Dual Framework for Classification and Detection of Third Molar Impaction in Panoramic Radiographs.

BACKGROUND: The surgical extraction of impacted mandibular third molars present significant clinical challenges, where accurate preoperative assessment is crucial to mitigate risks such as Inferior Alveolar Nerve injury. Although artificial intelligence shows promise in dental radiology, existing approaches are often limited to binary classification, affected by class imbalance, and lack standardized evaluation protocols, thereby restricting their clinical applicability. METHODS: This study proposes two independent deep learning frameworks for comprehensive analysis of third molar impactions.

Case Report: Soft-tissue-preserving fixation for displaced segmental maxillary fractures involving partially detached bone segments.

BACKGROUND: Displaced segmental maxillary fractures involving partially detached bone segments are difficult to manage because treatment requires both stable fixation and preservation of residual vascularity. Excessive mucoperiosteal elevation may further compromise the blood supply of a displaced segment and increase the risk of bone resorption, sequestration, or necrosis. This report presents two cases of displaced segmental maxillary fractures treated with soft-tissue-preserving fixation strategies. CASE PRESENTATION: Two patients with displaced segmental maxillary fractures were managed u

Impacted mandibular first molar with root encircling the inferior alveolar nerve and an odontogenic cyst formation.

BACKGROUND: Dentigerous cysts are among the most common benign lesions encountered in oral and maxillofacial surgery. While these cysts typically develop in association with impacted mandibular third molars, their occurrence in relation to mandibular first molars represents an exceptionally rare clinical entity. This study aims to systematically describe the surgical management, standardized postoperative care protocols, and long-term follow-up strategies for this rare presentation. CASE PRESENTATION: A 35-year-old female patient presented to our institution with a 12-month history of persist

Relation of Mandibular Canal Discontinuity and IAN Paresthesia in Operated Cases of Mandibular Fracture.

INTRODUCTION: Mandibular fractures are common in maxillofacial trauma, often requiring surgical intervention for optimal recovery. One critical concern during such procedures is the potential for nerve damage, specifically involving the mandibular canal. This study aims to investigate whether the presence of postoperative mandibular canal discontinuity is associated with an increased risk of nerve damage. METHODOLOGY: A retrospective analysis was conducted on a cohort of 72 postoperative mandibular fracture patients. In total, 58 patients had entire mandibular canal continuity, while 14 showe

An imaging‑based diagnostic approach to vascular anomalies of the oral and maxillofacial region.

The accurate diagnosis of vascular anomalies (VAs) is considered a challenging endeavor. Misdiagnosis of VAs can lead clinicians in the wrong direction, such as the performance of an unnecessary biopsy or inappropriate surgical procedures, which can potentially lead to unforeseen consequences and increase the risk of patient injury. The purpose of the present study was to develop an approach for the diagnosis of VAs of the oral and maxillofacial region based on computed tomography (CT), magnetic resonance imaging (MRI) and dynamic contrast-enhanced MRI (DCE-MRI). In the present study, the CT a

Manual versus rigid intraoperative maxillo-mandibular fixation in the surgical management of mandibular fractures: A European prospective analysis.

PURPOSE: Intraoperative stabilisation of bony fragments with maxillo-mandibular fixation (MMF) is an essential step in the surgical treatment of mandibular fractures that are treated with open reduction and internal fixation (ORIF). The MMF can be performed with or without wire-based methods, rigid or manual MMF, respectively. The aim of this study was to compare the use of manual versus rigid MMF, in terms of occlusal outcomes and infective complications. MATERIALS AND METHODS: This multi-centric prospective study involved 12 European maxillofacial centres and included adult patients (age ≥1

Complex-type penetrating injuries of craniomaxillofacial region.

Craniofacial traumas are one of the most common clinical events of the 21st century. The possibility of associated injuries of the head and neck may also determine functional and cosmetic problems in these patients. The most frequent pathologic conditions observed are contusions, lacerations, abrasions, avulsions, and the inclusion of foreign bodies. In particular, penetrating injuries represent a rare but complex variety of craniofacial trauma. Generally, the penetrating material is stiff enough to cross through different anatomic structures during a particularly violent collision caused by a

Pediatric facial fractures: evolving patterns of treatment.

