Background: The distal radius is commonly used as a bone graft donor site for surgery in the hand and wrist. The aim of this study was to evaluate the volume and relative density of cancellous bone in the distal radius. Methods: Thirty-four consecutive computed tomographic scans of the wrist in 33 patients without distal radius pathology were included. For each subject, 6 spherical regions of interest (ROIs) were identified within the distal radius. In each ROI, volumetric measurements and mean Hounsfield unit (HFU) values were recorded by 2 observers using a 3-dimensional imaging reconstructi

BACKGROUND: Because of its form, size and position, the scaphoid is known to be the foundation stone of the carpus, with an exceptionally important biomechanical function. Therefore evaluation of fracture morphology is essential. Experts already agree that computed tomography (CT scan) in the long axis of the scaphoid is the best approach for diagnosing scaphoid fractures, but also for therapy planning. But this method is hardly a standard in most hospitals and studies showing superiority of this technique for therapy planning are non-existent.
HYPOTHESIS: The purpose of this retrospective st

Injuries to the scapholunate joint are the most frequent cause of carpal instability and account for a considerable degree of wrist dysfunction, lost time from work, and interference with activities. Although it is insufficient to cause abnormal carpal posture or collapse on static radiographs, an isolated injury to the scapholunate interosseous ligament may be the harbinger of a relentless progression to abnormal joint mechanics, cartilage wear, and degenerative changes. Intervention for scapholunate instability is aimed at arresting the degenerative process by restoring ligament continuity a

PURPOSE: Scaphoid fracture nonunion remains prevalent, and it was our purpose to examine the initial care, fracture site, and patient gender and age to determine factors contributing to fracture nonunion.
METHODS: The charts of 96 consecutive patients with 99 scaphoid fracture nonunions were reviewed for demographic information, and contact was made with 85 patients (with 88 scaphoid nonunions) to determine the pattern of presentation and initial treatment, if any.
RESULTS: Of the 88 scaphoid nonunions, 78 were in men, and 46 were sports injuries; 7 patients had no recollection of an injury.

Scaphoid fractures are the most common fractures of the carpus, accounting for 79% of all carpal fractures. Early diagnosis of scaphoid fractures is imperative owing to potential complications following the fracture, including non-union, avascular necrosis, carpal instability and osteoarthritis. Plain radiography remains the initial imaging modality to assess scaphoid fractures. Magnetic resonance imaging (MRI) is excellent in the detection of clinically suspected, but initially radiographically negative, scaphoid fractures. Cost-effectiveness analysis studies have demonstrated MRI is effectiv

A retrospective analysis was performed on patients who had undergone surgery for delayed union or non-union of a scaphoid fracture. This was conducted to investigate if there was a difference in outcome between patients treated with the Acutrak screw or the Herbert screw. Twenty two cases were treated with a Herbert screw and 23 with an Acutrak screw. The patient demographics, aetiology of injury and fracture types were similar between the two groups. There was no significant difference in union rate or time to union for fractures treated with the Herbert screw or the Acutrak screw. There appe

We have had favorable results with our early experience using the Mayo modification of the dorsal intercarpal ligament capsulodesis. Our early experience with this technique has shown improvement in carpal alignment and pain relief with the tradeoff of a mild loss of wrist flexion. These results compare with reported results of the various capsulodesis techniques in evolution. Attention must also be paid as to the stage of dissociation at time of surgery. Dynamic dissociation has been treated with capsulodesis alone, whereas a combination of ligament repair in conjunction with dorsal intercarp

Metal implants produce susceptibility artefacts in magnetic resonance imaging. We have explored the effects of scaphoid screw characteristics and orientation on MR susceptibility artefact. Titanium alloy, smallness and longitudinal alignment with the z-axis of the main magnetic field reduce the size of the susceptibility artefact.

