Relationship between childhood trauma and postpartum psychotic experiences: the role of postnatal anxiety and depression as mediators.

BACKGROUND: Postpartum psychosis (PP) is a psychological emergency requiring rapid intervention, hospitalization and psychiatric management. However, PP has been neglected in the postpartum literature. Understanding the detrimental consequences of childhood trauma across mother's life span is crucial to prevent this serious condition. The study's objectives were to demonstrate the relationship between childhood trauma and postpartum psychotic experiences (PPEs) and to look over the mediating role of postnatal depression (PD) and anxiety (PA) in this relationship. METHODS: This cross-sectional

Associations of postpartum mother-infant bonding with maternal childhood maltreatment and postpartum mental health: a cross-sectional study.

BACKGROUND: After delivery, some women experience impairment of their mother-infant bonding (MIB), which can lead to long-term disturbances of the mother-child relationship and the child's social-emotional development. Little is known about the association between early maternal bonding problems and mothers' own adverse childhood experiences, even though the hypothesis of the intergenerational transmission of caregiving indicates continuity in parenting quality across generations. Therefore, the current study aimed at examining the relationship between maternal childhood maltreatment and postp

Biological stress response in women at risk of postpartum psychosis: The role of life events and inflammation.

BACKGROUND: Postpartum psychosis (PP) is the most severe psychiatric disorder associated with childbirth, and the risk is particularly high in women with a history of bipolar disorder, schizoaffective disorder or in those who have suffered previous episodes of PP. While studies in patients with psychosis not related to the puerperium have demonstrated that abnormalities in stress response are important risk factors for psychosis, it remains unknown whether this is also the case for PP. METHODS: This study includes 30 postpartum women, assessed, on average, at postpartum week 14.8 ± 10.1 eithe

Postpartum Headache: A Prospective Study.

AIMS: To prospectively assess the incidence and etiology (ie, primary vs symptomatic) of headache in women during the first month postdelivery, with particular emphasis on the type of presentation as a clue for identifying potentially harmful etiologies. A secondary aim was to evaluate the relative frequency of migraine- vs tension-type headache in cases of primary headache. METHODS: A total of 900 consecutive women were enrolled in the study and examined within 3 days of delivery, both clinically and with transcranial color-coded sonography (TCCS). During the course of follow-up, all subject

[Predictors of postpartum post-traumatic stress disorder in primiparous mothers].

OBJECTIVES: Identify the potential predictors of postpartum post-traumatic stress disorder (PTSD) in primiparous mothers. MATERIALS AND METHODS: This longitudinal prospective study evaluated the prevalence of obstetric complications and psychiatric disorders (anxiety, depression, psychic dissociation, PTSD, personality) in the immediate postpartum and after 3-6 months. RESULTS: Among 456 hospitalized primiparous mothers, 314 were recruited and 212 followed-up prospectively. The prevalence of PTSD symptoms was high and stable through the postpartum (12.7% versus 13.6%). Significant risk facto

Panic disorder during pregnancy and postpartum period.

BACKGROUND: Earlier studies on the influence of pregnancy and postpartum period on the course of panic disorder have been inconsistent. The present study aims to quantify panic manifestations in these periods in large sample of women. METHOD: Panic manifestations, including exacerbations and new manifestations of panic disorder, were assessed retrospectively in a sample of 128 women with panic disorder with or without agoraphobia, 93 of whom had had 195 pregnancies. RESULTS: Panic manifestations were fewer during pregnancy and more frequent in the postpartum period when compared with the con

Hypothyroidism and women's health.

Hypothyroidism affects 4% to 10% of women, increasing with age. Symptoms, which are often nonspecific and subtle, can include: lethargy, mild weight gain, edema, cold intolerance, constipation, mental impairment, dry skin, depression, irregular menses, hoarseness, myalgias, hyperlipidemia, and bradycardia. TSH determination is usually warranted when some of these are present. Confirmation of diagnosis is based on laboratory tests, not symptoms. With Hashimoto's thyroiditis, the most common cause, the thyroid is usually firm and irregular to palpation. Screening is recommended by the American T

The relationship between condition-specific morbidity, social support and material deprivation in pregnancy and early motherhood. ALSPAC Survey Team. Avon Longitudinal Study of Pregnancy and Childhood.

