Impulsivity, neural deficits, and the addictions: the "oops" factor in relapse.

Impulsive behaviors are observed in a wide range of psychiatric disorders, including substance use, bipolar, attention-deficit hyperactivity, antisocial and borderline personality, gambling, and eating disorders. The shared phenotype of impulsivity is thought to significantly contribute to both the etiology and perpetuation of these disorders. In this review, we focus upon the relevance of impulsivity to the addictive disorders, particularly substance use disorders. First, the literature supporting the presence of impulsive behaviors prior to the onset of drug use and addiction is discussed. T

[Effects of pricking bleeding at "Weizhong" (BL40) on sciatic nerve conduction velocity and interleukin-1alpha content in the lumbar vertebral nucleus pulposus in experimental lumbar intervertebral disc protrusion rabbits].

OBJECTIVE: To observe the effect of "Weizhong" (BL40) bleeding on sciatic nerve conduction velocity (SNCV) and interleukin-1alpha (IL-1alpha) content of the lumbar vertebral nucleus pulposus tissue in experimental lumbar intervertebral disc protrusion (LIDP) rabbits so as to explore the underlying mechanism of blood letting in the treatment of LIDP. METHODS: Forty healthy New Zealand rabbits were randomly assigned to control, model, BL40 and non-point (1 cm medial to BL40) groups, with 10 cases in each group. LIDP model was established by pushing the intervertebral disc tissue rightward to pr

Symptoms associated with electrophysiologically verified carpal tunnel syndrome in Asian patients.

Sensory symptoms are crucial for the diagnosis of carpal tunnel syndrome (CTS). We have studied the spectrum of sensory symptoms in 67 cases of neurophysiologically confirmed CTS in Singapore and investigated the correlation of semi-quantitative clinical ('Historical-Objective') and neurophysiological ('Bland') scales. The range of sensory symptoms was large. The more frequent use of the terms 'woodenness' and 'aching' rather than 'pins and needles' suggests racial and cultural differences in symptom expression, or recognition of previously ignored descriptors. 'Numbness' (97%) was the most co

On the activation threshold of nerve fibers using sinusoidal electrical stimulation.

Carpal tunnel syndrome (CTS) diagnosis could be improved by selectively activating different types of nerve fibers traversing the carpal tunnel based on their diameter. The objective of this study was to establish the types of fibers activated by different sinusoidal electrical stimuli. The frequencies selected correspond to those used in an available application known as current perception threshold (CPT). This method has been proposed in the literature to assess the severity of CTS in a patient. CPT operates by varying the amplitude and frequency of stimulating sine wave currents. Subthresho

Nerve palsies related to pediatric upper extremity fractures.

In every child who has a fracture, neurologic examination is essential at initial assessment so that early diagnosis of nerve injury can be made. Electrodiagnostic studies may be helpful in diagnosis when the examination is equivocal and in follow-up to look for signs of recovery. In a patient who has neurologic deficits associated with a fracture, nerve exploration should be considered for open fractures, fractures that require open reduction, and palsies that develop after fracture reduction. For closed fractures associated with nerve palsy at the time of initial injury, observation and seri

QSART in idiopathic pure sudomotor failure.

QSART (quantitative sudomotor axon reflex testing) was performed in a patient with idiopathic pure sudomotor failure. Generalized reduction in thermoregulatory sweating and complete absence of axon reflex sweating were observed, suggesting a deficit of sweat gland cholinergic synaptic transmission or receptors. QSART responded promptly to treatment. Putative pathophysiological mechanisms are discussed.

[Carpal tunnel syndrome revealed by digital ulcerations caused by arterial vasospasm].

Carpal tunnel syndrome presenting as a trophic disorder in the hand is unusual. We report the case of a 66 year old man in whom nerve conduction tests confirmed compression of the median nerve in the carpal tunnel, and vasospasm in the corresponding area was demonstrated by arteriography for, we believe, the first time. Simple decompression of the carpal tunnel brought about total recovery. The mechanism of the autonomic disorders found in carpal tunnel syndrome remains controversial. The clinical case that we report objectively demonstrates the vasospasm caused by the compression of the auton

[Clinical techniques for use in neurological physical examinations. I. General organisation, cranial and peripheral spinal nerves].

