Involuntary treatment in dementia care at home: Results from the Netherlands and Belgium.

AIMS AND OBJECTIVES: To gain insight into the request, use and associated factors of involuntary treatment in people with dementia (PwD) receiving professional home care in the Netherlands and Belgium. BACKGROUND: Most of the PwD remain living at home as long as possible. Due to complex care needs, this can result in an increased risk for care provided against the wishes of the client and/or to which the client resists, referred to as involuntary treatment. DESIGN: Secondary data analyses of two cross-sectional surveys. METHODS: Dementia case managers and district nurses filled in a questio

Behavioral techniques to optimize success of in-office pediatric tympanostomy tube placement without sedation.

OBJECTIVE: Tympanostomy tube insertion is the most common pediatric surgery, but it typically requires general anesthesia. To facilitate in-office tube placement without general anesthesia, two complementary technologies have recently been developed comprising an iontophoresis system for delivering local anesthesia and an integrated tube delivery system. The purpose of this study was to evaluate behavioral support techniques used during a clinical study of the new technology for pediatric in-office tube placement without general anesthesia or physical restraints. METHODS: As part of an IRB-ap

Patellar subluxation at terminal knee extension: isolated deficiency of the medial patellomeniscal ligament.

BACKGROUND: The medial patellomeniscal ligament has been determined in vitro to be the only substantial distal-medial restraint to lateral patellar displacement. The purpose of this study of prospectively collected data was to determine if isolated deficiency of the medial patellomeniscal ligament, resulting in excessive lateral patellar translation and associated symptoms, could be identified. We hypothesized that, if symptoms were associated with isolated deficiency of that ligament, the laxity and symptoms should resolve after plication of that ligament alone. METHODS: Testing for lateral

Addressing structural restrictive practices in disability support organizations.

BACKGROUND: The Convention on the Rights of Persons with Disabilities emphasizes individual rights and autonomy. However, certain restrictive practices in support services for people with intellectual and developmental disabilities (IDD) may limit these rights and undermine autonomy. Structural practices are a type of restrictive practice usually defined as blanket rules or prohibitions that affect all users equally. They are considered restrictive because constrain the freedom of individuals with disabilities. Since these practices are so deeply ingrained in the organizational culture, they o

Emergency Department Hallways Are Associated With Incident Delirium and Severe Agitation in Older Adults Living With Dementia.

BACKGROUND: Geriatric patients awaiting medical admission frequently experience extended emergency department (ED) stays and receive care in ED hallways. While prolonged ED stays are associated with increased delirium and mortality, the effect of placement in hallways remains unknown. This study's objective was to assess whether prolonged ED hallway exposure is associated with incident delirium and severe agitation in older adults and older persons living with dementia (PLWD). METHODS: We conducted a multi-site retrospective cohort database study between January 1, 2022 and December 31, 2023

Trachemys dorbigni as a metal(loid) bioindicator: a study in rural and urban areas.

Metals and metalloids are persistent environmental pollutants with the potential for bioaccumulation, posing significant health risks, including genotoxicity. These contaminants are prevalent in industrial and agricultural runoff. This study utilizes Trachemys dorbigni, an aquatic reptile, as a bioindicator to assess environmental contamination by metals and metalloids in both rural and urban settings in Pelotas, Rio Grande do Sul, Brazil. We captured specimens using pit-type traps with barriers (fyke nets), dividing them into two groups: 15 from a rural area and 15 from an urban area. Each an

Fructose-Rich Diet Attenuates Stress-Induced Metabolic Disturbances in the Liver of Adult Female Rats.

BACKGROUND: Both fructose consumption and chronic stress contribute to the development of metabolic disorders. The consequences of such combination are not fully understood. OBJECTIVE: We investigated whether fructose supplementation and chronic stress synergistically disturb hepatic lipid and glucose metabolism. The role of energy sensing, redox, and inflammatory status during development of metabolic disturbances was investigated. METHODS: Female Wistar rats, aged 2.5 mo, were divided into 4 experimental groups: control (C) fed a standard diet (commercial food and drinking water); fructose

Effects of auditory and visual stimuli on glucose metabolism in Holstein dairy cattle.

BACKGROUND: Standardization of the intravenous glucose tolerance test (ivGTT) in cattle has received little attention despite its widespread use to monitor glucose metabolism. The impact of management practices including several sensorial stimuli on test responses has not yet been described in young cattle. The objective of this study was to analyze the effects of noise exposure, and visual food stimuli in combination with physical restraint on ivGTT and insulin traits in Holstein cattle. A total of 108 ivGTT (6 tests per animal) were performed in bulls (n = 6), steers (n = 6), and heifers (n

11 analytically confirmed cases of mexedrone use among polydrug users.

INTRODUCTION: Mexedrone, 3-methoxy-2-(methylamino)-1-(4-methylphenyl)propan-1-one, is the alpha-methoxy-derivative of mephedrone (4-methyl-N-methyl cathinone). Mexedrone inhibits the re-uptake of serotonin and dopamine in a dose-dependent manner and has affinity for serotonin and dopamine membrane transporters and receptors (5-HT2 and D2 receptors), producing sympathomimetic effects similar to amfetamines. To date there are no published clinical reports on mexedrone use that are analytically confirmed. OBJECTIVE: To characterise the features of mexedrone use in patients who presented to our h

Why peer-led crisis plan practice does - and does not - transform psychiatric ward culture: a realist-informed qualitative study from Japan.

BACKGROUND: Japan's psychiatric inpatient environment - characterized by the world's highest per-capita psychiatric bed rate, entrenched coercive care, and persistent physical restraint - has resisted policy reform. Crisis Plans (CP) are meant to be authored by the service users who may experience a crisis, but in practice, clinicians often write them instead. Peer-led CP practice has been proposed as a catalyst for recovery-oriented transformation, yet how it actually generates organizational change remains theoretically underspecified. AIM: This study aimed to identify the generative mechan
