuantiFERON-TB assay for prediction of tuberculosis development in kidney transplant patients in an intermediate-tuberculosis-burden country: lack of evidence for enhanced prediction for short-term tuberculosis development.

INTRODUCTION: Although a latent tuberculosis (TB) infection is a risk factor for active TB, the diagnosis of latent TB infection is difficult in end-stage renal disease patients. PATIENTS AND METHODS: We retrospectively compared the results of the QuantiFERON-TB (QFT) test and the tuberculin skin test in patients on the waiting list for kidney transplantation (KT), and investigated whether the QFT test can predict TB development in KT recipients in an intermediate-TB-burden country. RESULTS: The incidence of post-KT TB was 283 cases/100,000 patient-years among 1274 KT recipients at the Seoul

A machine learning model for prediction of early-onset neonatal sepsis in low-income and middle-income countries: development and validation study.

OBJECTIVE: Early-onset sepsis (EOS), which occurs within the first 72 hours of life, can often be fatal for neonates. Machine learning (ML) models demonstrate promise for timely diagnosis. However, current ML models primarily rely on data from high-income countries, which reduces their applicability to low-income and middle-income countries (LMICs) that have a higher burden and different disease profiles. We developed an ML model for the timely prediction of culture-proven EOS in LMICs. METHODS: We conducted a secondary analysis of the Delhi Neonatal Infection Study (2011-2014) carried out in

Expert consensus on pre-eclampsia risk screening tools for low- and middle-income countries: Development of a new Target Product Profile.

Risk screening tools can identify women at high-risk for developing pre-eclampsia, allowing for timely preventative care and monitoring. However, tools that are accurate, feasible and affordable for limited-resource settings are lacking. Target Product Profiles (TPPs) define the minimum and optimal criteria required for new products to achieve a specific health need. Through the Accelerating Innovation for Mothers (AIM) project, we developed the first TPP for pre-eclampsia risk screening tools, suitable for use in low- and middle-income countries (LMICs). We used a mixed-methods approach, incl

Financing the transition: a roadmap for sustaining disease control in low- and middle-income countries after the 2025 contraction in development assistance for health.

In 2025, development assistance for health fell by about 21% in a single year, driven largely by a 67% reduction in United States financing, disrupting HIV, tuberculosis, malaria, immunization and maternal health programs across low- and middle-income countries. Modelling indicates millions of avoidable deaths by 2030 if the resulting gap is not filled, yet most recipient governments have not reprioritized budgets or raised domestic revenue in response. This brief argues that the contraction is structural rather than cyclical and sets out a sequenced transition-financing roadmap. It proposes p