AIMS: The purpose of this work is to focus on the main practical aspects of the techniques used for the physical examination of the nervous system and to present an approach for the practice of this study in adult patients. DEVELOPMENT: We recommend clinicians to carry out a brief but consistent and effective exploration in a systematic, flexible and orderly manner to check for alterations in the functioning of the nervous system. Should any anomalies be detected, then a more detailed and thorough neurological exploration must be performed selectively. The techniques and data from this examin

Peripheral neuropathy in Behçet's disease.

Behçet's disease (BD) is a multisystem disorder. Since its first description, the involvement of many organ-systems has been studied. However, the involvement of the peripheric nervous system has not been well documented yet. Twenty-six patients involved in the study were without prominent complaints of neuropathic symptoms. They were surveyed through the outpatient clinic of the department of dermatology. Each patient filled out a total neuropathy score (TNS) questionnaire after their neurologic exam. A neurology specialist conducted the electrophysiological study. Our results were noteworthy

Copper-induced changes in locomotor behaviors and neuronal physiology of the freshwater oligochaete, Lumbriculus variegatus.

The behavioral and neurotoxic effects of copper exposure were examined in the freshwater oligochaete, Lumbriculus variegatus. The 24 h LC50 for worms exposed to copper sulfate in an artificial pond water was 0.45 microM. Almost all animals that died due to copper exposure died during the first day of exposure. Immersion in water containing 0.2 or 0.4 microM copper produced time- and concentration-dependent reductions in the ability of tactile stimulation to evoke two stereotyped locomotory behaviors, body reversal and helical swimming. Helical swimming was more severely affected by copper expo

Forearm mixed nerve conduction velocity: questionable role in the evaluation of retrograde axonal atrophy in carpal tunnel syndrome.

The objective of this study was to determine whether forearm mixed nerve conduction velocity (Fmix) reflects the real conduction velocity of forearm motor nerve (Fmot) and forearm sensory nerve (Fsen) fibers passing through the carpal tunnel. Forearm mixed nerve conduction velocity is presumed to be indicative of the conduction velocity of the median nerve over the forearm. Therefore, Fmix is used widely to assess the causes of slowing forearm conduction velocity in carpal tunnel syndrome. However, some authors claim that Fmix comes chiefly from the undamaged fibers in carpal tunnel syndrome,

Prevalence of carpal tunnel syndrome in patients with fibromyalgia.

This study was planned to investigate the prevalence of carpal tunnel syndrome (CTS) in patients with fibromyalgia (FM) and the normal population. Paresthesia in the hands, sensory and motor deficits, and atrophy of the thenar muscles of 50 patients with FM and 50 matched control subjects were evaluated. Tinel's and Phalen's signs and bilateral electrophysiological studies of the median nerves were performed. The differences between the groups in terms of paresthesia (13 FM patients, two control subjects, P<0.01) and sensory deficits (four FM patients, 0 control subjects, P<0.05) were statisti

When to use the combined sensory index.

A recently developed electrodiagnostic technique, the combined sensory index (CSI), has been recommended for its greater sensitivity in diagnosing carpal tunnel syndrome (CTS). The CSI requires a greater number of procedures and therefore involves greater time, cost, and patient discomfort than does conventional electrodiagnostic testing. The CSI is composed of three commonly used electrodiagnostic techniques. There is a close correlation between the components of the CSI, and in most cases, all three components of the CSI are in agreement. We performed a study to develop and validate an algor

Nerve conduit using fascia-wrapped fibrocollagenous tube.

The purpose of this study was to evaluate the application of autogenous fascia as a framework of a fibrocollagen tube for a nerve conduit in Japanese white rabbits. The fascia was wrapped with a double layer around a silicone rod 3.0 mm in diameter. After the implantation of the fascia-wrapped silicone rod into a subcutaneous pocket for 2 weeks, a 3.0 x 30-mm fascia-wrapped fibrocollagen (FFC) tube was prepared. With microvascular techniques, the tube was interposed into a right sciatic nerve gap which was 25 mm long. The results of nerve regeneration in the FFC tube group, in particular, the

Muscle sympathetic nerve activity in patients with lumbar spinal canal stenosis.

The present study aimed to measure sensory nerve conduction velocity (SNCV) and muscle sympathetic nerve activity (MSA) in both normal subjects and patients with lumbar spinal canal stenosis (LSCS), and to determine what sensory and sympathetic nerve systems relate to the development of abnormal sensation in the lower limbs of the patients. The study population was 12 patients and 10 age-matched healthy control subjects. A statistical difference in the mean MSA intervals was found between the LSCS patients and the normal subjects. There was a fairly large difference between them in the values
