Invasive Medullary Type A Thymoma With Recurrent Distant Metastases.

Thymomas are rare epithelial mediastinal tumors showing a certain propensity for local and intrathoracic recurrences. Otherwise, extrathoracic metastases are very rarely reported and are usually associated with type B thymomas or thymic carcinomas. We present a challenging patient with an invasive (Masaoka-Koga stage IIb) medullary type A thymoma with recurrent extrathoracic metastases (pulmonary and vertebral localizations occurring 2 and 7 years after the initial radical thymectomy, respectively). Despite type A thymoma being considered a low-grade malignancy with a scarce propensity to recu

Pediatric Pharyngeal IgD-positive Monoclonal Plasmacytoid and Plasma Cell Neoplasm.

Pediatric neoplasm with monoclonal proliferation of lymphoplasmacytoid lymphocytes and plasma cells is exceedingly rare and has essentially never been reported in immunocompetent children. Here, we report a previously healthy 13-year-old girl with a pharyngeal mass and enlarged cervical lymph nodes. The pharyngeal mass was composed of CD138, CD79a, MUM-1, IgD, CD20, PAX-5, CD43, λ-restricted monoclonal plasmacytoid, and plasma cells. Scattered CD20, PAX-5 B cells were present in the background. The patient was treated as localized non-Hodgkin lymphoma (stage II) with cyclophosphamide, doxorubi

Thymoma and solid-organ transplantation.

Solid-organ-transplant recipients are at high risk of developing cancer, including thoracic malignancies. Herein, we report on two cases of thymoma in solid-organ recipients. One case was a female kidney-transplant recipient who had a type B2, Masaoka stage IVA thymoma that was treated by extended surgical resection followed by postoperative radiation therapy. The second case was type A Masaoka II thymoma that occurred after a liver transplant. The role of EBV in the oncogenesis of thymoma is controversial. We suggest that viruses may induce neoplasms in this specific population, as observed i

Radiological manifestations of skeletal lymphoma.

Lymphoreticular neoplasms primarily arise in extraskeletal locations with skeletal involvement usually secondary to hematogenous spread or by direct invasion from surrounding involved lymph nodes or soft tissues. Primary lymphoma of bone is relatively rare in comparison. Lymphoma encompasses Hodgkin's and non-Hodgkin's disease, Burkitt's lymphoma, and mycosis fungoides. Skeletal disease may present with symptoms localized to the site of bone involvement, as an incidental finding on imaging for other reasons, or as part of the staging of the disease. It is important that the radiologist is cogn

A rare case of jugular foramen schwannoma arising from Jacobson's nerve.

The jugular foramen (JF) region is a complex area of the cranial base where venous structures such as the jugular bulb and the inferior petrosal sinuses are strictly related to the lower cranial nerves IX, X and XI. The most common tumours include glomus jugulare, schwannomas of the mixed cranial nerves (IX-XI) and meningiomas. Schwannomas involving the jugular foramen are rare neoplasms and in most of the cases are thought to originate from the X cranial nerve. We report a case of a schwannoma of the JF diagnosed at an early stage, allowing radiological and surgical evidence to support its or

[Results of the surgical treatment of patients with basal extracerebral parastem tumors of large and giant dimensions].

A retrospective analysis of the results of treatment of 123 patients with large and giant basal extracerebral parastem tumors was made. The majority of the neoplasms (81%) were acoustic neuromas and meningiomas. The nearest results of the treatment were good in 103 patients and unfavorable results were noted in 20 patients. Postoperative mortality was 8.9%. Four factors negatively influencing the outcome were revealed during the statistical analysis (p < 0.05): low Karnofsky score at admission, regrowth (recurrence) of the tumor, previously performed radiation therapy and shunt surgery as the

"Early" cancer of the larynx: the concept as defined by clinicians, pathologists, and biologists.

