Intra-abdominal abscess caused by toothpick injury.

We present the case of a 42-year-old female who presented to our emergency department (ED) complaining of epigastric pain for four days. She had been seen in the outpatient department and ED previously for evaluation, but continued to experience epigastric pain with fever. Emergency panendoscopy was performed and a toothpick was discovered impacted in the duodenal bulb. The gastroenterologist was unable to remove the toothpick endoscopically. Computed tomography of the abdomen revealed a long and straight hyperdense foreign body, and intra-abdominal abscess formation. An emergency laparotomy w

[Abdominal aortic injury as a complication of laparoscopic cholecystectomy].

A 58-year-old woman with a history of high blood pressure and cesarean section by median laparotomy underwent elective laparoscopic cholecystectomy under general anesthesia. After initiation of pneumoperitoneum, hemodynamic instability developed (blood pressure 75/40 mm Hg, heart rate 47 beats/min) and resolved after volume replacement and administration of vasoactive drugs. The rest of the procedure was completed without further adverse events. After extubation, the patient was transferred to the anesthetic recovery ward, where she tended to hypotension that was unresponsive to volume replace

[The problem of the closed abdominal injury in polytrauma patients].

The histories of 66 patients with blunt abdominal trauma requiring surgery in the period from 1985 to 1989 were analysed. The patients were divided into three groups on the basis of the other injuries present. Group I, isolated blunt abdominal trauma and blunt abdominal trauma with slight concomitant injuries (18 patients, ISS 17.17 +/- 1.40); group II, blunt abdominal trauma with severe concomitant injuries but without craniocerebral trauma (23 patients, ISS 29.34 +/- 1.45); and group III, blunt abdominal trauma with severe concomitant injuries and an additional craniocerebral trauma (25 pati

Traumatic abdominal wall hernias and intra-abdominal organ injuries in children: A retrospective cohort study using the National Trauma Data Bank.

BACKGROUND: Traumatic abdominal wall hernias (TAWHs), though rare in pediatric trauma, may signal severe associated intra-abdominal injury, particularly hollow viscus injury (HVI). This study evaluated whether pediatric TAWH is associated with a higher incidence of HVI and solid organ injury (SOI) compared with abdominal wall hematoma or ecchymosis. METHODS: We performed a retrospective analysis of the National Trauma Data Bank (2018-2022), including patients 18 years of age or older with TAWH or abdominal wall hematoma/ecchymosis. Bivariate statistics and multivariable logistic regression as

Selective Non-Operative Management After Abdominal Gunshot and Blast Injuries in A Porcine Model.

The role of selective non-operative management (SNOM) after gunshot and blast injuries has been increasingly recognized but not explored in animal models. Overall, 24 male pigs were prepared, of which 12 were used to create a gunshot wound model, and the remaining 12 a blast injury model. Animals in each injury model were randomly divided into immediate surgery and SNOM groups using a computer-generated random sequence. Gunshot injury was created by firing a 7 mm, 1.45 g steel ball at the right upper abdomen from 10 cm under 170 bar. Blast injury was created by exposing the right upper abdomen

Timing of thromboprophylaxis in patients with blunt abdominal solid organ injuries undergoing nonoperative management.

BACKGROUND: Decision making regarding the optimal timing for initiating thromboprophylaxis in patients with blunt abdominal solid organ injuries (BSOIs) remains ill-defined, with no guidelines defining optimal timing. In this study, we aimed to evaluate the relationship of the timing of thromboprophylaxis with thromboembolic and bleeding complications in the setting of BSOIs. METHODS: A retrospective analysis of the Trauma Quality Improvement Program database was performed between 2013 and 2016. All patients with isolated BSOIs (liver, spleen, pancreas, or kidney, Abbreviated Injury Scale sco

Obesity is associated with a reduced odds for blunt intra-abdominal injuries in children.

OBJECTIVES: Children with obesity may possess unique injury characteristics that may affect their emergency care. To better understand this relationship, we investigated the association of obesity in pediatric trauma patients and intra-abdominal injuries (IAIs) and routinely utilized emergency department (ED) diagnostic procedures (computed tomography (CT) scans and ultrasound (US) examinations). METHODS: This secondary data analysis utilized Pediatric Emergency Care Applied Research Network (PECARN) data from 2007 to 2010. Since height data were not available, children (2-17 years) with obes

Seat belt injuries of the abdominal aorta in adults--case report and literature review.

