Underlying neurological dysfunction in children with language, speech or learning difficulties and a verbal IQ--performance IQ discrepancy.

INTRODUCTION AND AIM: We investigated the relationship between possible underlying neurological dysfunction and a significant discrepancy between verbal IQ/performance IQ (VIQ-PIQ) in children with language, speech or learning difficulties. METHODS: In a retrospective study, we analysed data obtained from intelligence testing and neurological evaluation in 49 children with a significant VIQ-PIQ discrepancy (> or = 25 points) who were referred because of language, speech or learning difficulties to the Multidisciplinary University Centre for Logopedics and Audiology (MUCLA) of the University H

3D markerless tracking of speech movements with submillimeter accuracy.

Speech movements are highly complex and require precise tuning of both spatial and timing of oral articulators to support intelligible communication. These properties also make measurement of speech movements challenging, often requiring extensive physical sensors placed around the mouth and face that are not easily tolerated by certain populations such as young children. Recent progress in machine learning-based markerless facial landmark tracking technology demonstrated its potential to provide lip tracking without the need for physical sensors, but whether such technology can provide submil

Analyzing the Impact of Four Cognitive Constructs on Nonverbal Intelligence Test Performance: Implications for Children With Neurodevelopmental Disorders.

PURPOSE: Children with neurodevelopmental disorders historically exhibit lower and more variable nonverbal intelligence (NVIQ) scores compared to their typically developing peers. We hypothesize that the intrinsic characteristics of the tests themselves, particularly the cognitive constructs they assess, may account for both the lower scores and variability across tests and over time. Using a qualitative content analysis approach, we examined the extent to which key cognitive constructs are engaged in NVIQ tests and how these constructs compare across different tests. METHOD: Current editions

Outcomes of Facial Artery Musculomucosal Flap in Repair of Post-palatoplasty Fistula - A Retrospective Study.

INTRODUCTION: Closure of palatal fistula is difficult due to excessive scarring of the palate after previous surgery. The facial artery musculomucosal (FAMM) flap is considered a better option for palatal fistula closure due to its axial blood supply and minimal donor site morbidity. The aim of this study was to evaluate the results of harvesting FAMM flap for closure of medium and large palatal fistulas in terms of its versatility, reliability, feeding and speech assessment. MATERIALS AND METHODS: A retrospective analysis was performed on patients with post-palatoplasty fistula admitted to t

Telemedicine as a means of effective speech evaluation for patients with cleft palate.

Providing long-term multidisciplinary care for cleft lip/palate is a challenge for international humanitarian organizations that perform surgery across borders. The use of telemedicine as a means of evaluating speech in patients with cleft lip/palate has not been studied previously. We looked at determining whether a speech evaluation performed by a speech-language pathologist (SLP) using telemedicine would be equivalent to a speech evaluation performed in-person, in an international setting between Tijuana, Mexico and San Diego, CA. Spanish-speaking SLPs developed an informal protocol to eval

Discriminating dysarthria type from envelope modulation spectra.

PURPOSE: Previous research demonstrated the ability of temporally based rhythm metrics to distinguish among dysarthrias with different prosodic deficit profiles (J. M. Liss et al., 2009). The authors examined whether comparable results could be obtained by an automated analysis of speech envelope modulation spectra (EMS), which quantifies the rhythmicity of speech within specified frequency bands. METHOD: EMS was conducted on sentences produced by 43 speakers with 1 of 4 types of dysarthria and healthy controls. The EMS consisted of the spectra of the slow-rate (up to 10 Hz) amplitude modulat

Effects of online augmented kinematic and perceptual feedback on treatment of speech movements in apraxia of speech.

Apraxia of speech (AOS) is a motor speech disorder characterized by disturbed spatial and temporal parameters of movement. Research on motor learning suggests that augmented feedback may provide a beneficial effect for training movement. This study examined the effects of the presence and frequency of online augmented visual kinematic feedback (AVKF) and clinician-provided perceptual feedback on speech accuracy in 2 adults with acquired AOS. Within a single-subject multiple-baseline design, AVKF was provided using electromagnetic midsagittal articulography (EMA) in 2 feedback conditions (50 or

Feasible cognition requires latent state.

Intelligent behavior across motor control, speech production, learning, and social cognition operates under uncertainty: sensory feedback is noisy and delayed, actions are imprecise, and observable outcomes often have ambiguous causes. When multiple latent states generate identical or sufficiently similar observations, regulation, learning, and evaluation based on behavior alone become underdetermined. We argue that inference over latent state is therefore a feasibility condition for cognition under partial observability, rather than merely a modeling convenience. Observable signals-movements,

CharMark: A Markov Approach to Linguistic Biomarkers in Dementia.

