Adenosine is a reliable agent to convert supraventricular tachycardia to regular sinus rhythm with few complications. Also, adenosine may be useful in the diagnosis of narrow and wide QRS complex tachycardias. Original research articles published predominantly in the last 10 years were reviewed. These articles examined the therapeutic and electrophysiologic effects of adenosine compounds and compared their actions to other antiarrhythmics.

The efficacy of adenosine triphosphate (ATP, Spofa) in controlling supraventricular tachycardia (SVT) was assessed in a group of 20 consecutive patients with this condition. Sinus rhythm was restored in a total of 19 patients (95% efficacy). SVT termination was followed, in a full 40% of patients, by asystole with a mean duration of 8.6 seconds; there were, however, no sequelae and no measures had to be taken. ATP can be regarded as a welcome addition to the current range of pharmacological options available for the treatment of SVT.

The inhalation of nebulized adenosine causes bronchoconstriction in asthmatics. In order to explore whether endogenously produced adenosine may contribute to the pathophysiologic aspects of asthma, we measured adenosine concentrations in bronchoalveolar lavage (BAL) fluid in seven subjects with asthma, eight asymptomatic cigarette smokers, and eight normal subjects. The concentration of gamma interferon (IFN gamma) and adenosine deaminase (ADA) activity were measured in the pleural fluid of 162 patients to compare their diagnostic significance and to establish a possible correlation between both tests. Wasted (wst) is a spontaneous mutation with autosomal recessive inheritance. Abnormally low levels of adenosine deaminase have been found in erythrocytes from the wasted mouse. Enzyme activity in wst/wst mice is reduced to 38% of that found in the erythrocytes from control mice, and the apparent Km for adenosine is reduced to 51% of control. Diazepam (3 microM) potentiates the adenosine-induced relaxation of caecum. Other benzodiazepine receptor ligands, such as inosine and hypoxanthine, failed to modify adenosine responses. The stimulatory and inhibitory effects of adenosine on the adenylate cyclases of human and pig platelets were studied. Stimulation occurred at lower concentrations than did inhibition, and the stimulatory effect was prevented by methylxanthines. We describe a patient with severe combined immunodeficiency because of aberrations in adenosine deaminase (ADA) who despite adequate replacement with polyethylene glycol-linked ADA (PEG-ADA) for 13 years developed Burkitt's lymphoma. Although treatment corrected the metabolic abnormalities caused by ADA deficiency, it failed to fully restore cellular immunity.