Blunt abdominal trauma with major vascular involvement is found to be rare. Although few series have been reported in the literature, the true incidence of blunt abdominal aortic injury is unknown. Different modalities of blunt trauma may occur among civilians with steering wheel and seat belt injury secondary to motor vehicle accident the most frequent. Mechanical forces produce variable patterns of injury; therefore, the onset of signs and symptoms can be different. Dissection and thrombosis of the abdominal aorta have been frequently described among seat-belted adult patients with major vas

Small bowel injuries in penetrating abdominal trauma during war: ten-year follow-up findings.

BACKGROUND: Injuries of the small intestine are common in penetrating abdominal trauma. This article presents 10-year follow-up results for 23 patients with penetrating small bowel injuries who were treated in Nova Gradiska City Hospital during the 1991-1992 war in Croatia. The early hospital mortality rate was 13% (three deaths), and good results were found for 16 (84%) of 19 patients after 10 years. METHODS: The hospital charts of 23 patients who sustained small bowel injuries during an 8-month period were reviewed. Of 20 patients who survived, 19 came for an examination and interview 10 ye

Spontaneous rupture of abdominal aortic aneurysms in patients with non-related blunt traumatic injuries.

As the population ages, abdominal aneurysms are expected to increase in frequency, as well as in the number of elderly patients involved in automobile trauma. Therefore, the number of incidental abdominal aortic aneurysms found in elderly trauma patients should be expected to rise. The purpose of this paper is to report two cases of ruptured abdominal aortic aneurysms found after nonassociated blunt trauma. We review the literature and discuss possible etiologic factors and management associated with this problem.

Imaging and clinical features of intra-abdominal injuries in children with suspected physical abuse.

The effective diagnosis and management of inflicted intra-abdominal injuries require a comprehensive, collaborative evaluation that includes a thorough understanding of the clinical presentation, laboratory findings, injury mechanisms, and potential clinical outcomes. This review presents the various aspects of inflicted intra-abdominal injuries, including the clinical presentation, mechanisms of injury, and the utility of available screening and imaging tools. The specific types of solid organ and hollow viscus injuries relevant to child physical abuse and their imaging appearance are also di

Abdominal Injuries from Civilian Conflicts: An Emerging Global Health Challenge in Rural Southeast Nigeria.

BACKGROUND: In recent time, incidence of abdominal injuries continues to increase steadily in most major regions of West Africa due to emergence of various religious, social and political conflicts. Indeed, violence and social conflicts constitute major global public health challenges that commonly lead to injuries and long-term physical and mental health problems. In our setting, increasing cases of abdominal trauma resulting from civilian violence led to additional workload in the general surgery unit and the audit of our experiences is presented in this paper. OBJECTIVE: To analyze the eti

Gunshot Abdominal Injuries: A Report of Two Cases and a Review of the Literature.

Abdominal injuries in children caused by guns are a rare clinical entity globally. But, in countries with undefined legal regulations and in war zones, urban violence is a tremendous social problem among older children and adolescents. This manuscript provides details regarding two cases of severe gunshot injuries in young children. The injuries were very complicated and included damage to the parenchymatous and hollow organs and major blood vessels. The clinical presentation on admission was severe and dramatic, but the patients survived. However, one patient developed numerous complications

Athletic injuries of the lateral abdominal wall: review of anatomy and MR imaging appearance.

The lateral abdominal wall is comprised of three muscles, each with a different function and orientation. The transversus abdominus, internal oblique, and external oblique muscles span the abdominal cavity between the iliocostalis lumborum and quadratus lumborum posteriorly and the rectus abdominis anteriorly. The lateral abdominal wall is bound superiorly by the lower ribs and costal cartilages and inferiorly by the iliac crest and inguinal ligament. The lateral abdominal wall may be acutely or chronically injured in a variety of athletic endeavors, with occasional acute injuries in the setti
