Typhoid perforation of the ileum in children.

Since 1974, 59 children with typhoid perforation of the ileum were studied prospectively using two protocols. In the first 36 cases a temporary total bypass of the terminal ileum was done (end-to-side ileotransverse colostomy) and several weeks later the intestinal continuity was restored (end-to-end ileoileostomy). The second protocol consisted, in 23 cases, of a partial bypass of the terminal ileum (one stage lateral tube ileostomy). Of the first group 10 died (27.7%). There was no reperforations or fistulae in this group. Of the 26 lateral tube ileostomies 8 died (34.8%). There was one repe

Transabdominal ultrasound for standardized measurement of bowel wall thickness in normal children and those with Crohn's disease.

BACKGROUND: The intestinal wall can be visualized using high resolution transabdominal ultrasound (TUS). TTUS measurement of the bowel wall thickness has been described in adults but data are lacking in children. The purpose of this prospective study was to sonographically investigate bowel wall thickness in healthy children and children with Crohn's disease. MATERIAL AND METHODS: TUS (5-15 MHz) of the intestine was performed in 58 healthy children (age range 3 to 16 years) and in 30 children with Crohn's disease (age range 8 to 17 years). The following regions were assessed and bowel wall th

Two forensic autopsy cases of death due to upper gastrointestinal hemorrhage: a comparison of postmortem computed tomography and autopsy findings.

In this report, we describe two autopsy cases of death due to upper gastrointestinal hemorrhage (Case 1: gastric ulcer, Case 2: aortoduodenal fistula). Postmortem computed tomography (CT) images from both cases revealed pooling of gastric fluid, which contained high attenuation areas, although these images also mirrored the different sources of the gastrointestinal hemorrhage. Fluid collection was observed in the small intestine for both cases, although the high attenuation areas were only remarkable in Case 2. The autopsy in Case 1 revealed a peptic ulcer, with small vessels exposed on the su

Percutaneous embolization of persistent low-output enterocutaneous fistulas.

OBJECTIVES: To present and retrospectively evaluate the technique of percutaneous embolization of chronic enterocutaneous fistulas (ECFs) using n-butyl-2-cyanoacrylate and Lipiodol under fluoroscopic guidance. METHODS: Six patients with a total of seven post-operative low-output ECFs of the large intestine were treated. After fistulography a hydrophilic guide wire and a catheter were advanced through the ECF into the intestine. After dilation of the bowel with saline and contrast medium, the catheter was withdrawn into the enteric orifice and glue together with Lipiodol was injected while sim

Complications of typhoid perforation of the ileum in children after surgery.

Sixty six consecutive cases of typhoid perforation of the ileum in children between the ages of four and 14 years were analysed. 62.1% of the children were boys and 37.9% girls, a boy/girl ratio of 1.6:1.0. Postoperative complications included: wound infection, gaped wounds, intestinal obstruction secondary to adhesions, wound dehiscence and pneumonia. The most serious complication was enterocutaneous fistula and the most frequent was wound infection. These postoperative complications adversely affected the morbidity and mortality rates of these severely ill and debilitated patients with typho

Surgical management of colonic diverticulitis and complicated diverticulosis.

The aim of surgery in the management of diverticulitis and complicated diverticulosis is to remove the disease process as quickly as possible. This approach reduces the likelihood of complications and often avoids the necessity of multistage procedures. Indications for resection include failure of a first attack to subside, recurrent attacks, and free perforation with spreading peritonitis. Three-stage procedures should be reserved for extremely serious disease. Other complications requiring surgery are fistula, obstruction, and massive and uncontrolled bleeding. Massive bleeding is caused by

Pediatric ingestion of multiple ball magnets leading to intestinal perforation: A case report.

INTRODUCTION AND IMPORTANCE: Ingestion of foreign bodies is a relatively common cause of abdominal pain in the emergency department among pediatric patients. A less common but potentially life-threatening ingestion is that of magnets. PRESENTATION OF CASE: Here, we report the case of a 6-year-old female who ingested 7 ball magnets in a rural town in Brazil and presented with a 4-day history of abdominal pain. Surgical approach was necessary in this case. It consisted of midline laparotomy that revealed one small intestinal perforation of the antimesenteric aspect of the small bowel. The magne

Definitive surgical treatment of infected or exposed ventral hernia mesh.

OBJECTIVE: To discuss the difficulties in dealing with infected or exposed ventral hernia mesh, and to illustrate one solution using an autogenous abdominal wall reconstruction technique. SUMMARY BACKGROUND DATA: The definitive treatment for any infected prosthetic material in the body is removal and substitution. When ventral hernia mesh becomes exposed or infected, its removal requires a solution to prevent a subsequent hernia or evisceration. METHODS: Eleven patients with ventral hernia mesh that was exposed, nonincorporated, with chronic drainage, or associated with a spontaneous enteroc

[Biliary ileus: a review of the literature and report of a clinical case treated by minilaparotomy].

OBJECTIVE: To evaluate the diagnostic and therapeutic approach to gallstone ileus. MATERIAL AND METHODS: A gallstone ileus report, diagnosed by integrated radiological approach, treated by minilaparotomy plus enterolithotomy and followed up for two years. RESULTS: Traditional radiologic findings, ultrasonography and computed tomography showed both the stone in the bowel lumen and the cholecystoduodenal fistula with air in the biliary tract. The enterolithotomy alone worked out the obstruction and no complication was observed. DISCUSSION: Integrated radiologic examinations are indicated in d

Management of intra-abdominal abscesses in Crohn's disease.

