Traumatic bilateral carotid artery dissection following severe blunt trauma: a case report on the difficulties in diagnosis and therapy of an often overlooked life-threatening injury.

BACKGROUND: Traumatic carotid artery dissections are very rare, often overlooked and life-threatening injuries. Diagnosis and treatment are difficult especially in multiple injured patients. CASE PRESENTATION: We report on a 28-year-old female major trauma patient (injury severity score, ISS 50) who was involved in a motor vehicle accident. She was primarily transferred to a level II trauma center. After initial assessment and operative management, an anisocoria was diagnosed on the intensive care unit. Subsequent CT angiography and extracranial duplex sonography revealed a bilateral internal

Spontaneous vertebral artery dissection with thunderclap headache: a case report and review of the literature.

PURPOSE: Vertebral artery dissection (VAD) is an important and under-recognized etiology of stroke in young patients. No clinical symptoms or signs appear to be specific for VAD. This report describes a representative patient and reviews the headache pattern and imaging findings commonly noted in VAD to help with the early diagnosis of VAD. CASE REPORT: A 44-year-old female presented with severe right posterior neck pain and vertigo followed closely by thunderclap headache that was confirmed as right VAD (V4 segment) with delayed right dorsal medullary infarction two days later. Her headache,

Asymptomatic carotid artery dissection caused by blunt trauma.

A 42-year-old man presented with asymptomatic traumatic carotid artery dissection 3 months after sustaining blunt injury with tracheal laceration. Magnetic resonance imaging performed as a screening procedure for asymptomatic carotid artery injury unexpectedly showed dissection of the carotid artery. Angiography confirmed carotid artery dissection with narrowing of the true lumen. Balloon angioplasty and stent placement were carried out to dilate the true lumen and isolate the false lumen from blood flow. Intravascular ultrasonographic virtual histology defined the precise anatomic structure o

Angiosarcoma of the submandibular salivary gland: case report and review of the literature.

OBJECTIVES: The purpose of this report is to describe an example of angiosarcoma (AS) of the submandibular salivary gland. The clinical and … endothelial cell differentiation. Owing to the lateralized nature of the lesion, a left functional neck dissection was performed and postoperative radiotherapy was planned. RESULTS: AS is a malignant tumor of endothelial cell origin that may occur in any region of the body. The commonest sites include the extremities and the retroperitoneal space, with only 4% …

[A new method of tongue reconstruction with neurovascular infrahyoid muscle-fascia flaps].

Surgical treatment of tongue cancer can lead to extended defunctionalization, depending upon the size and localisation of the defect. Great problems of swallowing arise after total glossectomy or extensive resections of the base of the tongue even after reconstruction with myocutaneous island flaps or free flaps. We developed a neuromuscular island flap derived from the infrahyoidal musculature to reconstruct a total tongue or large defects of the tongue base. This fasciomuscular flap is formed by the M. sternothyroideus, M. sternohyoideus and the upper part of the M. omohyoideus. The axial bl

Thyrotoxicosis With Medullary Thyroid Carcinoma: A Rare Endocrine Diagnostic Challenge.

The incidence of a hyper-functioning thyroid gland presenting alongside an underlying malignancy rarely occurs. Herein, we present a rare case of a 56-year-old female patient presenting with features of hyperthyroidism only to be later diagnosed with an underlying malignancy, medullary thyroid carcinoma (MTC). Ultrasonography (USG) of the neck revealed a suspicious nodule with microcalcifications in the right lobe of the thyroid. Subsequent CT imaging of the neck indicated possible cervical and mediastinal lymph node involvement. Fine-needle aspiration cytology (FNAC) confirmed the presence of

Common carotid artery injury caused by a camel bite: case report and systematic review of the literature.

A 25-year-old man was bitten in the neck by an aggressive camel, causing three small puncture wounds. The left carotid pulse of the patient was weakly palpated. Angiography showed irregular dissection of the distal part of the left common carotid artery. Neck exploration confirmed the findings. An interposition autogenous saphenous vein graft was performed successfully. The patient was discharged home in good condition. We have systematically reviewed the literature on this topic, and only four other similar cases were reported previously. Although camel bite wounds are small, they may penetra

Osteosarcoma of the mastoid process following radiation therapy of mucoepidermoid carcinoma of the parotid gland--a case report.

Radiation therapy is frequently used method in treatment of the head and neck malignancies. Osteosarcoma is a rare complication of radiation therapy and usually occurs after a long latent period. We report the case of 75-year-old female with osteosarcoma of the mastoid process. Twelve years before presentation she received radiation therapy after total parotidectomy and radical neck dissection in treatment of mucoepidermoid carcinoma of the parotid gland. Diagnostic procedures included contrast-enhanced CT and MRI of the head and neck and HRCT of the temporal bone. The final diagnosis of the l…

Laparoscopic extraperitoneal bladder neck suspension (LEBNS) for stress urinary incontinence.

Seventy-nine patients of bladder neck suspension using an extraperitoneal variation of laparoscopic surgery were performed for the treatment of stress urinary incontinence. Using a balloon dissector the anterior vesical pelvic space is secured. The bladder neck suspension similar to the Burch operation was performed through a laparoscopic procedure. Symptoms of patients were assessed preoperatively and at one and six months following surgery. Operative times and complications were also evaluated. Success rate was 89.8% at six months. Complications such as bladder perforations were observed. La

Transient perivascular inflammation of the carotid artery syndrome in a 52-year-old female patient: a case report.

BACKGROUND: Transient perivascular inflammation of the carotid artery syndrome is a rare and under-recognized clinical condition. It may mimic more serious vascular disorders such as carotid dissection or vasculitis, leading to unnecessary investigations or treatment. Recognition of this benign, self-limiting entity is essential to ensure appropriate management. CASE PRESENTATION: We describe the case of a 52-year-old Caucasian Greek woman who presented with acute, well-localized pain on the left side of her neck, centered over the carotid artery and radiating toward the ipsilateral jaw. She

Minilifting technique for treatments of the lower third of the face and anterior cervical neck with tumescent anesthesia.

BACKGROUND: Minimally invasive surgical techniques with a low rate of complications and rapid return to daily activities are in high demand for dermatological procedures and include lasers, microfocused ultrasound, botulinum toxin, suture strings, and filling techniques with hyaluronic acid. The facelift, on the other hand, has been criticized for complications such as bruising, relatively slow recovery rates (lasting up to a month), facial nerve lesions, and flap necrosis. Earliest descriptions of the facelift technique described a procedure associated with skin traction and detachment. Howev
