The most constant radiological features of acromegaly include soft tissue hyperplasia, and increased joint spacing, especially in the limbs by articular cartilage overgrowth. BACKGROUND: Gamma knife radiosurgery (GKR) is an adjuvant treatment for acromegaly if surgery fails to normalize GH hypersecretion.
OBJECTIVE: To examine the effect of GKR on tumor growth and hypersecretion, and to characterize the adverse effect of this treatment.

External radiotherapy has been used as primary treatment for acromegaly in 29 patients and in combination with surgery in 41 patients in whom growth hormone levels remained elevated postoperatively. Spinal stenosis is a well defined clinical entity that is occasionally encountered in patients with acromegaly. Persistent elevation of growth hormone promotes hypertrophy of the bones and ligaments, resulting in widening of the thoracolumbar vertebral bodies, and developmental narrowing of the spinal canal. This report describes the case of a 54-year-old acromegalic man with a 30-year history of low-back pain. Who developed symptoms of spinal stenosis. This case and two similar cases from the literature are discussed, and the appropriate therapeutic approaches reviewed.

Excess growth hormone in acromegaly induces characteristic acral and soft tissue overgrowth (particularly in the face and hands), arthropathies, as well as cardiovascular and metabolic complications. Similar proliferative changes can occur in the larynx, where hypertrophy of the arytenoid and cricoid cartilages may impair vocal fold mobility. We describe a rare case of acromegaly affecting the cricoarytenoid joints (CAJ), leading to proximal airway obstruction and demonstrating that these diarthrodial joints are susceptible to systemic arthropathies.

INTRODUCTION: Acromegaly is due in almost all cases to a GH-secreting pituitary tumor. GH and IGF-1 excesses lead to its multi-system clinical manifestations and comorbidities. Acromegaly is under-diagnosed and typically presents with advanced disease. When early or mild, clinical recognition and biochemical confirmation are especially challenging. Individualized treatment may optimize patient outcome.
