Alveolar Air and O2 Uptake During Exercise in Patients With Heart Failure.

BACKGROUND: Peak exercise pulmonary oxygen uptake (V̇O2) is a primary marker of prognosis in heart failure (HF). The pathophysiology of impaired peak V̇O2 is unclear in patients. To what extent alveolar airway function affects V̇O2 during cardiopulmonary exercise testing (CPET) has not been fully elucidated. This study aimed to describe how changes in alveolar ventilation (V̇A), volume (VA), and related parameters couple with exercise V̇O2 in HF. METHODS AND RESULTS: A total of 35 patients with HF (left ventricular ejection fraction 20 ± 6%, age 53 ± 7 y) participated in CPET with breath-to-b

Exercise capacity is not decreased in children who have undergone lung resection early in life for congenital thoracic malformations compared to healthy age-matched children.

PURPOSE: The purpose of this study was to compare (i) the exercise capacity and (ii) lung function prior to and immediately post cardiopulmonary exercise tests (CPET) of children who underwent early life lung resection for Congenital Pulmonary Airway Malformations (CPAM) to healthy control children. METHOD: Eight children with CPAM (four males, age 9.6 ± 1.8 years) and eight control children without respiratory disease (three males, age 9.4 ± 1.4 years) performed a CPET on a cycle ergometer, during which maximal oxygen consumption (V̇O2max ) and heart rate were measured. Prior to and immediat

Eccentric exercise in ischemic cardiac patients and functional capacity: A systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Eccentric (ECC) exercise is an "economical" type of exercise with low energy requirements and does not cause early fatigue. Therefore, it is used for cardiac patients, who have low physical activity and exercise intolerance, as an easier kind of training. OBJECTIVE: This systematic review aimed to investigate the efficacy of ECC exercise for functional capacity (FC) in patients with ischemic heart disease. DESIGN: Systematic review. METHODS: MEDLINE via PubMed and EBSCO databases were searched for articles of randomized controlled trials of adults with ischemic heart disease who

Small, unrepaired ventricular septal defects reveal poor exercise capacity compared with healthy peers: A prospective, cohort study.

BACKGROUND: Small ventricular septal defects (VSDs) are considered to be without hemodynamic influence and most remain unrepaired. However, studies recently described late cardiac adverse consequences that could potentially affect functional capacity. Yet, this has never been assessed in adulthood. Therefore, the aim was to determine peak exercise capacity in adults with small VSDs compared with healthy, matched controls. METHODS: In a prospective, cohort study we included patients with unrepaired VSDs and healthy controls, (age 18 to 40years). Functional capacity was determined through incre

Myopathic causes of exercise intolerance with rhabdomyolysis.

We review the muscular dystrophies and metabolic myopathies associated with myalgia and rhabdomyolysis together with some less well-recognized associations based upon the personal practice of the authors. A careful history and clinical examination will direct investigation towards an accurate molecular diagnosis. Non-specific exercise-induced myalgia in the presence of muscle hypertrophy and a high creatine kinase will point towards a muscular dystrophy. Symptoms occurring within minutes of exercise and with isometric contraction, especially with a history of a 'second wind' phenomenon, sugges

A non-ischemic forearm exercise test for the screening of patients with exercise intolerance.

BACKGROUND: The forearm exercise test is a common investigation that allows detection of some metabolic myopathies. It is not completely standardized and, when performed in ischemic conditions, may induce rhabdomyolysis in patients with glycogenosis. OBJECTIVE: To develop a standardized non-ischemic exercise test for a safe screening of patients with exercise intolerance. METHODS: Twenty-six healthy subjects and 32 patients with exercise intolerance performed an isometric exercise at 70% of the maximal voluntary contraction during 30 seconds in non-ischemic conditions. Blood concentrations o

Development of a hockey-specific, skate-treadmill VO2 max protocol.

