BACKGROUND: Underwater endoscopic mucosal resection without submucosal injection (UEMR) is an appealing therapy for large colorectal polyps. However, this technique is not practiced widely and there are limited data evaluating UEMR in community settings.
METHODS: The study comprised patients undergoing UEMR of large (≥20 mm) sessile colorectal lesions at a community-based center. Residual neoplasia was assessed via follow-up colonoscopy.

Intussusceptions originating in the jejunum are rare. We report a 20-year-old woman who had a chronic jejuno-jejunal intussusception due to an inflammatory fibroid polyp manifesting in the post-partum period as peritonitis. Resection-anastomosis of the intussuscepted segment was done. She is well one year later.

Prophylactic colectomy is generally recommended for patients with familial adenomatous polyposis (FAP) who are inevitably affected with large bowel cancer. Endoscopic polypectomy should be applied only for early colorectal carcinomas. Intramucosal carcinoma do not have a risk of lymph node metastases. However, there is an about 10% risk of lymph node metastases among carcinomas showing submucosal invasion (sm carcinoma). Of the 2704 patients operated on for duodenal ulcer disease within the 10-year period, a benign tumour of the duodenum was revealed in 8 (polyp--in 6, myoma--in 1, fibroma--in 1), and a malignant one--in 3. Endoscopic polypectomy was performed in 6 patients. For malignant tumours, pancreaticoduodenal resection was performed in 1 patient, resection of 2/3 of the stomach with removal of the suprapapillary duodenal portion--in 1, resection of the distal duodenum--in 1. Between September 1983 and March 1989, 36 loop ileostomies were performed on 34 patients (16 male and 18 female, mean age 36 years, range 11 to 68). Thirty-two patients had ileoanal pouch procedures (30 for ulcerative colitis and two for familial polyposis). One patient had a low anterior resection and another had a coloanal procedure. A pilot study has been carried out to evaluate three aspects of screening of first-degree relatives of patients with colon cancer in four Hamilton hospitals; yield of adenomas, feasibility of a one-visit approach to screening and treatment, and compliance. We present here our findings on patients with an elevated lesion of the duodenal bulb. All these patients were treated in our clinics between the years 1984 and 1988. These lesions were present in 36 of 8,802 patients who underwent upper gastrointestinal pan-endoscopy. Two patients had a duodenal carcinoma, 2 an adenoma, and 1 a Brunner's gland adenoma.
