Effect of the BFR-7211 on lower limb muscle strength in older adults: A randomized controlled trial.

OBJECTIVE: To evaluate the efficacy of a proprioceptive neuromuscular facilitation (PNF) stretching program integrated into a massage chair (BFR-7211) on lower limb strength, balance, and functional mobility in older adults, as a potential intervention for preventing sarcopenia and enhancing musculoskeletal health. DESIGN: A randomized controlled trial with a 4-week follow-up (12 visits) and 1:1 allocation ratio, conducted without blinding due to the nature of the interventions. SETTING AND PARTICIPANTS: Sixty-two community-dwelling older adults, 65 years and older, from a senior welfare cen

P0.1 is a useful parameter in setting the level of pressure support ventilation.

OBJECTIVE: The purpose of this study was to investigate whether changes in breathing pattern, neuromuscular drive (P0.1), and the work involved in breathing might help to set the individual appropriate level of pressure support ventilation (PSV) in patients with acute respiratory failure (ARF) requiring ventilatory assistance. DESIGN: A prospective, interventional study. SETTING: An 8-bed multidisciplinary intensive care unit (ICU). PATIENTS: Ten patients with ARF due to adult respiratory distress syndrome (ARDS), sepsis or airway infection were included in the study. Chronic obstructive pu

To Breathe or Not to Breathe: Spontaneous Ventilation During Thoracic Surgery in High-Risk COPD Patients-A Feasibility Study.

Background: Spontaneous ventilation with intubation (SVI) during video-assisted thoracoscopic surgery (VATS) has been introduced as a hybrid technique that combines the physiological benefits of spontaneous breathing with the safety of a secured airway. However, its application in patients with chronic obstructive pulmonary disease (COPD) remains controversial due to concerns about hypercapnia, hypoxemia, and dynamic hyperinflation. To date, no study has directly compared COPD and non-COPD patients undergoing VATS lobectomy under SVI using identical anesthetic and surgical protocols. Methods:

Thirty years of paediatric colonic motility assessment modalities: a systematic review.

INTRODUCTION: Disorders of paediatric colonic motility may arise following operative repair of congenital colorectal conditions, as well as spontaneously. This may lead to symptoms of faecal incontinence, in addition to constipation. Management of such typically requires several investigations, clinic appointments and treatment strategies. Different investigation modalities are available to objectively and accurately describe gastrointestinal transit time. In the paediatric setting, most of these investigations are invasive, involve radiation, or require patient hospital admission. This system

Position-specific lower extremity performance and machine learning-based injury-risk screening classification in adult male basketball players: a random forest and SHAP approach.

BACKGROUND: Basketball imposes high position-dependent neuromuscular demands that predispose athletes to lower extremity injuries. Machine learning offers the potential to identify multivariable risk-screening patterns that single-threshold approaches miss. This study aimed to (1) compare position-specific lower extremity performance profiles across five playing positions and (2) develop and explain a Random Forest (RF) classifier of a composite, evidence-based injury-risk screening index in adult male basketball players, interpreted via SHapley Additive exPlanations (SHAP). No injuries were p

Can a writing intervention using mainstream Assistive Technology software compensate for dysgraphia and support reading comprehension for people with aphasia?

BACKGROUND: Stroke profoundly affects quality of life (QOL), including loss of employment, reduced social activity, shrinking social networks and low mood. Dysgraphia (impaired writing) is a common symptom of aphasia yet is rarely targeted in rehabilitation. Recent technological advances might challenge this, since much communication is now conducted digitally through writing. The rehabilitation of writing may therefore help to address the wider consequences of stroke and aphasia. AIMS: Can assistive technology (AT) training for people with dysgraphia: (1) improve written output, and are gain

Aconite poisoning.

INTRODUCTION: Aconitine and related alkaloids found in the Aconitum species are highly toxic cardiotoxins and neurotoxins. The wild plant (especially the roots and root tubers) is extremely toxic. Severe aconite poisoning can occur after accidental ingestion of the wild plant or consumption of an herbal decoction made from aconite roots. In traditional Chinese medicine, aconite roots are used only after processing to reduce the toxic alkaloid content. Soaking and boiling during processing or decoction preparation will hydrolyze aconite alkaloids into less toxic and non-toxic derivatives. Howev

Multidimensional Measurements of Dysarthria in Myotonic Dystrophy Type 1.

BACKGROUND: Myotonic dystrophy type 1 (DM1) is a heterogeneous neuromuscular disorder characterized by progressive muscle weakness and myotonia. Dysarthria is a known symptom of DM1, but literature is lacking about the patient's own perception in relationship to dysarthria characteristics and severity. AIMS: The aim of the study was to describe the acoustic speech characteristics of dysarthria in patients with DM1, examine the perceptually determined dysarthria severity through speech and language therapy assessment, gather subjective evaluations of speech and intelligibility from patients an

[Effect of open-lung ventilation strategy on oxygenation-impairment during laparoscopic colorectal cancer resection].