Over a 5-year period, 54 intra-abdominal abscesses were observed in 40 (20.8%) of 192 patients with Crohn's disease. The median age was 39 years (range 17-76 years); median interval from diagnosis, 7.5 years (range 0-24 years) and the median number of surgical operations was 2 (range 0-7). Forty abscesses (74.1%) were spontaneous and 14 (25.9%) were postoperative. Thirty abscesses were initially managed by laparotomy, 14 by percutaneous drainage, nine by incision and drainage and in one case the abscess drained spontaneously. Intra-abdominal abscesses were managed successfully by laparotomy in

[Re-operation for Intrathoracic Complications after Surgery for Thoracic Esophageal Cancer (Chylothorax, Trachea-bronchial Fistula, Post-operative Bleeding)].

Post-operative complications in thoracic esophageal cancer surgery occurred in more than 40% of patients, of which intrathoracic complications are the most serious complications and may require re-operation. Surgeons require a high degree of judgment, skill, and experience at all stages for surgical indications, surgical procedures, and post-operative managements, because re-operation puts a great degree of stress on the patient's mind and body. This article focuses the relatively common post-operative complications that require re-operation, chylothorax, tracheal/bronchial fistula, and post-o

Endoscopic Management of Bouveret's Syndrome in an Elderly Patient: A Case Report.

BACKGROUND: Bouveret syndrome, a rare complication of cholelithiasis, is nonspecific symptoms and high mortality rates, with no established standard treatment. Here, we detail a case of effective endoscopic intervention in an elderly patient. CASE PRESENTATION: A 91-year-old female patient who presented with a one-month history of nausea, vomiting, and significant abdominal distension. An abdominal CT scan revealed a biliary-enteric fistula, pneumobilia, gallstones, a calcified mass in the horizontal portion of the duodenum, and gastric dilatation. Endoscopic examination confirmed the presenc

A date pit induced aorto-oesophageal fistula: a case report and concise literature review.

Aorto-oesophageal fistula (AEF) is rare and fatal without intervention. Having consumed a date pit 2 weeks prior, the patient in this case presented with the 'Chiari' triad of chest pain, sentinel arterial upper gastro-intestinal haemorrhage and exsanguination after an asymptomatic interval. Following resuscitation, the patient was managed with a Blakemore tube with both oesophageal and gastric balloons inflated to systemic pressures. An aortic stent graft was planned but the patient died on the operating table. AEFs can be treated surgically with either open or endovascular repair. Open repai

Genetic factors in esophageal atresia, tracheo-esophageal fistula and the VACTERL association: roles for FOXF1 and the 16q24.1 FOX transcription factor gene cluster, and review of the literature.

Esophageal atresia with/without tracheo-esophageal fistula is a relatively common malformation, occurring in around 1 in 3500 births. In around half of cases, additional malformations are present, forming either a syndrome of known genetic aetiology, or a recognised association, of which the VACTERL association (Vertebral anomalies, Anal atresia, Cardiac malformations, Tracheo-Esophageal fistula, Renal and Limb malformations) is the most recognised. Recently, microdeletions of the FOX gene cluster at 16q24.1, comprising four genes, FOXF1, MTHFSD, FOXC2 and FOXL1, were reported to cause a pheno

Jejunoileal perforation and volvulus caused by multiple magnet ingestion.

Foreign body ingestion is a common problem in children, but magnet ingestion is relatively rare. However, when it occurs, it tends to have a high rate of complications. This is a case report of a 3-year-old child who swallowed multiple magnetic toys, subsequently developing jejunoileal perforation and volvulus. This case report indicates that it is best to surgically remove multiple ingested magnets without delay to avoid intestinal perforation, fistula, and other complications such as volvulus.

[Surgical treatment of necrotizing hemorrhagic pancreatitis with a prolonged course].

The authors describe the course of acute haemorrhagic necrotizing pancreatitis complicated by prolonged septicaemia, the formation of abscesses, acute haemorrhage and a intestinal fistula--"Critically ill patient". Early surgical elimination of necrotic septic foci is the prerequisite of effective treatment and cure. In the diagnosis CT and magnetic resonance prove most useful. With the latter so far little experience as been assembled. Ultrasonographic visualization technique is insufficient for the diagnosis of focal changes in the area of the pancreas.

[Treatment of post-partum urogenital fistulas in Africa. 261 cases observed in 10 years].

A homogeneous series of 261 post-partum urogenital fistulae treated over the last ten years in Africa by the same operator is analysed. Mostly occurring in young, primiparous women in a rural environment, the predominant cause was neglected dystocic delivery in the bush (65.5%). The fistula was traumatic in 27.9% of cases and secondary to hysterectomy in 6.8%. 247 patients underwent surgical repair. The choice of repair procedure, generally performed via the transvaginal route (92.3%), was based on the anatomical classification of the various lesions into three groups of indications: Group I:

Laparoscopic surgery for gallstone ileus.

We present a case of a woman in her 70s who arrived in the emergency department with signs of small-bowel obstruction. CT scanning revealed acute cholecystitis with a cholecystoduodenal fistula, pneumobilia and small-bowel obstruction possibly secondary to gallstone ileus although no radio-opaque gallstones were seen. The patient underwent an emergency operation and intra-operative findings revealed mechanical small-bowel obstruction of the proximal jejunum where a 4×2 x 3 cm gallstone was impacted. A laparoscopic-assisted enterotomy was performed with stone extraction and primary closure. The
