Tuberculosis of the spine (Pott's disease) is the most common skeletal involvement in tuberculosis, and is often accompanied with neurological deficiency. Surgical treatment was previously the mainstay of therapy in cases of Pott's disease with neurological deficiency, but since clinical recovery was found in patients awaiting surgery, conservative management with antibiotic treatment as a first Line treatment has been advocated. This conservative approach may improve the patients' neurological status while avoiding the morbidity and mortality of surgical treatment.

BACKGROUND: Although modified electroconvulsive therapy (ECT) may rarely result in musculoskeletal complications, it has also been safely administered to patients with orthopedic morbidity. There is no literature on the safety of repeated courses of ECT in patients with Harrington rod implants.
CASE REPORT: We describe a 30-year-old asthenic bipolar woman who, in 1997, had a Harrington compression assembly stabilizer fitted for spinal tuberculosis and who, between 2000 and 2004, received 3 courses of modified ECT with a total of 28 treatments. In October 2006, she experienced her index episod

Tuberculosis is a common disease in Africa, Asia, and South America. In Denmark the incidence is 10/100,000 and highest in HIV positive patients, alcoholics, and some immigrant groups. We describe a young Dane, who was referred to the department of rheumatology for back pain. It turned out to be tuberculosis spondylitis with a major psoas abscess.

In adults, infectious spondylitis is a rare but severe disease, caused by a bacterial thrombus in tissue of reduced resistance. In conventional radiographs initial findings are a narrowing of the intervertebral space, local osteoporosis and poorly defined erosive borders of the vertebral endplates. These changes can be found at least three to six weeks after the onset of disease. However, in Szintigraphy and MRT pathologic alterations are evident after ten to twelve days. Thus, early diagnosis and treatment becomes possible. In early stages of the disease a localized lysis surrounded by a reac

We reported a case of chronic constrictive pericarditis complicated with silicosis and lumbar caries, who was improved by the operation. The patient was a 65 year old man whose past occupation was a mason. He was admitted to our hospital with chronic heart failure on March, 1986. Atypical silicosis was diagnosed from the occupational history and the histopathological silicotic changes in mediastinal lymph nodes and fibrosis of alveolar wall. The diagnosis of chronic constrictive pericarditis was made from chest roentgenogram and intracardiac catheterization. The symptoms of chronic constrictiv

Nowadays, early diagnosis of spondylitis is possible. Technetium and gallium scintigrams are positive soon after the onset of the disease, while radiographs remain negative for weeks or even months. Blood and urine cultures and serologic tests may provide indications about the underlying infectious agent; however, needle biopsy establishes a precise bacteriologic diagnosis in up to 65% of the cases. Therefore, needle-biopsy is considered to be the most valuable diagnostic measure in spondylitis. Conservative treatment is indicated in cases of minimal destruction of the vertebral body. Surgery

BACKGROUND: Atlantoaxial tuberculosis (TB) is rare in clinical practice, accounting for only about 0.3%-1% of spinal TB. An anterior-only surgical approach cannot provide strong fixation, whereas a posterior approach cannot achieve complete removal of lesions. A method combining anterior and posterior approaches to treat atlantoaxial TB is advisable. The aim of this study was to evaluate the effectiveness of anterior transoral débridement combined with posterior fixation and fusion for atlantoaxial TB.
METHODS: Clinical data of 20 patients with atlantoaxial TB who underwent anterior transoral

Pertussigen, one of the biologically active proteins from Bordetella pertussis, was found highly active as an adjuvant to promote the induction of experimental allergic encephalomyelitis (EAE) in (SJL X BALB/c)F1 mice that had received at the same time an injection of mouse spinal cord (MSC) homogenized in complete Freund's adjuvant containing 4 mg of Mycobacterium tuberculosis H37RA per milliliter (CFA-H37). In this system 2 mg of MSC induced EAE, but a dose of 4 mg was more effective. As little as 250 ng of pertussigen facilitated induction of EAE, and 400 ng uniformly did so. Pertussigen wa

An algorithm of stepwise neurorehabilitation has been developed for patients with vertebrogenic myelopathy. The algorithm is constructed as a hierarchy of parallel and series tasks to recovery movement capacities of a paralyzed patient. Their solution requires the use of original procedures, such as initiation of locomotor activity by dermal and epidural spinal cord electrostimulation, propriospinal stimulation, dynamic vertical posture training, as well as treadmill therapy, tetrapedal walk, and therapeutic exercises. Neurorehabilitative treatment by the proposed algorithm was performed in 41

In this paper, a case is reported of intradural-extramedullary tuberculoma of the spinal cord which developed symptoms of spinal cord compression. Surgical removal of the lesion was followed by complete recovery.

Four patients with spinal epidural sepsis were evaluated with MR imaging. The lesions were best visualized with spin-echo techniques with long repetition (2000 msec) and long echo (80-100 msec) times. Sagittal and axial images were equally important in defining the extent of the lesions. Comparison with available contrast-enhanced CT scans showed that MR was more definitive in the early demonstration of the abscesses. This early recognition influenced the management greatly and improved the clinical outcome significantly. The findings in our four cases support previous reports that MR is super
