Sheldon Friel lecture 2026. Trials, quackery and social media: was it all a waste of time.

BACKGROUND: Over the past 40 years, randomized trials have become the mainstay of evidence-based orthodontics. MATERIALS AND METHODS: This paper represents the content of the Sheldon Friel lecture presented in 2026 at the Congress of the European Orthodontic Society. I outline the value of randomized trials and revisit the findings of some of the main studies I have completed. We must also consider the development of patient-relevant outcomes and core outcome sets. While we recognize the value of trials, we must also recognize that evidence-based care is undermined by claims from the orthodon

Machine learning-enabled behavioural and psychological symptoms of dementia management intervention for dementia caregivers: protocol for a hybrid factorial SMART-MRT trial.

BACKGROUND: Behavioural and psychological symptoms of dementia (BPSD) affect over 90% of people living with dementia and are a major contributor to stress and adverse health outcomes among family caregivers. Caregiver‑led behavioural management interventions based on the Antecedents-Behaviour-Consequences (ABC) framework are effective, yet their overall impact is modest. This may be due to the highly individualised and fluctuating nature of BPSD and insufficient adaptive support to sustain engagement. Mobile health interventions offer scalable support; however, engagement and adherence remain

Effects of aquatic exercise on improving body composition and muscle strength in the older adults: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Due to the challenges posed by aging such as decreased physical abilities and higher susceptibility to chronic illnesses, effective exercise interventions are crucial for older individuals. Despite the potential benefits of aquatic exercise, there is a lack of robust evidence supporting their efficacy. To address this gap, a systematic review and meta-analysis were conducted in this study to comprehensively assess the impact of aquatic exercise on muscle strength, body composition, and related physical (flexibility, mobility) and metabolic indicators (lipid profiles) in the older a

Effect of probiotic supplementation on the gut microbiota diversity in healthy populations: a systematic review and meta-analysis of randomised controlled trials.

BACKGROUND: Probiotics are widely used dietary supplements promoted to positively influence gut health and microbiota diversity, making them popular among healthy individuals. One of the purported benefits of probiotics is their ability to enhance gut microbiota diversity, a feature associated with improved resilience and overall health. However, evidence supporting this claim remains inconclusive. We aimed to investigate whether probiotics significantly modify gut microbiota diversity in healthy populations through a systematic review and meta-analysis. METHODS: A systematic search of MEDLIN

Cluster-Randomized Trials - Insights from Cardiovascular Prevention.

AbstractCluster-randomized trials are increasingly relevant to the evaluation of preventive and treatment strategies for chronic disease, especially when interventions are delivered through groups, practices, communities, or health systems rather than individuals. Using cardiovascular prevention as an illustrative example, we review key design, conduct, and analytic issues that determine when cluster randomization is appropriate and how such trials should be designed, analyzed, and interpreted. Particular attention is given to the choice of the unit of randomization, protection of internal val

Effect of Physical Activity on Cognitive Function of Patients With Alzheimer's Disease: A Meta-analysis.

BACKGROUND: Physical activity (PA) is a widely accepted non-pharmacological therapy for patients with Alzheimer's disease (AD). Existing studies have demonstrated that PA can improve cognitive function in AD patients. However, few of the meta-analyses conducted to date have included participants with a confirmed AD diagnosis that meets standardized diagnostic criteria, nor have they systematically evaluated the interactions between different intervention parameters. The aim of this study was therefore to investigate the effects of PA on cognitive function improvement in AD patients, and how di