Poorer health has been consistently associated with both low levels of social support and material deprivation. It has been proposed that social support constitutes a causal link between health and deprivation such that those with lower socio-economic status have poorer health because their lack of social support makes them more vulnerable to disease. This assumption was tested in this study for women moving from pregnancy to early motherhood. The sample of 9208 women was drawn from the Avon Longitudinal Study of Pregnancy and Childhood (ALSPAC). Health status was measured by self-report of mo

Paresthesias and motor dysfunction after labor and delivery.

The incidence of paresthesias and motor dysfunction associated with 23,827 deliveries at Winnipeg Women's Hospital during a 9-yr period (1975-1983) was 18.9/10,000 deliveries. All the symptoms resolved within 72 hr after supportive therapy only. The frequency of paresthesias and motor dysfunction was greater in primiparas, women who had forceps- or vacuum-assisted deliveries, and women who had epidural or general anesthesia. Significant neurologic deficits after labor and delivery with or without epidural anesthesia were rare. Epidural anesthesia is a safe technique in this regard.

Maternal mental health screening and management by health workers in southwestern Uganda: a qualitative analysis of knowledge, practices, and challenges.

BACKGROUND: Maternal mental health (MMH) problems, such as perinatal depression, maternal anxiety, suicide ideation and puerperal psychosis among others, have a significant impact on maternal morbidity and mortality, as well as the health and development of children. One in every four pregnant women and one in every five postpartum women in low-income countries, suffer from maternal mental health (MMH) problems. Despite this, MMH screening, diagnosis, and reporting remain scanty in Uganda. Consequently, this study aimed to investigate the knowledge, practices, and impediments that maternity ca

Cognitive biases in processing infant emotion by women with depression, anxiety and post-traumatic stress disorder in pregnancy or after birth: A systematic review.

Perinatal psychological problems such as post-natal depression are associated with poor mother-baby interaction, but the reason for this is not clear. One explanation is that mothers with negative mood have biased processing of infant emotion. This review aimed to synthesise research on processing of infant emotion by pregnant or post-natal women with anxiety, depression or post-traumatic stress disorder (PTSD). Systematic searches were carried out on 11 electronic databases using terms related to negative affect, childbirth and perception of emotion. Fourteen studies were identified which loo

Autoimmune Encephalitis in Postpartum Psychosis.

OBJECTIVE: Significant immunological alterations have been observed in women with first-onset affective psychosis during the postpartum period. Recent studies have highlighted the possibility that a subset of patients with first-onset severe psychiatric episodes might suffer from undiagnosed autoimmune encephalitis. Therefore, the authors performed a three-step immunohistochemistry-based screening for CNS autoantibodies in a large cohort of patients with postpartum psychosis and matched postpartum comparison subjects. METHOD: Ninety-six consecutive patients with postpartum psychosis and 64 he

[Acute central neurologic complications and the pregnancy-puerperal status. 105 cases in an intensive care unit. Contribution to the study of the relation between pregnancy toxemia and cerebral vascular accidents].

Central neurological pathology in the course of puerperium was studied in 105 observations. One could distinguish: --meeting pathology (tumoral, metabolic, infectious processes) 12 cases, --cerebral vascular accidents (arterial, venous...) 30 cases, These two groups corresponded to a pathology without any obvious connection with gravidic toxemia. --eclamptic or not eclamptic toxemia with encephalopathy: 21 cases, --finally, toxemia associated to or complicated with focused neurological syndromes: 42 cases. Concerning cerebral vascular accidents, one could verify the importance of hemorrhagical