This study reviews the treatment of facial trauma between October 1986 and December 1990 at a major pediatric referral center. The mechanism of injury, location and pattern of facial fractures, pattern of facial injury, soft tissue injuries, and any associated injuries to other organ systems were recorded, and fracture management and perioperative complications reviewed. The study population consisted of 137 patients who sustained 318 facial fractures. Eighty-one patients (171 fractures) were seen in the acute stage, and 56 patients (147 fractures) were seen for reconstruction of a secondary d

An interdisciplinary approach to aesthetic single implant outcomes for young patients.

Clinicians carry a significant obligation to young patients with single missing teeth to ensure that they are prepared for a lifetime of healthy dental function and long-term aesthetic success. Adolescents and young adults in need of implant treatment embody a large and growing patient population that requires distinctly different treatment approaches than those that apply to older patient groups. The primary causes of single missing teeth in young patients are trauma and congenital absence, whereas older populations typically lose teeth due to failure of traditional dental procedures. This ar

Use of rigid external distraction device in treatment of complex maxillofacial fractures.

Collapse of maxillofacial skeletal structures after trauma results in inadequate anteroposterior projection and increase in facial width with deformed facial aesthetics and various functional problems. After successful use of rigid external distractor for treatment of congenital facial hypoplasia, they have been used for treating more complex craniofacial anomalies. Six patients with posttraumatic orbitozygomaticomaxillary deformity were managed with intraoperative and/or postoperative use of rigid external distractor device. All the patients maintained the early postoperative aesthetic and fu

Do CBCT scans alter surgical treatment plans? Comparison of preoperative surgical diagnosis using panoramic versus cone-beam CT images.

UNLABELLED: Cone beam CT and/or panoramic images are often required for a successful diagnosis in oral and maxillofacial surgery. The aim of this study was to evaluate if 3D diagnostic imaging information had a significant impact on the decision process in six different classes of surgical indications. MATERIAL AND METHODS: Records of all patients who had undergone both panoramic X-ray and CBCT imaging due to surgical indications between January 2008 and December 2012 were examined retrospectively. In February 2013, all surgically relevant diagnoses of both conventional panoramic radiographs

Incidence and patterns of mandibular fractures during a 5-year period in a London teaching hospital.

We retrospectively evaluated mandibular fractures in patients who presented to a London teaching hospital between June 2005 and May 2010. A total of 1261 patients sustained 1994 mandibular fractures (mean 1.6, range 1-5). The incidence of mandibular fracture was higher in male patients (87%) than in females (13%) (male:female ratio 6.6:1), and the peak incidence was during the third decade for both genders. The most common site of fracture was the angle (30%), followed by the parasymphysis (27%), and condyle (27%). Overall, interpersonal violence was the most common cause (72%) followed by fal

Trifocal distraction-compression osteosynthesis in conjunction with passive self-ligating brackets for the reconstruction of a large bony defect and multiple missing teeth.

Reconstruction of a maxillary dentoalveolar defect and closure of a wide oronasal fistula in a patient with a traumatic injury are challenging for both orthodontists and surgeons. A conventional bone graft is used to fill the alveolar bone defect, to restore continuity between bony segments, and to provide bony support for tooth eruption adjacent to the defect or for orthodontic tooth movement into the bony defect. However, if the defect is too large to allow for a conventional bone graft, transport distraction osteogenesis can be used for reconstruction of the alveolar bone and implant placem

Maxillofacial fixation with absorbable miniplates: computed tomographic follow-up.

Rigid internal fixation has become a mainstay of treatment for maxillofacial trauma. Refinement in materials and manufacturing have led to unobtrusive titanium miniplates and microplates. However, concerns remain regarding the presence of plates and screws after fracture healing has occurred. Absorbable rigid fixation systems ideally would provide sufficient strength and fixation to allow osseous healing and then be absorbed without sequelae. Plates and screws made from polylactic acid-polyglycolic acid copolymer approach this ideal. Four maxillofacial trauma patients underwent open reduction
