Treatment of fissure in ano- revisited.

INTRODUCTION: Fissure in ano is a troubling and painful condition that affects a great majority of the population world over. The nature and … st-line treatment in anal fissure. When this fails or fissures recur, lateral subcutaneous internal sphincterotomy should be the preferred options for the treatment of chronic fissure in ano. Nevertheless, all the option should be presented to the patient with complete information about the method, cure rates, complications, and recurrence of the disease.

[The treatment of gallstone disease in the elderly].

Gallstone diseases (asymptomatic, symptomatic and complicated) are frequently seen in the elderly; the prevalence increases proportionally w … patients with an acute cholecystitis. Endoscopic retrograde cholangiopancreaticography (ERCP) with sphincterotomy is also the treatment of choice for common bile duct stones in the elderly. After removal of common bile duct stones (whether or not accompanied by cholangitis or pancreatitis) a laparoscopic cholecystectomy should be performed, unless contraindicat …

Gallstones. Modern management.

BACKGROUND: Management of gallstones has changed as a result of new technologies, such as ultrasound, endoscopic retrograde cholangiopancrea … rgeon. Patients with obstructive jaundice can undergo laparoscopic duct exploration or have an ERCP/sphincterotomy. Those with gallstone pancreatitis should have laparoscopic cholecystectomy within the same hospital admission.

[Histamine-producing neuroendocrine tumor of the ampulla of Vater].

Neuroendocrine tumors of the ampulla of Vater are rare (less than 100 cases reported). We report here a new case characterized by histamine secretion, a hitherto unreported feature. Clinical presentation is similar to that of other tumors of the ampulla of Vater. In our observation, the patient had noticed urticaria on the right forearm for several months. Tumor of the ampulla was confirmed by endoscopic ultrasonography, while neuroendocrine characterization was assessed on biopsies after endoscopic sphincterotomy. Histamine concentration in blood was the only elevated neuroendocrine marker an…

[The long-term results of mechanical anal sphincteroclasis].

BACKGROUND: The long-term results of 36 patients operated of manual dilatation of the anus are presented. Chronic anal fissure or benign anal stricture have been treated by this procedure in the years '80s. METHODS: Patients have been in part clinically examined and the other called up by phone interview. Average follow-up is 78 months. RESULTS: Nobody has been re-operated for the same cause, but 2 patients underwent sphincter sparing operation for rectal cancer. Twenty-five patients (72%) completely recovered, 5 (12.5%) complain sometimes from sudden anal pain and 3 (8%) report minor incont

Management of bile duct stones in the era of laparoscopic cholecystectomy.

The development of laparoscopic cholecystectomy has transformed many aspects of gallstone disease management, particularly the care of patients with known or suspected bile duct stones. New obstacles to operative access to the bile duct have stressed the importance of accurate clinical prediction and detection of bile duct stones and led to increased reliance on nonsurgical approaches, especially pre- or postoperative endoscopic retrograde cholangiopancreatography and sphincterotomy, as well as spurring the development of new techniques such as laparoscopic common bile duct exploration. This w…

Extracorporeal shock wave lithotripsy for obstructing pancreatic duct calculi.

PURPOSE: A review was done to determine the effectiveness of extracorporeal shock wave lithotripsy (ESWL) in the treatment of impacted pancreatic duct calculi. MATERIALS AND METHODS: A total of 19 patients, who were potential candidates for radical pancreatic surgery after unsuccessful endoscopic retrograde cholangiopancreatography, sphincterotomy and attempted stone extraction from the pancreatic ducts, underwent ESWL of the calculi. Followup ranged from 6 months to 6 years. RESULTS: Of the 19 patients 14 avoided a major operation and 6 have remained pain-free for the long term. Two patient…

[Endoscopic retrograde cholangiopancreatography combined with laparoscopic cholecystectomy].

