Historical and clinical aspects of pellagra and its relationship to alcoholism are reviewed from a biochemical perspective. Pellagra is caused by deficiency of niacin (nicotinic acid) and/or its tryptophan (Trp) precursor and is compounded by B vitamin deficiencies. Existence on maize or sorghum diets and loss of or failure to isolate niacin from them led to pellagra incidence in India, South Africa, Southern Europe in the 18th century and the USA following the civil war. Acrodermatitis enteropathica is a zinc deficiency disorder characterized by well-demarcated, erythematous, eczematous plaques in a periorificial and acral distribution. Hereditary and acquired forms have been described. We report a case of acquired acrodermatitis enteropathica secondary to alcoholism. Treatment of the underlying disorder and zinc replacement therapy resulted in rapid resolution of the condition.

The properties of acamprosate (calcium acetylhomotaurate), a new remedy for the treatment of alcoholism, are reviewed. The efficacy of this drug has been verified by numerous clinical tests in West Europe. The pharmacological activity of acamprosate is related to the drug action upon central neuromediator systems. No side or addiction effects of the drug were reported. Data on the clinical and pharmacological characteristics of acamprosate are summarized.

Some remarks on the individual medical and social problem of alcoholism are presented. The most significant epidemiological data are reported and the most frequently observed medical complications, namely liver, pancreatic, gastroenteric impairment, alcohol and cardiovascular diseases, alcohol and pregnancy, are analysed. Fifty patients with the diagnosis of alcohol dependence or abuse who were admitted to a university-based alcohol rehabilitation program were screened for obsessions and compulsions. The relationship existing between personality disorders (classified according to DSM III) and alcohol treatment outcome was evaluated in 404 alcoholics admitted to the Psychiatric Clinic of the University of Trieste, Italy. Chronic alcoholic patients are frequently deficient in one or more vitamins. The deficiencies commonly involve folate, vitamin B6, thiamine, and vitamin A. Although inadequate dietary intake is a major cause of the vitamin deficiency, other possible mechanisms may also be involved. Alcoholism can affect the absorption, storage, metabolism, and activation of many of these vitamins. Possible factors which cause alterations in the absorption, storage, and metabolism of these vitamins are discussed. Suggestions for management of vitamin deficiencies in chronic alcoholics are also discussed.

The present study reported a group of 168 chronically alcoholic patients with a daily ingestion superior to 80 grams. Twenty of these patients (10 percent) did not have liver disease, and 148 (68.5 percent) had different forms of liver disease classified by histopathologic examination.
