Glenohumeral relationships: subchondral mineralization patterns, thickness of cartilage, and radii of curvature.

Subchondral mineralization represents the loading history of a joint and can be measured in vivo using computed tomography osteoabsorptiomet … eralization patterns exist in joints consisting of more flat glenoids compared to the corresponding humeral head. Monocentric distribution with a central maximum was found in specimens with glenoids being more curved, indicating higher degrees of congruence, which might represent an early stage of degenerative disease. The obtained information might also be im …

Traumatic anterior shoulder instability in the athlete.

Anterior glenohumeral dislocation is common among athletes and may progress to recurrent instability. The pathoanatomy of instability and sp … rimary dislocation, patients with recurrent instability often have coexistent osseous injury to the humeral head and glenoid. In patients without significant bone loss, open soft-tissue stabilizations have long been considered the 'gold standard treatment' for recurrent instability, but with advances in technology, arthroscopic procedures have gained popularit …

Patterns of fractures in accidental and non-accidental injury in children: a comparative study.

The incidence and pattern of fractures in children who had been abused were compared with those of fractures sustained by children of similar ages in whom abuse had been excluded. From 1976 to 1982 there were 35 children with fractures resulting from child abuse, and all were aged under 5. Of the 826 children in the control group, seen from January to June 1981, 85% were aged over 5. Abused children were much more likely to have multiple fractures (p less than 0.001) and bruising of the head and neck (p less than 0.001). Fractures of the ribs were common in children who had been abused, and th

Arthrography in acute shoulder dislocations.

Arthrography of the shoulder was performed on 50 patients with acute traumatic shoulder dislocations. The mean interval between injury and a … ed intact shoulder capsules, even after an acute initial joint dislocation (58%), suggests that the humeral head does not routinely rupture the capsule during dislocation, but rather tears the glenoid labrum at its bony attachment and dislocates subperiosteally, dissecting a false pouch below the periosteum and under the subscapularis.

[Open post-traumatic anterior luxation of the hip in children. Apropos of a case and review of the literature].

INTRODUCTION: Anterior open traumatic dislocation of the hip in children is rare. MATERIALS AND METHODS: We report the case of a child aged 9 years, who presented following a motor vehicle accident an anterior open dislocation of the left hip associated with a fracture separation of the trochanter, a fracture of the homolateral humerus and a fracture of the contralateral femur. He was treated by open reduction of the dislocation and pinning of the various fractures. RESULTS: After 15 months functional result of the hip was good. The radiograms showed early signs of necrosis of the femoral he

Can Hertel Criteria Reliably Predict Avascular Necrosis After Intracapsular Proximal Humerus Fractures in the Elderly? A Retrospective Analysis.

Background/Objectives: Avascular necrosis (AVN) of the humeral head is a severe complication after intracapsular proximal humerus fractures in the elderly. Hertel's radiographic classification is widely used to estimate ischemic risk, yet its real-world accuracy remains debated. Methods: We retrospectively analyzed 204 patients aged ≥65 years treated between 2019 and 2022 for intracapsular proximal humerus fractures. Fractures were classified according to Hertel's criteria and the LEGO system. The incidence of AVN and its association with radiographic predictors were assessed. Diagnostic perfo…

The smoke sign: a secondary sign of unsuspected acute pectoralis major tendon injury on routine shoulder MRI.

OBJECTIVE: To study the diagnostic utility of the "smoke sign" to detect unsuspected acute pectoralis major tendon injury on routine shoulder MRI. MATERIALS AND METHODS: Retrospective study of 52 shoulder MRI in patients with (38) and without (14) acute pectoralis major injury confirmed on imaging. Two readers independently reviewed shoulder MRI for the presence of the "smoke sign"-feathery soft tissue edema lateral or anterior to short head biceps/coracobrachialis on fluid-sensitive coronal-oblique and sagittal-oblique images, respectively. RESULTS: The smoke sign was present on shoulder MR

[Animal model of humeral joint for shoulder arthroscopy training].

UNLABELLED: Shoulder arthroscopy used since sixties of the XXth century, becomes now dominating operative technique. Surgeon's skills are most important that's why perfect training model is still needed. Ideal training model for shoulder arthroscopy should combine best possible image of human anatomical structures, good accessibility and low price. Despite of great similarity, animal shoulder joint differs from human one. More spherical shape of animal glenoid gives better support for humeral head. As a result of different shape of glenoid, labrum is significantly less developed, and can actua…

Deltoid flap reconstruction for massive rotator cuff tears: mid- and long-term functional and structural results.

BACKGROUND: Various open and arthroscopic techniques for irreparable massive rotator cuff tears have been described. Muscle-tendon transfers … 12.5% at long-term follow-up, and correlated with better clinical outcome. Cranial migration of the humeral head progressed in all cases and could not be prevented by the interposition of a deltoid flap. CONCLUSION: Based on the results of this series, we no longer use nor recommend this technique.

Sex-specific differences of subacromial space width during abduction, with and without muscular activity, and correlation with anthropometric variables.

The objectives of this study were to determine sex-specific differences of the subacromial space width during active and passive arm abduction and to analyze the correlation of this space with general and regional anthropometric variables. Fourteen healthy subjects (7 men, 7 women) were examined with an open magnetic resonance system at 30 degrees and 90 degrees of abduction (with and without muscle activity). After 3-dimensional reconstruction, the minimal acromiohumeral distance, the glenoid size, and the humeral head radius were determined. At 30 degrees of abduction, a significant differen…

Comparison between partial and full coverage repair in massive rotator cuff tears. A minimum five year follow-up.

BACKGROUND: Massive rotator cuff surgical management can be challenging. Controversy exists in the literature regarding the potential benefit of full head coverage for arthroscopic rotator cuff repair. The purpose of this study was to compare re-tear rate and clinical outcome of arthroscopic repair of massive rotator cuff tears in relation to the achievement of full humeral head footprint coverage or not. Hypothesis We hypothesized that there will be no difference in re-tear rate and clinical outcomes between full and partial coverage rotator cuff repairs. PATIENTS AND METHODS: A retrospectiv…
