Although the semitendinosus and gracilis tendons have long been used in ligamentous reconstruction procedures of the knee, their anatomic relationships have not been explicitly detailed. Therefore, cadaveric dissections were performed on fresh-frozen adult knees to examine these relationships. Several key anatomic points are useful in the harvest of these tendons. Their conjoined insertion site is medial and distal to the tibial tubercle. They become distinct structures at a point that is farther medial and slightly proximal. Tendon harvest is facilitated by identifying the tendons proximal to

OBJECTIVE: To ascertain the possibility of non-uniform stress within the achilles tendon due to individual force contributions of the triceps surae. DESIGN: Calculation of non-uniform stress through discrepancies in moments about the ankle joint. BACKGROUND: Non-uniform stress over the cross-sectional area have been implied in the etiology of achilles tendon injury and may influence functional aspects. However, this has not been empirically demonstrated. METHODS: In vivo achilles tendon forces were measured with an optic fibre technique during isometric plantarflexions at systematically varied

In the present case, we describe the first reported instance of a noncontact distal adductor magnus avulsion injury in a collegiate Division I football player. A 22-year-old male player initially presented with an acute posterolateral corner injury with partial thickness tears of the fibular collateral ligament (FCL), biceps femoris tendon (BFT), and popliteus tendon (PT). Appropriate management was completed. Two months later, without contact or a specific inciting event during gameplay, his condition worsened to an acute full-thickness tear of the ischiocondylar (hamstring) portion of the ad

Bilateral patellar tendon ruptures are exceedingly uncommon, especially when they occur in individuals without predisposing risk factors or systemic disease. Due to its rarity, many cases are missed on initial presentation resulting in poor patient outcomes. Identifying associated risk factors aids in diagnosis and mitigates this oversight. We report a case of a healthy, recreational weightlifter who sustained bilateral patellar tendon ruptures during an acute high-loading resistance exercise bout. We discuss how a spike in acute workload may have predisposed our patient to this injury. Resear

BACKGROUND Contracture is related to modulation of passive stiffness in muscle and tendon after spinal cord injury (SCI). Current clinical assessments of stiffness in muscles and tendons are subjective in patients with spinal cord injury. We proposed a quantitative method to evaluate stiffness of the gastrocnemius and Achilles tendon (AT) with a portable device, the MyotonPRO. The purpose of this study was to investigate the intraoperator and interoperator reliability of the MyotonPRO when used in patients after spinal cord injury. MATERIAL AND METHODS Fourteen patients with SCI participated i

The rate of rotator cuff injury repair and retear is high in elderly patients due to the combination of different degrees of osteoporosis. To solve this problem, many surgeons try to reduce retear rate of rotator cuff injuries in these patients by increasing the initial fixation strength of anchors and changing local bone conditions. The rapid advances of tissue engineering have made it possible to use growth factors as an aid. However, repair of rotator cuff injury with osteoporosis is still a great challenge for clinical workers. How to better increase anchor fixation strength, improve micro

Rotator cuff injuries frequently require surgical repairs which have a high failure rate. Biological augmentation has been utilized in an attempt to improve tendon repair. Poly-N-acetyl glucosamine (sNAG) polymer containing nanofibers has been shown to increase the rate for healing of venous leg ulcers. The purpose of this study was to investigate the healing and analgesic properties of sNAG in a rat rotator cuff injury and repair model. 144 adult male Sprague-Dawley rats underwent a transection and repair of their left supraspinatus tendons. Half of the animals received a sNAG membrane on the

Magnetic resonance imaging (MRI) of the foot and ankle is useful for the diagnosis and staging as well as the surgical planning of multiple disorders: first, for osseous injury, particularly stress fractures and occult fractures, which appear like bone bruises but behave more like fractures. Then, for the Achilles tendon, MR is useful for staging the chronicity and extent of damage, as well as whether the tendon can be surgically repaired or reconstructed. For the diagnosis of posterior tibialis tendon tears and injuries, MR is an important tool in surgical planning. Often, MR is helpful for t

INTRODUCTION: Flexor tendon injuries of the hand often result in significant functional impairment. Rehabilitation protocols after surgical repair are critical for optimizing outcomes, yet the comparative effects of active versus passive rehabilitation remain debated. Shear wave elastography (SWE) offers an objective imaging method to monitor tendon stiffness, but its role in hand tendon recovery is not well established. This study aimed to compare functional outcomes between active and passive rehabilitation protocols, while exploring the potential utility of SWE in monitoring tendon healing.…

Rotator cuff injuries are one of the more common injuries that occur in the shoulder. Four muscles and tendons make up the rotator cuff. These include the supraspinatus, infraspinatus, subscapularis, and the teres minor. The teres minor is the smallest of the rotator cuff muscles. It originates from the middle third of the lateral border of the scapula, just below the insertion of the teres major. It inserts onto the inferior facet of the greater tubercle of the humerus. The teres minor is innervated by the axillary nerve, and its muscular action is that of external rotation, and it can be a w

INTRODUCTION: Prior studies of hamstring tendon tears have reported varied findings on whether increased delay from injury to surgery is associated with worse outcomes. The purpose of this study was to determine whether increased time from injury to surgical repair is associated with worse clinical outcomes in patients with proximal hamstring ruptures.
MATERIALS AND METHODS: Patients who underwent surgical repair of a proximal hamstring rupture from 2010 to 2019 were followed for a minimum of 24 months from surgery. A cutoff of 6 weeks from injury to the time of surgery was used to distinguis

Chronic ruptures of the Achilles tendon are often missed injuries, which is challenging for the surgeon. The complications from reconstruction are a considerable concern. Primary repair may be attempted, but the missed injury often presents later than 4 weeks with gaps greater than 4 cm, necessitating more complex reconstructions using local tissues such as turn-down flaps and VY plasty, requiring large incisions in an unfavorable area of the body. We describe a step-by-step technique of endoscopic flexor hallucis longus reconstruction for chronic Achilles rupture, which decreases local compli

BACKGROUND: Achilles tendon ruptures often result in long-term functional deficits despite accelerated (standard) rehabilitation.
PURPOSE/HYPOTHESIS: The purpose of this study was to investigate if delayed loading would influence functional, clinical, and structural outcomes of the muscles and tendon 1 year after a surgical repair. It was hypothesized that delaying the loading would reduce the heel-rise height deficit 1 year after Achilles tendon rupture.
STUDY DESIGN: Randomized controlled trial; Level of evidence, 1.
METHODS: In total, 48 patients with a surgically repaired Achilles tendo

BACKGROUND: Evidence supports use of the relative motion extension (RME) approach following extensor tendon repairs in zones V-VI yielding good or excellent outcomes.
PURPOSE: To demonstrate how a 3-year internal audit and regular review of emerging evidence guided our change in practice from our longstanding use of the Norwich Regimen to the RME approach using implementation research methods. We compared the outcomes of both approaches prior to the formal adoption of the RME approach.
STUDY DESIGN: Prospective clinical audit.
