Mechanically assisted bidirectional cavopulmonary shunt in neonates and infants: An acute human pilot study.

OBJECTIVE: Poor survival following surgical palliation for hypoplastic left heart syndrome (HLHS) raises the question of the need for a paradigm shift. This is the first human study to investigate the possibility of primary "in-series" palliation in neonates and infants with HLHS in an acute setting with the aid of 2 types of mechanical assist: superior vena cava (SVC)-to-pulmonary artery (PA) pump assist and SVC-to-right atrium (RA) oxygenation assist. METHODS: By rearranging the cannula sites and flow rates for modified ultrafiltration, 2 types of mechanically assisted bidirectional cavopul

Aerobic exercise influences quality of life of children and youngsters with congenital heart disease: a randomized controlled trial.

PURPOSE: To evaluate effects of an exercise program on health-related quality of life (HRQoL) in children and adolescents with tetralogy of Fallot (ToF) or a Fontan circulation. METHODS: Stratified, randomized, controlled intervention study conducted in five participating centers of pediatric cardiology in The Netherlands. In total, 93 patients, aged 10-25 years, with surgical repair for tetralogy of Fallot or with a Fontan circulation for single-ventricle physiology were included. They were randomly allocated with a ratio of 2:1 to: (1) a 12-week period with an exercise program for 3 times p

Customized covered stent graft for percutaneous closure of Fontan baffle leak.

We report the innovation of both a partly-covered and completely-covered, variable-diameter, balloon-expandable stent that was custom-designed by NuMed, Inc for percutaneous closure of a baffle leak after total caval pulmonary connection (TCPC). A 50-year-old patient, born with tricuspid atresia, who had undergone TCPC, developed severe persistent cyanosis due to a right-to-left shunt through a TCPC baffle leak. Re-operation was deemed too high risk. Therefore, considering his complex anatomy, a custom-made, partly-covered, tapered, balloon-expandable stent was designed and successfully deploy

Changing epidemiology and mortality in adult congenital heart disease: looking into the future.

Advances in surgical and medical treatment of children born with congenital heart disease have led to a growing number of adult survivors, particularly to a growing number of adults with complex congenital heart disease. Childhood mortality has continuously decreased over the last few decades and mortality has shifted almost entirely to adulthood. However, most patients are not cured and many remain at risk of premature death. The extent of excess mortality among individual congenital disease entities is not well defined. In this article we outline the current demographics of adults with selec

Pathophysiology and management of heart failure in repaired congenital heart disease.

Up to 20% of all postoperative patients with repaired congenital heart disease later develop symptoms of heart failure, but slow disease progression makes patients difficult to study. The incidence is higher in the presence of lesions with a solitary or systemic right ventricle (RV). Several mechanisms cause the development of heart failure in this population, and the treatment options are not always aligned to conventional therapy for heart failure in a biventricular heart with cardiomyopathy. The pathogenesis and options for management of this complex problem in specific high-risk lesions ar

Device occlusion of Fontan fenestration--an economical alternative.

Fenestrating the Fontan circuit during total cavopulmonary anastomosis is commonly performed to reduce postoperative mortality and morbidity. The resulting systemic desaturation may progressively increase leading to symptoms. We report the case of a symptomatic eight year old, whose Fontan circuit fenestration was closed using a patent ductus arteriosus occluder. The marked improvement in the patient's clinical status immediately and after 3 months confirms this device to be a safe, and economically better alternative to the atrial septal defect occluder.

Impact of central hypercapnic chemosensitivity on enhanced ventilation in patients after the Fontan operation.

BACKGROUND: Central hypercapnic chemosensitivity (Chemo) influences the enhanced ventilatory and sympathetic responses in heart failure patients; however, its influence on these responses in Fontan patients is unknown. OBJECTIVES: To measure Chemo and compare the results with rest and exercise ventilatory characteristics in Fontan patients. METHODS AND RESULTS: We measured Chemo (l/min/mmHg), hemodynamics, pulmonary function, cardiac autonomic nervous and neurohumoral activities and compared the results with the ventilatory response during exercise in 42 Fontan patients and 12 referents. New technique for fenestration of the interatrial septum.

Achieving controlled flow between the systemic and pulmonary venous circulations is desirable in many complex congenital heart diseases. This includes the Fontan circulation, primary pulmonary hypertension, double inlet ventricles, or hypoplastic left heart with obstruction to the atrioventricular valve. As no specific device is available for this purpose, we developed a balloon-mounted stent technique to achieve a predetermined-sized fenestration of an atrial baffle in a patient with Fontan circulation. The details of the technique are described.

Tissue Doppler echocardiography before and after the surgical reconstruction of an insufficient mitral valve in a patient with Fontan circulation.

We report a patient with tricuspid atresia after total cavopulmonary connection who developed significant mitral valve regurgitation. Doppler myocardial echocardiography tracings showed "pseudonormalization" of diastolic velocities. After successful surgical reconstruction of the mitral valve, the diastolic myocardial velocities changed to the typical Fontan physiology with predominant late diastolic ventricular filling.

Redirection of the hepatic venous flow for the treatment of pulmonary arteriovenous malformations after Fontan operation.