Since a great deal of confusion surrounds the different uses made by clinicians, pathologists, and biologists of the term, this paper clarifies the concept of "early" cancer of the larynx. Clinically, this is usually a glottic neoplasm in which full cordal mobility is still present; early supraglottic cancer is infrequent, and usually a chance finding. Whatever the site, early laryngeal cancer is a minimally invasive neoplastic lesion that does not invade the muscle or cartilage, but is still capable of metastasis. Being confined to the lamina propria, it is more than a carcinoma in situ but l

Recurrent transient superior vena cava-like syndrome possibly associated with megestrol acetate.

BACKGROUND: Superior vena cava syndrome is a rare, life-threatening clinical entity associated with occlusion of venous outflow from the head, neck, and upper extremities; this syndrome produces facial edema, cyanosis, dyspnea, and prominent neck veins, and is usually caused by intrathoracic neoplasms. CASE: In this unusual case, a patient receiving megestrol acetate for stage IV endometrial carcinoma developed recurrent transient superior vena cava-like syndrome in the absence of venous obstruction. CONCLUSION: Superior vena cava-like syndrome may occur with megestrol acetate. This unusual

Atypical pulmonary and neurologic complications of amiodarone in the same patient. Report of a case and review of the literature.

Atypical manifestations of pulmonary toxicity and previously unreported autonomic nervous system dysfunction complicating amiodarone therapy were observed in a patient being treated for sustained ventricular tachycardia. Pulmonary and hepatic nodules on computed tomographic scan masquerading as metastatic carcinoma were initially noted. Focal infiltrates and a large left pleural effusion mimicking infection, malignant neoplasm, or collagen vascular disease became manifest at a later stage. Autonomic dysfunction presented as incapacitating orthostatic hypotension and persisted for six weeks aft

Papillary adenocarcinoma of the male urethra. Case report and review of the literature.

Mucinous papillary adenocarcinoma of the bulbous urethra developed in a 40-year-old man with congenital stricture. The tumor appeared as a localized polypoid exophytic mass without evidence of submucosal invasion (Stage 0). The tumor was determined not to be of prostatic origin clinically and by absence of immunoperoxidase staining for prostatic acid phosphatase, prostate specific antigen, and carcinoembryonic antigen. Surgical resection of the urethra revealed extensive squamous and glandular metaplasia (urethritis glandularis) in association with the neoplasm. The mucosa displayed a spectrum

Tumor classification and size in germ-cell testicular cancer: influence on the occurrence of metastases.

The influence of local tumor spread (T-classification) and of tumor size on the occurrence of metastases was studied in 241 patients with testicular germ-cell neoplasms. All patients underwent thorough clinical or pathologic staging, or both, and were treated at the University of Minnesota Hospitals. Spread of tumor through the tunica vaginalis (T2) was associated with abdominal lymph node or distant metastases in eight of nine patients. Local tumor extension to the spermatic cord (T4a) was associated with metastatic spread in 29 of 31 men. Tumor size did not appear to correlate with metastati

Renal arteriovenous fistulas secondary to bilateral renal cell carcinoma.

Arteriovenous fistulas commonly occur in cases of renal cell carcinoma, but they rarely produce sufficient left-to-right shunting to cause decompensation of the cardiovascular system. A case of refractory hypertension and congestive heart failure secondary to arteriovenous shunting in bilateral renal cell carcinoma is presented. Renal blood flow studies at the time of staged nephrectomy, together with measurements of cardiac output, established parenchymal arteriovenous shunting as the cause of the patient's cardiovascular symptoms. Subsequent nephrectomy achieved a marked clinical and symptom

The peripheral immune landscape of human breast cancer.

The clinical management of patients with breast cancer is largely dictated by tumor stage, disease subtype and (at least in some settings) specific molecular features thereof (e.g., BRCA1 mutations, PD-L1 positivity). However, not all patients with breast cancer respond to therapeutic approaches that a priori are precisely targeted to their disease. At least in part, this may reflect the fact that breast neoplasms are highly heterogeneous from a broad immunological perspective, even when they exhibit similar disease subtype and molecular features. Thus, integrating extra immunological paramete

Diagnostic value of iris biopsies for the assessment of iris pigment changes and diffuse iris melanoma (DIM) in cats: A descriptive histopathological study.

