Detection of occult sentinel lymph node micrometastases by immunohistochemistry in breast cancer. An NSABP protocol B-32 quality assurance study.

BACKGROUND: Occult metastases, by definition, are not detected on initial examination. They may be present on slides but missed during screening or may be present in paraffin embedded tissue blocks and undetected without additional levels. Anticytokeratin immunohistochemistry (CK IHC) enhances detection of occult metastases, particularly micrometastases (> 0.2 mm but not larger than 2.0 mm) or isolated tumor cell clusters (< or = 0.2 mm). This study defines the rate at which pathologists miss metastases on CK IHC of sentinel lymph nodes (SLN). METHODS: CK IHC sections 0.5 and 1.0 mm from the …

Preoperative lymphoscintigraphy and internal mammary sentinel lymph node biopsy do not enhance the accuracy of lymphatic mapping for breast cancer.

Lymphoscintigraphy (LS) may identify sentinel lymph nodes (SLNs) outside the axilla. Biopsy of these nodes could improve the accuracy of lymphatic mapping (LM) for breast cancer (BC) if a significant number of tumor-positive extra-axillary sentinel nodes are identified. To address this, we evaluated the impact of the use of preoperative LS and biopsy of axillary and internal mammary SLNs in women with BC. From October 1997 to July 2003, 175 women with breast cancer received technetium sulfur colloid, and images were obtained. Isosulfan blue dye was injected intraoperatively, and LM of the axil…

Clinical Impact of Fine Needle Aspiration Cytology on Sentinel Node Biopsy after Preoperative Chemotherapy for Core Needle Biopsy-Proven Metastatic Lymph Nodes.

INTRODUCTION: Sentinel node biopsy (SNB) has been increasingly performed for patients with lymph node (LN)-positive (cN1) breast cancer that converted to LN-negative (ycN0) status after neoadjuvant chemotherapy (NAC). This study aimed to clarify the SNB avoidance rates using fine needle aspiration cytology (FNAC) for metastatic LNs after NAC. METHODS: This study included 68 patients with cN1 breast cancer undergoing NAC from April 2019 to August 2021. Patients with biopsy-proven metastatic clip-marked LNs (clipped LNs) underwent eight cycles of NAC. Ultrasonography (US) was performed to evalu

Validation of a nomogram to predict the risk of nonsentinel lymph node metastases in breast cancer patients with a positive sentinel node biopsy: validation of the MSKCC breast nomogram.

BACKGROUND: Completion axillary lymph node dissection (ALND) remains the standard of care for patients with disease-positive sentinel lymph nodes (SLN). However, approximately two-thirds will have no additional disease-positive nodes. To identify the patient's individual risk for non-SLN metastases, the Memorial Sloan-Kettering Cancer Center (MSKCC) developed a nomogram. METHODS: The records of 182 breast cancer patients who underwent SLN and ALND were selected. Serial hematoxylin and eosin (HE) analysis and immunohistochemistry were routinely performed on each sentinel node. For application …

Sentinel lymph node biopsy in cT1-2N0 lip squamous cell carcinoma: site-specific diagnostic accuracy, anatomic mapping, and survival impact.

BACKGROUND: Optimal management of the clinically node-negative (cN0) neck in early-stage (cT1-2) lip squamous cell carcinoma (SCC) remains controversial. Sentinel lymph node biopsy (SLNB) offers a minimally invasive staging alternative to observation or elective neck dissection, but its diagnostic accuracy, lymphatic mapping patterns, and survival impact in lip SCC are not well-established. METHODS: We conducted a retrospective study of 101 patients with cT1-2N0 lip SCC treated at a tertiary cancer center (2005–2022), comparing SLNB-guided management (n = 46) to observation (n = 55). Diagnosti…

Explainable machine learning versus known nomogram for predicting non-sentinel lymph node metastases in breast cancer patients: A comparative study.

INTRODUCTION: Axillary lymph node dissection (ALND) is the standard of care for breast cancer patients with positive sentinel lymph nodes (SLN), which are the first lymph nodes that drain the breast. However, many patients with positive SLNs may not have additional positive nodes, making the prediction of non-sentinel lymph node (NSLN) metastasis challenging. Reliable prognostic tools are essential for accurately assessing NSLN metastasis. The Memorial Sloan Kettering Cancer Center (MSKCC) nomogram has demonstrated effectiveness in this context, but it requires further evaluation within the Ir…