A 45-month-old boy who had complex cardiac anomalies with interrupted inferior vena cava and polysplenia underwent extracardiac Fontan operation. He redeveloped deep cyanosis postoperatively. Cardiac catheterization showed a preferential flow of the hepatic venous blood to the right lung and arteriovenous malformations (PAVMs) in the left lung. He underwent revision of the conduit to attain balanced hepatic venous drainage with subsequent regression of the PAVMs and disappearance of cyanosis. Our experience shows that deficiency of hepatic venous flow played a crucial role in the development o

Acute myocardial infarction as a cause of death in palliated hypoplastic left heart syndrome.

A 20 month old child with hypoplastic left heart syndrome died suddenly from a massive myocardial infarction 15 months after a hemi-Fontan operation. This was confirmed at postmortem examination and histological examinations. The sites of surgical reconstruction were all in good condition, there were no gross anatomical coronary abnormalities, and the coronary ostia were unobstructed. On microscopy the internal coronary arteries had notable intimal and medial thickening with narrowing of the lumen, although no thrombotic occlusion was seen. To the authors' knowledge, this is the first publishe

Balloon-expandable stenting, percutaneous coil embolization, and amplatzer septal occluder; treatments after complicated lateral tunnel Fontan: a case report.

The authors report a 7-year-old girl with univentricular heart physiology who developed prolonged pleural effusion due to discrete narrowing of the proximal right pulmonary artery, and progressive cyanosis which resulted from leakage of the atrial baffle, multiple veno-venous collaterals after the lateral tunnel Fontan operation. Percutaneous balloon-expandable stent implantation was used to correct the right pulmonary artery stenosis with an excellent result. Cyanosis was improved by coil embolization of the collaterals and occlusion of the baffle leakage with Amplatzer septal occluder. This

[A successful palliative arterial switch operation with arch repair for tricuspid atresia with ventriculoarterial discordance, subaortic stenosis, coarctation, and aortic arch hypoplasia].

A successful palliative arterial switch operation with arch repair in a 30-day-old infant with tricuspid atresia, ventriculoarterial discordance, subaortic stenosis, coarctation of the aorta, and aortic arch hypoplasia is reported. A concomitant Blalock-Taussig shunt was required because of insufficient pulmonary blood flow through the restrictive bulboventricular foramen. Postoperative arterial oxygen saturation was maintained around 90% and the pressure gradient between the left ventricle and the pulmonary artery was 45 mmHg according to a Doppler echocardiogram. The infant becomes free of s

Five-year clinical and echocardiographic evaluation of the Das AngelWings atrial septal occluder.

BACKGROUND: The late outcome of patients treated with atrial septal occluder devices remains incompletely defined. The purpose of this study was to assess the late outcome (range 4-7 years postprocedure) of patients in whom the Das AngelWings septal occluder device was implanted in the atrial septum. We report the clinical and echocardiographic outcome, at an average of 5 years following the procedure, of patients treated with the Das AngelWings device used to close either a secundum atrial septal defect (ASD) or a patent foramen ovale (PFO). METHODS: Thirty-two patients underwent successful

Impact of Fenestration Patency on Long-Term Fontan Outcomes.

OBJECTIVES: Routine creation of a fenestration during Fontan completion is our policy to stabilize early circulation. This study evaluated its validity by examining the impact of fenestration patency at 1 year on long-term outcomes. METHODS: Among 112 patients who underwent fenestrated total cavopulmonary connection at 4 institutions, 105 with 1-year catheterization and ≥3 years of follow-up were analysed. Patients were classified by fenestration status at 1 year: patent (Group F, n = 43) and closed (Group C, n = 62). Preoperative and 1-year catheter data and long-term complications-Fontan-as

Genomic and transcriptomic profiling of hepatocellular carcinoma in patients with Fontan-associated liver disease.

BACKGROUND AND AIMS: Fontan-associated liver disease (FALD) could lead to liver cirrhosis and hepatocellular carcinoma (HCC). The prognosis of FALD patients is worse when HCC develops. Therefore, a better understanding of the mechanisms of FALD hepatocarcinogenesis is critical. We investigated the genomic and transcriptomic signatures of FALD HCC. APPROACH AND RESULTS: Ten patients with FALD HCCs, 2 patients with non-Fontan congenital heart surgery (CHS) HCC, and 1 patient with FALD focal nodular hyperplasia (FNH) were enrolled. Whole-genome sequencing and transcriptome sequencing were perfor

Mac-2 binding protein glycosylation isomer contributes to identify advanced Fontan-associated liver disease: A retrospective cohort study.

The usefulness of several noninvasive assessments for evaluating liver fibrosis has been reported in Fontan-associated liver disease (FALD). However, no studies have reported on the usefulness of Mac-2 binding protein glycosylation isomer (M2BPGi) in FALD. Here, we investigated the usefulness of M2BPGi in identifying advanced FALD. This retrospective study included patients diagnosed with FALD. Based on the clinical signs of portal hypertension, we defined advanced FALD as a varices, ascites, splenomegaly, and thrombocytopenia (VAST) score ≥ 2, and we compared the cardiac parameters, liver par