Objective: After general anesthesia and mechanical ventilation for laparoscopic colorectal cancer resection, about 90% of patients would have different degrees of atelectasis. Authors speculated that an open-lung strategy (OLS) comprising moderate positive end-expiratory pressure (PEEP) and intermittent recruitment maneuvers (RM) can reduce atelectrauma and thus reduce the incidence of oxygenation-impairment during low-tidal-volume ventilation for laparoscopic colorectal cancer resection. The purpose of this study was to verify this hypothesis and provide a better intraoperative ventilation sc

Task-dependent cortical responses to peripheral and central neuromodulation during water and saliva swallowing after stroke: an exploratory fNIRS pilot study.

BACKGROUND: Post-stroke dysphagia is a common and serious complication. Neurorehabilitation strategies include peripheral neuromuscular electrical stimulation (NMES) and central transcranial direct current stimulation (tDCS), which operate via distinct bottom-up and top-down mechanisms, respectively. However, their differential effects on cortical hemodynamic responses during functionally distinct swallowing tasks remain poorly understood. This pilot study aimed to investigate the immediate modulatory effects of NMES and tDCS on cortical hemodynamics during different swallowing tasks in post-s

Transcutaneous Electrical Nerve Stimulation (TENS) Versus Microcurrent Electrical Neuromuscular Stimulation (MENS) Combined with Kinesiotherapy in Partial-Thickness Rotator Cuff Tears: A 12-Month Randomized Controlled Trial.

Background/Objective: Partial-thickness rotator cuff tears represent a distinct, highly prevalent clinical entity governed by a multifactorial continuum of intrinsic senescence and extrinsic biomechanical overload, rather than an oversimplified acute traumatic versus degenerative dichotomy. While conservative management centered on kinesiotherapy is primary, Transcutaneous Electrical Nerve Stimulation (TENS) and Microcurrent Electrical Neuromuscular Stimulation (MENS) are widely utilized as adjunctive electrophysical modalities. However, empirical trials directly comparing their long-term ther

Comparative effectiveness of azathioprine and mycophenolate mofetil for myasthenia gravis (PROMISE-MG): a prospective cohort study.

BACKGROUND: Myasthenia gravis is an autoimmune disorder of the neuromuscular junction. Treatment typically includes symptomatic oral cholinesterase inhibitors, immunosuppression, and immunomodulation. In addition to corticosteroids, azathioprine and mycophenolate mofetil are the most frequently used immunosuppressants in North America. We aimed to evaluate the comparative effectiveness of these two drugs, and to assess the effect of the dose and duration of treatment. METHODS: We did a prospective cohort study at 19 academic centres in Canada and the USA. We included patients (aged ≥18 years)

Interventions for promoting physical activity in people with neuromuscular disease.

BACKGROUND: The World Health Organization (WHO) recommends that people of all ages take regular and adequate physical activity. If unable to meet the recommendations due to health conditions, international guidance advises being as physically active as possible. Evidence from community interventions of physical activity indicate that people living with medical conditions are sometimes excluded from participation in studies. In this review, we considered the effects of activity-promoting interventions on physical activity and well-being in studies, as well as any adverse events experienced by p

Efficacy and safety of aerosolized intra-tracheal dornase alfa administration in patients with SARS-CoV-2-induced acute respiratory distress syndrome (ARDS): a structured summary of a study protocol for a randomised controlled trial.

OBJECTIVES: Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) may trigger severe pneumonia in coronavirus disease of 2019 (COVID-19) patients through release of damage-associated molecular patterns (DAMPs) and recruitment of neutrophils in the lungs. Activated neutrophils induce inflammation and severe alveolar injury by releasing neutrophil extracellular traps (NETs). The backbones of many DAMPs and NETs are made of extracellular, cell-free DNA decorated with highly toxic compounds such as elastase, myeloperoxidase and citrullinated histones. Dornase alfa is a FDA-approved recombin

Interventions for promoting physical activity in people with chronic obstructive pulmonary disease (COPD).

BACKGROUND: Escalating awareness of the magnitude of the challenge posed by low levels of physical activity in people with chronic obstructive pulmonary disease (COPD) highlights the need for interventions to increase physical activity participation. The widely-accepted benefits of physical activity, coupled with the increasing availability of wearable monitoring devices to objectively measure participation, has led to a dramatic rise in the number and variety of studies that aimed to improve the physical activity of people with COPD. However, little was known about the relative efficacy of in
