Erroneous intramedullary placement of spinal cord stimulator: A case report and review of the literature.

BACKGROUND: Spinal cord stimulation is a common treatment for patients with medically refractory chronic neuropathic pain. Before permanent implantation of spinal cord stimulation, patients will undergo a percutaneous trial to ensure the efficacy of the treatment modality and determine the optimal location of placement. While complications from this procedure are rare, there are reports in the literature of infection, epidural hematoma, and even paralysis. There are few reports of percutaneous leads tracking through the spinal cord itself, and subsequently, few reports of management should suc

Long-Term Monitoring of an Intracorneal Bee Stinger Using Anterior Segment Optical Coherence Tomography and In Vivo Confocal Microscopy.

A 19-year-old male patient presented to our clinic with complaints of foreign-body sensation, pain, redness, tearing, and decreased vision in the right eye. One day prior, a bee struck his eye while he was riding a motorcycle. Biomicroscopic examination of the right eye showed periorbital edema, chemosis, ciliary injection, corneal edema with radiating Descemet folds, paracentral corneal infiltrate, and a brown needle-like foreign body resembling a bee stinger. Anterior segment optical coherence tomography (AS-OCT) confirmed corneal edema with Descemet folds, whereas in vivo confocal microscop

Blunt Chest Trauma-Related Cardiovascular Intervention in Two Pediatric Cases. A Silent Threat.

Blunt thoracic traumas due to traffic accidents or falls from a height may conceal the degree of the underlying injury, leading to an underestimation of the actual clinical picture. Two pediatric poly-traumatized patients were admitted to the emergency room following traffic accidents. The first patient was overrun by a motor vehicle and seemed to be doing well in ICU when, all of a sudden, he developed heart failure with hemodynamic instability on day 14. Transesophageal and color flow Doppler echocardiography demonstrated an acquired 10 mm ventricular septal defect with left-to-right shuntin

Awake at the wheel: how auto technology innovations present ongoing sleep challenges and new safety opportunities.

Individuals and society are dependent on transportation. Individuals move about their world for work, school, healthcare, social activities, religious and athletic events, and so much more. Society requires the movement of goods, food, medicine, etc. for basic needs, commerce, cultural and political exchanges, and all of its dynamic, complex elements. To meet these critical daily demands, the transportation system operates globally and around the clock. Regardless of their role, a basic requirement for the individuals operating the transportation system is that they are awake and at optimal al

Brain signatures of catastrophic events: Emotion, salience, and cognitive control.

Anticipatory brain activity makes it possible to predict the occurrence of expected situations. However, events such as traffic accidents are statistically unpredictable and can generate catastrophic consequences. This study investigates the brain activity and effective connectivity associated with anticipating and processing such unexpected, unavoidable accidents. We asked 161 participants to ride a motorcycle simulator while recording their electroencephalographic activity. Of these, 90 participants experienced at least one accident while driving. We conducted both within-subjects and betwee

Surgical options for improving the ipsilateral femoral neck, shaft, and distal femoral fracture: A case report and literature review.

INTRODUCTION AND IMPORTANCE: Ipsilateral proximal, shaft, and distal femur fractures are extremely uncommon. It might be challenging and contentious to treat ipsilateral multi-level femur fractures. There are still unanswered questions regarding the order of fracture types that should be repaired first and the type of implant that should be used. CASE PRESENTATION: A twenty-nine-year-old male patient was assessed at the emergency department after a motorcycle accident. The patient had a clearly deformed left lower extremity and was complaining of pain in the left thigh. Preoperative radiograp

Complex Axial Carpal Dislocation with Scapholunate Ligament Injury.

Background  Axial carpal dislocations (ACDs) are high-impact injuries, involving both proximal and distal carpal rows, adding complexity to the diagnostic and treatment challenges. This article explores a distinctive case of axial radial carpal dislocation, highlighting an irreducible second carpometacarpal joint (CMCJ) and concomitant scapholunate (SL) ligament injury. Understanding the background of axial carpal injuries is crucial for recognizing and addressing these complex cases effectively. Description  A 27-year-old man presented with a left wrist injury following a motorcycle accident,

Combined tracheoesophageal transection following a life-threatening clothesline-type blunt neck trauma: A case report.

INTRODUCTION: Blunt neck trauma is an uncommon, life-threatening injury that may result in tracheoesophageal transection. The manifestations of these traumas are rather vague and nonspecific; therefore, the injury may be missed, if a careful attention is not paid. CASE PRESENTATION: A 23-year-old young man presented with complete transection of the trachea and concurrent esophageal injury, caused by clothesline-type blunt neck trauma, while riding a motorcycle. On early examination, the patient was hemodynamically stable; however, after a few minutes, he manifested respiratory distress and pr

Extensive tension pneumocephalus presented in the setting of a challenging etiology.

BACKGROUND: Pneumocephalus (PNC) is a well-described consequence in postoperative settings and skull fractures that is usually self-limiting. It can get complicated into tension PNC on some rare occasions, leading to an intracranial mass effect. PNC was also reported after unintentional dural puncture throughout the epidural anesthesia process. However, tension PNC resulting from epidural anesthesia procedures is an extremely rare outcome that implies urgent intervention to relieve the tension within the brain. Here, we report a case of an extensive tension intraventricular PNC 2 days followin

Surgical rib fixation as an alternative method of treatment for multiple rib fractures: an audit of results compared with traditional medical management.

BACKGROUND: Rib fractures are a common cause of morbidity and chronic pain, delaying return to normal activities. Reports suggest that surgical fixation improves acute and long-term outcomes. METHODS: A single centre retrospective review of multiple rib fractures, comparing the outcomes of cases managed using surgical fixation with cases managed only with best medical therapy (BMT) over 2 years. RESULTS: Thirty-five patients with rib fractures were admitted over the study period. The most common causes of rib fractures were motorcycle crashes (34.2%) and falls (31.4%). Fourteen patients had

Treatment of purely ligamentous dorsoulnar radiocarpal dislocation with ulnar translation of the carpus: A case report.

RATIONALE: Radiocarpal fracture-dislocations are extremely infrequent injuries caused by high-energy trauma and involve significant osseous and ligamentous injuries. If not treated properly, it can lead to serious complications such as ulnar translation of the carpus, multidirectional instability, loss of motion, and post-traumatic arthritis. Purely ligamentous injuries are rarer than fracture-dislocation injuries. Because previous studies have reported small patient cohorts, there has been no standardized treatment strategy for purely ligamentous radiocarpal dislocation. PATIENT CONCERNS: A