In 11 of 82 patients undergoing laparoscopic cholecystectomy, preoperative endoscopic retrograde cholangiopancreatography (ERCP) was performed. Indications were biliary pancreatitis in 5 and suspected common bile duct (CBD) stones in the other 6 (based on US or liver function tests, or both). The biliary tree was normal in 9 and laparoscopic cholecystectomy was performed. In 8 there was uneventful recovery but in the ninth open surgery was required when the attempted cholecystectomy resulted in uncontrolled cystic artery bleeding. Of the remaining 3 patients, 1 underwent sphincterotomy via ERC…

[How great is the risk in surgical treatment of choledocholithiasis?].

With a success-rate of approximately 95 percent non-operative removal of bile duct stones following endoscopic sphincterotomy is a valuable alternative to operative treatment. If the gallbladder has not been removed previously an acute cholecystitis may develop in up to 20 percent of the patients. Approximately 50 percent of complications following non-operative treatment require emergency surgery. The most serious complications following operative treatment are retained bile duct calculi (2.8%), wound infection and biliary fistulae. In our experience with 499 patients postoperative morbidity …

Endosonography: promising method for diagnosis of extrahepatic cholestasis.

Endosonography, ultrasonography, and computed tomography (CT) were carried out prospectively in 52 patients with extrahepatic cholestasis. 35 patients had extrahepatic biliary obstructions (21 tumorous, 14 non-tumorous) and 17, with recent gallstone migration within the bile duct, had no extrahepatic obstruction at the time of investigation. The definitive diagnosis was established by surgery (in 39 patients), by transendoscopic sphincterotomy (11 patients), or by retrograde biliary opacification (2 patients). Endosonography was significantly more sensitive than ultrasonography or CT (100% vs …

Endoscopy in the diagnosis and treatment of benign stenosis of the papilla of Vater.

Of 921 endoscopic sphincterotomies (EST) performed by the authors, 110 (12%) were done for benign stenosis of papilla of Vater. Based on these data, they consider the possibilities of endoscopic diagnosis and treatment in the stenosis of papilla. One of the conditions for the diagnosis of papilla stenosis is the detectability of dilated biliary tract by endoscopic retrograde cholangiography. But this cannot be taken as a specific sign because, e.g., the biliary tract may become dilated in spite of normal sphincter tone after gall-bladder removal. At the same time, biliary tract dilation is a d

Chronic Anal Fissure.

Diagnosis of chronic anal fissure is easy and common in clinical practice. Little is known about the etiology and pathogenesis of this disor … ence. Surgical treatment is the most effective procedure for chronic anal fissure. Lateral internal sphincterotomy achieves healing in most cases (more than 95%) and the recurrence rate is low (1% to 3%). However, permanent fecal incontinence may appear after surgery and available data about this complication are controversial. In recent years, chemical sphincte …

Botulinum toxin A injection for chronic anal fissure: A retrospective observational study.

Chronic anal fissure (CAF) is a common anorectal disorder associated with severe pain and impaired quality of life. Botulinum toxin A (BTA) injection has emerged as a minimally invasive alternative to lateral internal sphincterotomy; however, data regarding high-dose Dysport® protocols and the clinical relevance of booster injections remain limited. This retrospective single-center observational study was conducted at the General Surgery Clinic of Mersin City Training and Research Hospital. Patients treated for CAF between January 1, 2025, and December 1, 2025, were identified through institut…

Comparable Long-term Outcomes of 1-Minute vs 5-Minute Endoscopic Papillary Balloon Dilation for Bile Duct Stones.

BACKGROUND & AIMS: Endoscopic papillary balloon dilation (EPBD) is an alternative to endoscopic sphincterotomy for choledocholithiasis. Unlike endoscopic sphincterotomy, EPBD preserves biliary sphincter function, reducing long-term risk of recurrent choledocholithiasis by 50%. Guidelines recommend that duration of EPBD exceeds 2 minutes, to adequately loosen the sphincter and reduce risks of failed stone extraction and post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis. However, it is unclear whether this long duration of EPBD impairs sphincter function and negates the lon…