The purpose of this study was to investigate a protocol for the determination of VO2 max utilizing a motor-driven skate treadmill (ST). On separate days, 6 male hockey players completed a ST and a cycle ergometer (BK) VO2 max protocol. The results showed no significant difference between the ST and BK protocols for relative (60.4 +/- 5.09 vs. 59.0 +/- 8.31 ml.kg-1.min-1) and absolute VO2 max values (4.51 +/- 0.50 vs. 4.39 +/- 0.59 L.min-1), respectively. Significantly higher HR max was recorded during the ST protocol (202.3 +/- 4.27 vs. 200.7 +/- 4.55 b.min-1) (p < 0.05). Peak VE and VT were n

McArdle's syndrome: the reaction to a fat-rich diet.

The effect of a diet enriched with fats on the muscle performance of a patient with McArdle's syndrome was studied. The tolerance to physical activity was studied during exercise (ergometric examination) and by sustained abduction to 90 degrees of the deltoid muscle, both after a three-day period on a normal diet and after a three-day period on a fat-rich diet. After the first period a woody spasm of the deltoid muscle was found which lasted several days. After a period on fat-rich diet the patient's physical fitness was increased and the recovery period after the acute load was shorter. Moreo

Validation and prognostic value of the five-repetition sit-to-stand test in patients with severe exacerbation of COPD.

BACKGROUND: A severe exacerbation of COPD (ECOPD) may negatively impact patients' functioning; however, there is a lack of validated, simple, and low-cost tools to assess physical function in this setting. Thus, the aims of this study were to explore the construct validity of the five-repetition sit-to-stand (5STS) test for assessing muscle strength and exercise tolerance, and to investigate its prognostic value in predicting exacerbation-related readmission risk in patients with severe ECOPD. METHODS: This was a prospective observational study. Fifty-eight patients hospitalized with a severe…

Delaying task failure in high-intensity exercise: a Pi-afferent-effort framework for targeted sports nutrition.

BACKGROUND: Task failure during high-intensity exercise may reflect the convergence of intramuscular, neural, perceptual, and behavioral boundaries rather than the consequence of a single fatigue mechanism. METHODS: This hypothesis-generating narrative review proposes the Pi-afferent-effort model of task failure during high-intensity exercise and applies the model to targeted performance nutrition. RESULTS: Rapid ATP turnover increases inorganic phosphate (Pi), while phosphocreatine breakdown through the creatine kinase reaction buffers ATP availability. As phosphate-linked disturbance progr

Neutrophil extracellular traps and peptidylarginine deiminase 4-mediated inflammasome activation link diabetes to cardiorenal injury and heart failure.

BACKGROUND AND AIMS: Diabetes is associated with increased risk of cardiovascular and renal disease. This study investigated the role of peptidylarginine deiminase 4 (PAD4), neutrophil extracellular traps (NETs), and inflammasome activation in diabetic cardiomyopathy (DCM) and kidney disease (DKD). METHODS: Endomyocardial biopsies (EMB) from heart failure (HF) patients (n = 20) with or without diabetes were stained for NETs. Wild-type (WT) and PAD4⁻/⁻ mice were subjected to streptozotocin (STZ)-induced diabetes and cardiac function, blood glucose, body weight, and exercise tolerance were asse…

Effects of Radiofrequency Catheter Ablation on Cardiac Reserve Using Preload Stress Echocardiography in Paroxysmal and Persistent Atrial Fibrillation.

The effects of catheter ablation on exercise tolerance and quality of life in patients with atrial fibrillation (AF) have been reported. We assessed cardiac function in more detail using the leg positive pressure (LPP) technique and found that contractile reserve is particularly important in relation to exercise tolerance and prognosis. In this study, we used the LPP technique to examine changes in contractile reserve immediately after ablation and 6 months later. We prospectively enrolled patients who underwent catheter ablation for AF at 2 institutes. We performed LPP stress echocardiography…

Effects of downhill walking in pulmonary rehabilitation for patients with COPD: a randomised controlled trial.

The development of contractile muscle fatigue (CMF) affects training responses in patients with chronic obstructive pulmonary disease (COPD). Downhill walking induces CMF with lower dyspnoea and fatigue than level walking. This study compared the effect of pulmonary rehabilitation (PR) comprising downhill walking training (DT) to PR comprising level walking (conventional training (CT)) in patients with COPD.In this randomised controlled trial, 35 patients (62±8 years; forced expiratory volume in 1 s (FEV1) 50±17% predicted) were randomised to DT or CT. Exercise tolerance (6-minute walk test di…
