Late repair of an osteochondral fracture of the patella.

Acute patellar dislocations are a common injury in young patients. Osteochondral fractures are commonly associated with this injury, frequently necessitating surgical treatment. This article presents a case of a 23-year-old woman who sustained a large osteochondral fracture of the medial facet of the patella during a first-time acute patellar dislocation. The patient was lost to follow-up; consequently, surgical treatment was not performed until 8 weeks after injury. Diagnostic arthroscopy demonstrated a large osteochondral fragment in the suprapatellar pouch and a defect on the medial facet o

[Injuries of the major blood vessels in joint dislocations].

Experience with the treatment of 12 patients with injuries of major vessels resulting from dislocations of joints is generalized. In 5 of the patients an associated injury of nerve trunks was diagnosed, in 4 patients there were injuries of major veins. Reconstructive operations were made in 11 patients. In 8 patients full recovery was stated, three patients were subjected to amputation of the extremity. The main cause of the failure was late diagnosis of the closed trauma of the artery and progressing ischemia of the extremity.

Reconstruction of the Medial Patellofemoral Ligament: Clinical Outcomes and Return to Sports.

BACKGROUND: Recent studies have shown that medial patellofemoral ligament (MPFL) reconstruction using a standardized technique provides significant improvements in all outcome scoring systems, with low complication rates and good patient satisfaction. Although numerous studies have assessed clinical results, there is little published literature investigating return to sporting activities after reconstruction of the MPFL. PURPOSE: To demonstrate postoperative outcomes and the return-to-sports rate a minimum of 2 years after isolated MPFL reconstruction in a young patient cohort. STUDY DESIGN:

Distal realignment (tibial tuberosity transfer).

Although tibial tuberosity (TT) transfer has for many years been the basis of many protocols for the management of patellar instability, the role of pure medial transfer in particular appears to be declining. In contrast, the greater recognition of the importance of patella alta as a predisposing factor to recurrent patellar dislocation has resulted in a resurgence in the popularity of distal TT transfer. When TT transfer is performed, the direction and amount of transfer is based on the patellar height and the lateralization of the TT relative to the trochlear groove. Patellar height is best

Early results of one-stage knee extensor realignment and autologous osteochondral grafting.

We treated 49 patients with recurrent patellar dislocations or persistent patellar subluxations. Chondral defects were graded according to the International Cartilage Repair Society (ICRS). Thirty patients (group I) had chondral defects grade I or II, and 19 patients (group II) had chondral defects grade III or IV. All patients were treated with proximal and distal realignment of the knee extensor mechanism, but group II also had a simultaneous autologous osteochondral grafting of the chondral defect. Patients were followed for 2 years and clinically assessed using the Marshall score comparing

[Osteotomy to lengthen the femur neck with distal adjustment of the trochanter major in coxa vara after hip dislocation].

Hip dysplasia and treated dislocations of the hip can lead to deformity of the proximal femur with shortening of the femoral neck and proximal displacement of the greater trochanter. Shortening the femoral neck causes a reduction in the leg length and insufficient performance of the abductors in the hip. Furthermore, the mechanical axis of the knee joint is lateralized. We used a technique involving lengthening the femoral neck and distal transfer of the greater trochanter to restore the normal anatomy and normal biomechanics of the hip joint. We used three osteotomies: one at the greater troc

What is the Optimal Treatment Protocol for Traumatic Popliteal Artery Injury? A Comparative Study between Two Institutions.

INTRODUCTION: While rare, popliteal artery injury is a potentially devastating complication of knee trauma. Delayed diagnosis or inadequate management of popliteal artery injury can result in limb ischemia, amputation, and long-term disability. This study aimed to compare the treatment outcomes of popliteal artery injury between different medical centers and to reconsider the optimal protocol at our institution. METHODS: Medical records of patients sustaining traumatic popliteal artery injury from 2011 to 2022 at two institutions (one hospital: Group A; the other hospital: Group B) were retro

[Clinical efficacy of arthroscopic medial patellofemoral complex reconstruction for recurrent patellar dislocation with high-grade trochlear dysplasia].

OBJECTIVE: To investigate the midterm clinical efficacy of medial patellofemoral complex (MPFC) reconstruction for recurrent patellar dislocation with high-grade trochlear dysplasia. METHODS: A retrospective analysis was carried out among adult patients who underwent arthroscopically assisted MPFC reconstruction between January 2014 and December 2020. Dejour classification was evaluated to grade trochlear dysplasia; tibial tubercle-trochlear groove (TT-TG) distance and Insall-Salvati index were measured. Preoperative and postoperative patient-reported outcome measures (PROMs) were compared, i

Treatment Decisions in Pediatric Sports Medicine: Do Personal and Professional Bias Affect Decision-Making?

BACKGROUND: Personal and professional biases can affect decision-making regarding important issues in pediatric sports medicine. Gaining insight into the opinions of health care professionals who specialize in pediatric sports medicine will provide information that may be useful for directing ongoing research in this field. HYPOTHESIS: It was hypothesized that surgeons would demonstrate bias toward early surgical intervention versus nonsurgeons. In addition, it was hypothesized that youth sports medicine professionals who were parents of a child with a previous major sports injury or concussi

Editorial Commentary: When Properly Indicated, Combined Tibial Tubercle Osteotomy Reduces Risk of Recurrent Patellofemoral Instability More Than Does Isolated Medial Patellofemoral Ligament Reconstruction.

Clinical recognition and surgical treatment of patellofemoral instability has evolved dramatically over the past 3 decades. However, few patellofemoral patients present with an isolated medial patellofemoral ligament (MPFL) tear. Rather, patients often demonstrate patella alta, increased tibial tubercle to trochlear groove (TT-TG) distance, dysplasia, coronal malalignment, or combinations thereof. Given this, concomitant procedures such as tibial tubercle osteotomy (TTO) have become increasingly popularized, given their ability to anteriorize, medialize, and even distalize the patella to corre

The severity of patellar and trochlear dysplasia are correlated.

PURPOSE: Patellar instability is a frequent clinical situation in adolescents and young adults. One of the most important risk factors of this condition is trochlea dysplasia. Few studies have analyzed the role of patellar dysplasia. The objective of this paper was to assess the correlation between trochlear and patellar dysplasia. The hypothesis is that patellar dysplasia increased in parallel with femoral trochlear dysplasia. METHODS: The study included patients operated on at least one knee for patellar instability between 2008 and 2020. For these patients, symptomatic knees (operated or n

Simulation of patella alta and the implications for in vitro patellar tracking in the ovine stifle joint.

Patella alta is associated with adverse cartilage adaptations, patellofemoral pain, and instability. It is defined by a relatively long patellar tendon and patella positioned in a more proximal location within the patellar groove of the femur. This study used the ovine stifle joint model to investigate the effect of patellar tendon lengthening on the 3D passive kinematics of the patellofemoral and tibiofemoral joints. Eight patellar tendons were lengthened in 2 mm increments up to a maximum of 12 mm (20%) using a device placed in series with the transected patellar tendon. Three-dimensional ki