Feline diffuse iris melanoma (FDIM) is a common ocular neoplasm, typically affecting older cats, characterized by progressive hyperpigmentation and potential malignancy. This study evaluated the diagnostic value of iris biopsies in distinguishing FDIM from benign iris melanosis. Seventeen cases with suspected FDIM were analyzed through ex vivo iris biopsies and corresponding enucleated globes. Histological evaluation by 8 pathologists revealed an average concordance of 81.6% between biopsy and globe diagnoses. The reliability of biopsy samples was influenced by tissue quality, with 12/136 slid

Primary Cilia as a Tumor Marker in Pituitary Neuroendocrine Tumors.

Pituitary neuroendocrine tumors (PitNETs) account for approximately 15% of all intracranial neoplasms. Although they usually appear to be benign, some tumors display worse behavior, displaying rapid growth, invasion, refractoriness to treatment, and recurrence. Increasing evidence supports the role of primary cilia (PC) in regulating cancer development. Here, we showed that PC are significantly increased in PitNETs and are associated with increased tumor invasion and recurrence. Serial electron micrographs of PITNETs demonstrated different ciliation phenotypes (dot-like versus normal-like cili

Follicular lymphoma: an update on biology and optimal therapy.

Follicular lymphoma (FL) is a mature B-cell neoplasm with a mostly indolent course. Genetic hallmark characteristics are Bcl-2 overexpression based on a t(14;18) translocation and additional secondary genetic and epigenetic aberrations. Standard treatment for early-stage patients has curative intent and usually consists of radiotherapy with or without rituximab. In the advanced stage, the main therapeutic focus is on prolonged remissions. Therefore, treatment in asymptomatic patients is usually deferred. Symptomatic patients are subject to immunochemotherapy induction followed by antibody main

Metastatic testicular cancer presenting with hematuria and flank pain.

Testicular cancer (TC) represents 1% of male neoplasms and 5% of urological tumors. Most of seminoma patients and about 55% of patients with nonseminoma TC have stage I disease at diagnosis. TC usually presents with a palpable testicular mass incidentally found by the patient himself or its partner by palpation. It shows excellent cure rates based on their chemosensitivity, especially to cisplatin-based chemotherapy, but careful staging at diagnosis, adequate early treatment based on a multidisciplinary approach and strict follow-up are necessary. We present a case of a 25-year-old male patien

Image-enhanced endoscopy for detection of second primary esophageal neoplasms in patients with hypopharyngeal cancer: Prevalence, risk factors, and characteristics.

BACKGROUND: Esophageal second primary neoplasms (ESPNs) are common in hypopharyngeal squamous cell carcinoma (HPSCC) patients and are associated with poor prognoses. The effectiveness of image-enhanced endoscopy (IEE) has not been well established. METHODS: We reviewed the patients between April 2016 and April 2018 with HPSCC receiving ESPNs screening via white-light imaging, narrow-band imaging, and Lugol chromoendoscopy. RESULTS: Of 99 eligible patients, ESPNs prevalence was 31%. Of the 69 patients assigned to the follow-up group, 23 with positive findings showed significantly increased pr

Eosinophilic solid and cystic renal cell carcinoma: A rare under-recognized indolent entity.

Eosinophilic solid and cystic renal cell carcinoma (ESCRCC) is an under-recognized, emerging new entity of sporadic renal neoplasms, with an approximate incidence of 0.2% of renal tumors. A total of 60 cases have been reported in the literature till date. ESCRCC are usually seen in adult females, with a low stage and indolent behavior, and rare incidence of recurrence or metastasis. They are solid and cystic tumors with variably sized cysts resembling eosinophilic RCC, showing a characteristic positive immune-expression for PAX-8, CK20 (in ~80% cases) and/or Melan-A (in ~6.7%), with negative C