Dementia, one of the most prevalent neurodegenerative diseases, affects millions worldwide. Understanding linguistic markers of dementia is crucial for elucidating how cognitive decline manifests in speech patterns. Current non-invasive assessments like the Montreal Cognitive Assessment (MoCA) and Saint Louis University Mental Status (SLUMS) tests rely on manual interpretation and often lack detailed linguistic insight. This paper introduces a first-of-its-kind interpretable artificial intelligence (IAI) framework, CharMark, which leverages first-order Markov Chain models to characterize langu

Gradient Porous Flexible Pressure Sensors with the Relay Effect for High-Accuracy Braille-to-Speech Recognition.

The development of highly sensitive, wide linear-range flexible pressure sensors is crucial for practical applications in human-computer interaction, physiological signal detection, and motion monitoring. However, traditional flexible pressure sensors often suffer from limited compressibility in their structural design, resulting in rapid saturation of the detection range and low sensitivity, which hinder their commercial viability. In this study, we propose a straightforward electrospinning method to fabricate a thermoplastic polyurethane/carboxyl-functionalized multiwalled carbon nanotube/po

Factors associated with nonword repetition skills in 4-to 6-year-old deaf and hard-of-hearing and typically hearing children.

Children identified as deaf and hard of hearing (DHH) exhibit an increased risk of speech and language difficulties. Nonword repetition (NWR) is a potential tool for identifying language difficulties in children with limited experience with the target language. In this study, we explored associations between auditory, linguistic, demographic and cognitive factors and NWR performance in DHH children and their typically hearing (TH) peers. We also examined the effect of the group on NWR outcomes when different factors were statistically controlled for. Our study included 68 4- to 6-year-old chil

Implementing a T2L Decoding Feature via Telepractice to Support Single Word Reading by a Young Boy with Down Syndrome: A Case Study.

PURPOSE: Literacy skills are essential for full inclusion in society, particularly for individuals with Down syndrome who have limited intelligible speech, as literacy provides an additional modality for accessing receptive and expressive language. Telepractice provides convenient access to services in the natural context that might otherwise be inaccessible for many individuals. The current case study explored the effect of a telepractice intervention with home practice that consisted of an AAC Transition to Literacy (T2L) decoding feature, designed to support single word reading. METHOD: Th

Need for velopharyngeal surgery after primary palatoplasty in cleft patients. A retrospective cohort study and review of literature.

BACKGROUND: Enabling intelligible speech plays an important role in achieving social inclusion and a good quality of life of cleft patients. A crude measure of primary palatal repair quality is the incidence of operations to correct velopharyngeal insufficiency (VPI) after speech-language therapy has proven inadequate. This study assessed the necessity for surgery to correct velopharyngeal insufficiency following our standardized two-staged protocol, compared the results with the literature, and identified factors that may influence velopharyngeal competence. METHODS: A review of the literatu

Total laryngopharyngectomy with circumferential reconstruction: Helsinki institutional study.

PURPOSE: Surgical complications after total laryngopharyngectomy (TLP) are common, reconstruction is challenging, and patients often lose their ability to swallow and speak. To evaluate these aspects, we analysed outcome after TLP. METHODS: We reviewed all patients who underwent TLP and subsequent circumferential pharyngeal reconstruction through 2004-2017 at the Helsinki University Hospital. RESULTS: For the 26 eligible patients, TLP was the primary treatment for 11 and salvage surgery for 15, followed by reconstruction with free flaps in 22 patients and pedicled flaps in 4. An early (≤ 30

Early developmental delays: neuropsychological sequelae and subsequent diagnoses.

Developmental delay is a frequent diagnosis given to young children when developmental milestones are not met in an age-expected time frame. Research on early delays in speech and motor milestones is unclear regarding possible long-term cognitive functioning. The purpose of this study was to investigate the neuropsychological profile of children who suffered early developmental delays in speech or motor function. Participants (N = 60) completed the Wechsler Intelligence Scale for Children-Fourth Edition, Wechsler Individual Achievement Test-Second Edition, Wisconsin Card-Sorting Test, Children

[Characterization of the linguistic profile of a family with Perisylvian Syndrome].

BACKGROUND: Perisylvian syndrome refers to a variety of clinical manifestations associated to lesions in the perisylvian or opercular regions. Polymicrogyria is the most common structural malformation found. The syndrome may be inherited and the clinical spectrum includes subtle language disturbances on one end and more severe characteristics such as prominent pseudobulbar signs and refractory epilepsy on the other end. Other studies have already associated perisylvian polymicrogyria with developmental language disorders or specific language impairment. AIM: to describe the language deficits