We have examined whether the rotatory subluxation of the scaphoid which is seen in patients with advanced Kienböck's disease is associated with scapholunate advanced collapse (SLAC) wrist. We studied 16 patients (11 men, 5 women) who had stage-IV Kienböck's disease with chronic subluxation of the scaphoid. All had received conservative treatment. The mean period of affection with Kienböck's disease was 30 years (14 to 49). No wrist had SLAC. In eight patients, 24 years or more after the onset of the disease, the articular surface of the radius had been remodelled by the subluxed scaphoid with

We investigated the diagnostic value of two different ultrasound scanning methods for the early diagnosis of acute scaphoid fractures. Fifty-seven patients with ten scaphoid fractures were assessed within a week of injury. The accuracy of the ultrasound assessment was 84% and its specificity was 91%. However, its sensitivity was only 50%. We conclude that ultrasound examination is unreliable for the diagnosis of acute scaphoid fractures.

The dorsal scapholunate (SL) ligament may disrupt in variety of ways. Each form of injury requires specific fixation. To investigate the incidence of each type of ligament failure, the records of 45 patients with adequate information, good quality pictures, or videos demonstrating injury characteristics were reviewed. Four types of SL injury were found: type 1 (lateral avulsion from the scaphoid) was the most frequent, present in 19 patients (42%); type 2 (medial avulsion from the lunate) was the least frequent, with 7 cases (16%); type 3 (mid-substance rupture) was found on 9 occasions (20%);

The term osteoarthritis (OA) of the wrist can be used as an umbrella term for various, often independent areas of OA, as the wrist is made up of several joints. Radiocarpal OA often occurs after untreated ligament injuries, incorrectly healed bone fractures in the carpus or after radius fractures involving the joint. A typical sequence of propagation is known for radiocarpal OA following scapholunate (SL) insufficiency or scaphoid pseudarthrosis. Other causes include inflammation, crystal deposits or bone necrosis. Ulnocarpal arthrosis occurs posttraumatically or primarily when there are diffe

PURPOSE: To describe fractures revealed by magnetic resonance imaging (MRI) in a series of skeletally mature patients with radial wrist pain after an acute injury and clinically suspected to have a scaphoid fracture. Additionally, we attempted to assess the diagnostic value of radiographs and computed tomography (CT) in patients with scaphoid and other carpal fractures verified by MRI.
METHODS: We conducted the study prospectively over a 3-year period on skeletally mature patients presenting at our emergency department with tenderness on the radial side of the wrist after an injury. A total o

Some authors recommend using an antirotation wire when performing percutaneous screw fixation of acute non-displaced scaphoid waist fractures. The aim of this study of 21 cadaveric wrists was to assess the usefulness of such a wire in Herbert's B2-type fractures. A B2-type fracture was created experimentally on each scaphoid. An antirotation wire was inserted in eight wrists. Retrograde percutaneous fixation using a double-threaded headless cannulated screw was performed on all wrists. Computed tomography was used to measure interfragmentary rotation. No interfragmentary rotation was noted in

Scaphocapitate syndrome is a rare injury and its incidence in immature skeleton is not well documented. We describe our experience of scaphocapitate syndrome in a 12-year-old boy and report the results after a 3-year follow-up period. Treatment involved open reduction internal fixation using K-wires. The 3-year follow-up evaluation revealed no evidence of avascular necrosis. The wrist was completely asymptomatic and the patient used it normally. The difficulty in examining an injured child and the presence of open physes on radiographs can make diagnosis difficult. Awareness among orthopedic s

PURPOSE: Kirschner wire (K-wire) fixation is one of many strategies for the treatment of scaphoid fractures. The present study investigated the strength and stiffness of convergent and parallel K-wire configurations in a cadaveric model using a simulated nonunion with a 6-mm defect.
METHODS: Eight pairs of cadaveric scaphoids (N = 16) were harvested. Morphological measurements were made to assess the influence of dimensional differences. A 6-mm defect was created in each specimen to replicate scaphoid nonunion. Two parallel or convergent K-wire orientations were used for fixation, and placeme
