Severe mental retardation, cataracts, short stature, and primary hypogonadism in two brothers.

Two severely mentally retarded brothers are described who had a similar facial appearance, cataracts, short stature, minor digital abnormalities, and primary hypogonadism. Their parents were first cousins. Numerous laboratory investigations failed to elucidate a basic metabolic cause for their disorder.

Crossed asymmetry in Russell-Silver syndrome.

Since the initial report by Silver et al (1953), more than 50 examples of the Russell-Silver syndrome have been reported. Unilateral congenital asymmetry of the extremities has been considered one of the major features of this disorder (Silver, 1964). We recently observed a child with otherwise typical features of the Russell-Silver syndrome who had enlargement of the right hand and of the left lower extremity. We know of no other recorded example of crossed asymmetry in this clinical entity.

Intrauterine growth restriction--diagnosis and management.

BACKGROUND: Fetal growth disorders are an important cause of perinatal morbidity and mortality with long term health implications for the survivors of intrauterine growth restriction (IUGR). The accurate assessment of fetal growth during pregnancy is difficult, but recent advances have improved this important aspect of obstetric care with positive implications for antenatal patients and their babies. OBJECTIVE: This article provides an overview of the detection of fetal growth problems in pregnancy, the determination of the likely cause, and the antenatal and intrapartum care of women with pr…

Admitting powerlessness empowers recovery from substance use disorder: a conceptual model.

Some individuals use substances to cope with their post-traumatic stress disorder symptoms. The emotional, behavioral, and spiritual experience of both their post-traumatic stress disorder and substance use disorder is manifested in a sense of powerlessness, which is caused by the inability to stop using despite negative consequences related to continued use. Thus, powerlessness is a prominent factor in recovery from substance use disorder. The aims of the present theoretical article were to (a) present the consequences of the co-morbid post-traumatic stress disorder and substance use disorder

(R)-ketamine as prophylactic and therapeutic drug for neurological disorders: Beyond depression.

Neurological disorders are the leading cause of disability and the second leading cause of death worldwide. The increasing social and economic burdens of neurological disorders are driven by global population growth and aging. Depression is a common psychiatric symptom in numerous neurological disorders. It is also a risk factor for Alzheimer's disease (AD) and other dementias, Parkinson's disease (PD), and stroke. The rapid-acting and sustained antidepressant actions of (R,S)-ketamine for severe depression was accidentally discovered. Interestingly, (R)-ketamine has greater potency and longer

Bone health in patients with inborn errors of metabolism.

Inborn errors of metabolism encompass a wide spectrum of disorders, frequently affecting bone. The most important metabolic disorders that primarily influence calcium or phosphate balance, resulting in skeletal pathology, are hypophosphatemic rickets and hypophosphatasia. Conditions involving bone marrow or affecting skeletal growth and development are mainly the lysosomal storage disorders, in particular the mucopolysaccharidoses. In these disorders skeletal abnormalities are often the presenting symptom and early recognition and intervention improves outcome in many of these diseases. Many d

Extensive middle cranial fossa arachnoid cysts and different clinical presentation in two patients.

Arachnoid cysts (ACs), particularly suprasellar cysts, cause a wide spectrum of endocrine disorders. Herein, we report two patients diagnosed with an extensive AC in the middle cranial fossa while being investigated for etiologies of precocious puberty and short stature. One of them required surgery due to his pubertal disorders associated with compression effects of cyst. After surgery, his puberty progression was regressed within one year. On the other hand, surgery was not planned for the second patient considering of his cranial imaging findings and the extremely low incidence of growth ho

How fibroblast growth factor 23 works.

There is a discontinuum of hereditary and acquired disorders of phosphate homeostasis that are caused by either high or low circulating levels of the novel phosphaturic hormone fibroblastic growth factor 23 (FGF23). Disorders that are caused by high circulating levels of FGF23 are characterized by hypophosphatemia, decreased production of 1,25-dihydroxyvitamin D, and rickets/osteomalacia. On the other end of the spectrum are disorders that are caused by low circulating levels of FGF23, which are characterized by hyperphosphatemia, elevated production of 1,25-dihydroxyvitamin D, soft tissue cal

[Extragastric manifestations of H. pylori infection].

Recently, H. pylori has been associated with several extradigestive diseases such as cardio-vascular, cutaneous, autoimmune, allergic, liver, hematologic disorders, diabetes mellitus, pediatric diseases (sudden infant death syndrome, growth retardation), extragastric MALT-type lymphomas and even colorectal cancer. The potential role of H. pylori infection in the pathogenesis of these extradigestive disorders has been based on the following facts: 1) local gastric infection with H. pylori may exert systemic effects; 2) during chronic H. pylori induced gastritis several different inflammatory me

Idiopathic central precocious puberty in a Nigerian boy.

A case of idiopathic central precocious puberty, a rare condition is reported in a 2 1/2 year-old Nigerian boy. He presented with progressive genital growth, a growth spurt, advanced skeletal maturation, and inappropriately high serum concentrations of pituitary and gonadal sex homones for his age. There was no family history of precocious puberty. Physical examination and investigations revealed no identifiable cause for his precocity. Despite the overwhelming problems of malnutrition and infectious diseases in our environment, clinicians must have a high index of suspicion for endocrine diso

Hyperthecosis of the ovaries in acromegaly.

Clinical and endocrine features of a patient with ovarian hyperthecosis and a growth hormone-secreting pituitary tumor are reported. A 36-year-old nulliparous woman had a history of severe hirsutism and virilization of long duration. Ovarian catheterization studies at surgery confirmed an ovarian source of excessive androgen production. Ovarian pathology revealed stromal hyperthecosis. Growth hormone levels were elevated and could not be suppressed with glucose ingestion; a pneumoencephalogram revealed the presence of a pituitary tumor. The possible cause-and-effect relationship of these 2 rel

Failure to thrive associated with renal disease.

Failure to thrive is a clinical syndrome usually associated with severely reduced increments in height and weight. It is commonly present in patients with chronic renal failure and occurs occasionally in patients with recurrent urinary-tract infections or tubular disorders. Renal osteodystrophy, protein-calorie malnutrition, and chronic glucocorticoid administration have been shown to be important causes of growth failure in patients with renal disease. Altered hormone production, metabolism, or peripheral utilization and acidosis may be causally related to growth failure. Such factors as anem

Neurological teratogenic effects of antiepileptic drugs during pregnancy.

Epilepsy is one of the few neurologic disorders that requires a constant treatment during pregnancy. Epilepsy affects 0.3-0.8% of pregnant women. Prescription of antiepileptic drugs (AEDs) to pregnant women with epilepsy requires monitoring and maintaining a balance between limiting seizures and decreasing fetal exposure to the potential teratogenic effects. AEDs are also commonly used for psychiatric disorders, pain disorders, and migraines. The types of malformations that can result in fetuses exposed to AEDs include minor anomalies, major congenital malformations, intrauterine growth retard

Rare Causes and Differential Diagnosis in Patients With Silver-Russell Syndrome.

Silver-Russell syndrome (SRS) is an imprinting disorder mainly characterized by pre- and postnatal growth restriction. Most SRS cases are due to 11p15.5 loss of methylation (11p15.5 LOM) or maternal uniparental disomy of chromosome 7 [UPD(7)mat], but several patients remain molecularly undiagnosed. This study describes the molecular investigation of children with a clinical diagnosis or suspicion of SRS at a tertiary center specialized in growth disorders. Thirty-nine patients were evaluated with multiplex ligation-dependent probe amplification, chromosomal microarray and/or massively parallel…
