Post-lumbar surgery complex regional pain syndrome.

BACKGROUND: This case report identifies a patient with complex regional pain syndrome Type 2 (causalgia) with sympathetically maintained pain in a distal extremity associated with an anterior sacroiliac fusion with local bone graft. CASE DESCRIPTION: A sacroiliac fusion was undertaken due to chronic back pain that was temporarily relieved with percutaneous injections to the joint space and lower lumber facets. A 65 year-old man underwent a left sacroliac arthrodesis for continued lumbosacral pain. The successful surgery with allograft had no intraoperative complications. The patient remained

Hypoglossal-facial nerve anastomosis: a review of forty cases caused by facial nerve injuries in the posterior fossa.

Hypoglossal-facial anastomosis has been our procedure of choice in the repair of the permanently injured facial nerve in the cerebellopontin … e had corrective cosmetic surgical procedures until the past few years. We have never used cervical sympathectomy to reduce the size of the palpebral fissure. Better surgical procedures to correct both extracranial and intratemporal facial nerve injuries have significantly reduced the indication for anatomosis procedures. The actions of bretylium: adrenergic neurone blocking and other effects.

Bretylium caused a specific and lasting depression of many excitatory and inhibitory responses evoked by electrical stimulation of the peripheral sympathetic nervous system, probably by impairing conduction of impulses in adrenergic neurones with consequent failure of noradrenaline and adrenaline release. This effect, which will be referred to as the adrenergic neurone blocking action, was preceded by weak sympathomimetic effects. In the presence of bretylium the effects of adrenaline and noradrenaline were increased, as after sympathectomy. Concentrations producing blocking of adrenergic neur…

[Epidural spinal cord stimulation in therapy-resistant angina pectoris].

Spinal cord stimulation (SCS) has routinely been used since the beginning of the 1970s. The initial indications for stimulation were the so-called deafferentation or neurogenic pain. Further work has confirmed that neurostimulation is useful in severe peripheral vascular disease in relieving pain and increasing capillary blood flow and oxygen tension. The effects are similar to those of sympathectomy. In 1964 Apthorp et al. discovered that sympathectomy relieved angina in about 75% of patients. The use of SCS to treat angina follows logically from its use in peripheral vascular disease. METHOD…

Renal denervation for managing hypertension: Key elements in selecting and referring patients.

Renal denervation (RDN) is an innovative procedure with regulatory approval for treating patients whose blood pressure remains uncontrolled despite lifestyle changes and the prescription of antihypertensive medications. It is important to consider how to create accessible patient referral pathways for this complementary therapy in the context of differing healthcare systems and the availability of appropriate resources. Today, most hypertensive patients are managed by primary care practitioners in the community. This paper considers various program models that integrate RDN into clinical pract

Increased plasma levels of erythropoietin after renal denervation in patients with resistant hypertension.

Erythropoietin (Epo) plays a crucial role in the formation and maturation of erythrocytes and is produced primarily by peritubular cells in the renal cortex. Previous studies suggest that renal sympathetic nerve activity (RSNA) may influence Epo regulation. Catheter-based renal denervation (RDN) has been shown to reduce renal noradrenaline spillover and blood pressure in patients with resistant hypertension. We therefore aimed to investigate whether RDN influences Epo levels. 33 patients with resistant hypertension (age 61 ± 12 y; baseline office blood pressure (BP) 165 ± 16/85 ± 18 mmHg despi

Cost-effectiveness of radiofrequency renal denervation for uncontrolled and resistant hypertension in The Netherlands.

INTRODUCTION: Radiofrequency renal denervation (RF RDN) has been recommended as an adjunct therapeutic option for uncontrolled including resistant hypertension. This analysis sought to evaluate the cost-effectiveness of RF RDN treatment in the Dutch healthcare setting. METHODS: A previously published decision-analytic model was utilised to project outcomes over a lifetime horizon for RF RDN vs. standard of care (SoC). Data from the SPYRAL HTN-ON MED study informed the cohort characteristics and base case treatment effect modelled (-4.9 mmHg office-based systolic blood pressure (oSBP) reductio

Safety and Efficacy of Renal Sympathetic Denervation Using Percutaneous Radiofrequency Ablation, a New Hypertension Treatment.

RATIONALE AND OBJECTIVES: Conventional catheter-based renal sympathetic denervation (RDN) as a treatment for patients with treatment-resistant hypertension cannot ablate renal sympathetic nerves ≥4 mm from the renal artery. We developed a percutaneous radiofrequency ablation (pRFA) method to ablate remote renal sympathetic nerves. This study aimed to assess the feasibility, efficacy, and safety of RDN with pRFA. MATERIALS AND METHODS: Fourteen Beagle dogs were divided into RDN and control groups. The RDN group underwent computed tomography (CT)-guided pRFA to ablate the sympathetic nerves aro

Pulmonary Artery Denervation for Idiopathic Pulmonary Arterial Hypertension: A Case Report.

Pulmonary arterial hypertension (PAH) is a life-threatening condition characterized by significantly elevated pulmonary artery pressure (PAP) and resistance. While targeted drugs can alleviate symptoms, their long-term efficacy is limited, and treatment is often associated with notable side effects and financial burden. Percutaneous pulmonary artery denervation (PADN) offers a novel interventional approach for PAH patients with inadequate responses to targeted therapy by selectively ablating sympathetic nerves and restoring neurohumoural balance. This case report describes a 67-year-old male w

Association of primary focal hyperhidrosis with anxiety induced by sweating: A cross-sectional study of Japanese university students focusing on the severity of hyperhidrosis and site of sweating.

Hyperhidrosis significantly reduces patients' quality of life, with many reporting feeling highly anxious. However, the relationship between hyperhidrosis and anxiety induced by sweating has not been examined in detail. The current study examined the relationship between: (1) the presence of hyperhidrosis symptoms, (2) hyperhidrosis severity, and (3) the sites of the most sweating and anxiety induced by sweating. A cross-sectional web-based survey was conducted among university students, and 1080 consenting participants (600 males and 480 females; mean age, 18.8 years) were included in the ana

Device-based therapies for arterial hypertension.

Arterial hypertension is the most prevalent modifiable risk factor associated with cardiovascular morbidity and mortality. Although antihypertensive drugs are widely available, in many patients blood pressure control to guideline-recommended target values is not achieved. Several device-based approaches have been introduced to lower blood pressure; most of these strategies aim to modulate autonomic nervous system activity. Clinical trials have moved from including patients with resistant hypertension receiving intensive pharmacological treatment to including patients with mild-to-moderate hype

Renal sensory nerves increase sympathetic nerve activity and blood pressure in 2-kidney 1-clip hypertensive mice.

Renal denervation lowers arterial blood pressure (ABP) in multiple clinical trials and some experimental models of hypertension. These antihypertensive effects have been attributed to the removal of renal afferent nerves. The purpose of the present study was to define the function, anatomy, and contribution of mouse renal sensory neurons to a renal nerve-dependent model of hypertension. First, electrical stimulation of mouse renal afferent nerves produced frequency-dependent increases in ABP that were eliminated by ganglionic blockade. Stimulus-triggered averaging revealed renal afferent stimu
