cN1A Antibody-Positive Inclusion Body Myositis Following Seminoma Manifesting With Slowly Progressive Paraparesis: A Case Report.

A case of inclusion body myositis (IBM) with autoantibodies against cN1A following orchiectomy for seminoma has not been previously described. A 60-year-old man developed slowly progressive, painless paraparesis of the lower extremities and mild hyper-creatine kinase (CK)emia three years after orchiectomy for a unilateral seminoma. Clinical examination 10 years after onset revealed weakness of hip flexion (Medical Research Council (MRC) 5-), knee flexion (MRC 5-), and foot extension (left MRC 4-, right MRC 5-); absent tendon reflexes; and thigh muscle atrophy. Needle electromyography was norma

Assessment of the Vestibulo-Ocular reflex in polycystic Ovary Syndrome: a Case-Control study using video head impulse testing.

BACKGROUND: Polycystic ovary syndrome (PCOS) is a common endocrine disorder that may affect multiple organ systems. While cochlear and otolith involvement has been investigated, the impact on semicircular canal-mediated vestibulo-ocular reflex (VOR) function remains unclear. AIMS/OBJECTIVES: To evaluate canal-specific, high-frequency VOR function in women with PCOS using the video head impulse test (vHIT) and compare findings with healthy controls. MATERIAL AND METHODS: This single-center case-control study included 42 women with PCOS (Rotterdam criteria) and 35 age-matched controls. vHIT wa

[Neurological examination and neuroanatomical localization in companion rabbits - an important step for diagnosis].

Neurological examination in pet rabbits aims at localizing lesions within the nervous system in order to determine differential diagnoses and appropriate further diagnostics. Due to species-specific behavior and anatomical differences, the standard examination protocol used in small animals requires modification. In 2 published studies involving clinically healthy pet rabbits, several neurological tests were found to be either non-applicable or to require adjustments. The examination includes the assessment of consciousness and behavior, posture, gait, cranial nerve function, postural reaction

[Spinal epidermoid cyst: Iatrogenic neuraxial anesthesia?].

BACKGROUND: Dermoid and epidermoid tumors are rare lesions that affect the skull and spinal canal, with an incidence of ≈ 0.9% of cases. They are benign lesions originating from ectodermal tissue during embryonic development. They may be congenital or acquired, and the latter are usually secondary to iatrogenic causes (such as lumbar puncture, neuraxial anesthesia or surgical procedures) or to penetrating trauma. CASE REPORT: A 40-year-old woman with a history of 2 lumbar punctures for neuraxial anesthesia. 5 years after the last procedure, she started with low-intensity pain in the upper rig

A Convenient Method to Estimate Alignment of Eyes Under General Anesthesia in Strabismus Subjects.

Introduction: This study aims to describe an objective and simple method to estimate the alignment of eyes under general anesthesia (GA) in strabismus subjects, which may vary from the angle measured preoperatively.Materials and Methods: Through the operating microscope, pictures of the eye position of 94 subjects with horizontal strabismus were captured under GA. A curved caliper was placed parallel to the horizontal meridian, and the distance between the center of cornea and the corneal reflex spot of the microscope was measured. The eye position was estimated with a quantified Hirschberg me

"I hope to get some hope": balancing expectations, hope, and acceptance in naturally occurring consultations without a medical diagnosis.

BACKGROUND: Chronic musculoskeletal pain (CMP) is a prevalent condition often lacking clear biomedical explanations, leading to frustration for both clinicians and patients. The uncertainty surrounding CMP deeply affects the dynamics of clinical consultations. AIM: This study investigates how clinicians and patients navigate expectations and uncertainty in CMP consultations, focusing on the interplay between hope and acceptance. METHODS: Nineteen naturally occurring consultations were observed and post-consultation interviews conducted with both patients and clinicians. Data were analysed us

Persistent Hiccups Shortly after Tramadol Intake: A Case Report and Literature Review.

BACKGROUND AND OBJECTIVE: Tramadol can inhibit serotonin and norepinephrine reuptake leading to stimulation of the central component of the hiccup reflex arc. We have found only two previous cases of tramadol-induced hiccups. Additionally, three pharmacovigilance studies have investigated the involvement of tramadol in cases who have developed hiccups as adverse effects. Herein, we have presented a case of a middle-aged male who has developed hiccups shortly after tramadol intake. CASE PRESENTATION: A 35-year-old male complaining of chronic pain in the right knee was treated with tramadol. Th

Vasovagal syncope during ultrasound-guided internal jugular vein cannulation: A case report.

INTRODUCTION: Vasovagal syncope (VVS) is a reflex syncope caused by excessive vagal activation and sympathetic inhibition, primarily manifesting as bradycardia, hypotension, and transient loss of consciousness. Although reports of VVS during ultrasound-guided internal jugular vein catheterization are rare, failure to actively identify and manage this condition may increase the risk of adverse events, warranting high clinical vigilance. PRESENTATION OF CASE: This paper describes a middle-aged male patient who experienced sudden VVS during ultrasound-guided right internal jugular vein catheteri

Neural correlates of executive function and attention in children with ADHD: An ALE meta-analysis of task-based functional connectivity studies.

We conducted a comprehensive meta-analysis of task-based functional MRI studies on executive function and attention in children with attention deficit hyperactivity disorder (ADHD). We searched for studies published before March 20, 2023, in PubMed, Cochrane Library, Web of Science, EBSCO, Embase, and Scopus. The ALE (activation likelihood estimation) method was used to detect differences in brain activation between children with ADHD and comparison children. Fifty-seven (57) studies were included, with 2,231 participants (1,062 children with ADHD). Based on the omnibus meta-analysis findings,

Bilateral Central Foot Drop in a Pediatric Patient.

Foot drop is an inability to dorsiflex the ankle and toe. Primary causes of foot drop are compression or lesion of the 5th lumbar nerve and entrapment of the peroneal nerve at the head of the fibula. Rarely, some central nervous system lesions lead to foot drop. A 16-year-old boy was admitted with blunt head trauma that had happened in an assault. The muscle strength of the bilateral tibialis anterior, bilateral extensor digitorum longus and bilateral extensor digitorum brevis were Medical Research Council grade 1. Deep tendon reflexes of both ankles were hyperactive, with bilateral clonus and

An unusual cause of dementia: essential diagnostic elements of corticobasal degeneration-a case report and review of the literature.

Corticobasal degeneration (CBD) is an uncommon, sporadic, neurodegenerative disorder of mid- to late-adult life. We describe a further example of the pathologic heterogeneity of this condition. A 71-year-old woman initially presented dysarthria, clumsiness, progressive asymmetric bradykinesia, and rigidity in left arm. Rigidity gradually involved ipsilateral leg; postural instability with falls, blepharospasm, and dysphagia subsequently developed. She has been previously diagnosed as unresponsive Parkinson's Disease. At our clinical examination, she presented left upper-arm-fixed-dystonia, spa
