Orthognathic surgery with two-segment le fort i and sagittal split ramus osteotomies of open bite deformity in an amelogenesis imperfecta patient via virtual planning: A case report.

Amelogenesis imperfecta (AI) is an enamel defect and is often associated with the anterior open bite (AOB) and transverse maxillary deficiency. It is known that in such cases when AI and AOB appeared together, posterior maxillary impaction with or without bilateral mandibular ramus osteotomies is a frequently preferred treatment option. Virtual planning is more reliable rather than the conventional model surgery planning, especially for complicated cases. Usage area of virtual 3D anatomical models reconstructed from Cone Beam Computed Tomography (CBCT) data is expanding day by day for both dia…

Fixed prosthodontic rehabilitation using a facially driven fully digital workflow of a patient with syndromic amelogenesis imperfecta associated with a rare form of ectodermal dysplasia, tricho-dento-osseous (TDO) syndrome.

Tricho-dento-ossseous (TDO) syndrome is a rare form of ectodermal dysplasia (ED) characterized by defects in the hair, bones, nails, and teeth. Dental findings consistently seen in patients with TDO syndrome include amelogenesis imperfecta and taurodontism, but additional findings such as periapical abscesses, mandibular prognathism, and impacted teeth have been reported. Because of the rarity of this disorder, the prevalence is unknown and limited research is available regarding its dental management. This clinical report describes the fixed prosthodontic implant rehabilitation of an adult pa…

The immunohistochemical localization of phospholipase Cgamma and the epidermal growth-factor, platelet-derived growth-factor and fibroblast growth-factor receptors in the cells of the rat molar enamel organ during early amelogenesis.

Findings on the localization and possible roles of the major growth factors, epidermal (EGF), platelet-derived (PDGF) and fibroblast (FGF) in early amelogenesis are contradictory and inconclusive. This study sought to localize immunohistochemically phospholipase (PLCgamma) and the EGF, PDGF and FGF receptors in the cells of the enamel organ during the events leading directly to early enamel formation in rat molars. PLCgamma is an immediate, downstream, signal-transduction pathway effector unique to the three receptors. A whole-head, freeze-dried sectioning method was used to reduce the possibi…

KMT2D Regulates Tooth Enamel Development.

Amelogenesis, the process of enamel formation, is tightly regulated and essential for producing the tooth enamel that protects teeth from decay and wear. Disruptions in amelogenesis can result in amelogenesis imperfecta, a group of genetic conditions characterized by defective enamel, including enamel hypoplasia, marked by thin or underdeveloped enamel. Mutations in the KMT2D (MLL4) gene, which encodes histone H3 lysine 4 methyltransferase, are associated with Kabuki syndrome, a developmental disorder that can involve dental anomalies such as enamel hypoplasia. However, the specific role of KM…

Two new families with enamel renal syndrome: A novel FAM20A gene mutation and review of literature.

Enamel renal syndrome (ERS) or so-called amelogenesis imperfecta type IG is a very rare disorder characterized by the triad of amelogenesis imperfecta, gingival enlargement and nephrocalcinosis. It is caused by biallelic mutations in the FAM20A gene. Herein, we report two unrelated patients with ERS. Our patients presented with the characteristic features of the syndrome, and amelogenesis imperfecta and gingival hyperplasia were the main complaint. Strikingly, they both had long face, thick lips, notched upper central incisors, and thick alveolar ridge which have never been reported before in …

An amyloid-based hydrogel with remineralizing and antibacterial properties for interrupting dental caries.

Dental caries is a multifactorial and dynamic disease primarily mediated by biofilm formation, resulting in a disruption of plaque microecological homeostasis and an imbalance in demineralization/remineralization of dental hard tissues. The development of antibacterial/remineralizing composite materials may help restore this balance. However, anticaries products that can mimic the amelogenesis process to achieve enamel remineralization and possess antimicrobial property are lacking. In this study, bovine serum albumin (BSA)-loaded ethylpyridinium chloride (CPC) was successfully used to form a …

The circadian clock in enamel development.

Circadian rhythms are self-sustaining oscillations within biological systems that play key roles in a diverse multitude of physiological processes. The circadian clock mechanisms in brain and peripheral tissues can oscillate independently or be synchronized/disrupted by external stimuli. Dental enamel is a type of mineralized tissue that forms the exterior surface of the tooth crown. Incremental Retzius lines are readily observable microstructures of mature tooth enamel that indicate the regulation of amelogenesis by circadian rhythms. Teeth enamel is formed by enamel-forming cells known as am…

Maturation stage enamel defects in Odontogenesis-associated phosphoprotein (Odaph) deficient mice.

BACKGROUND: Mutation in Odontogenesis-associated phosphoprotein (ODAPH) has been reported to cause recessive hypomineralized amelogenesis imperfecta (AI) in human. However, the exact role of ODAPH in amelogenesis is still unknown. RESULTS: ODAPH was identified as a novel constituent of the atypical basal lamina located at the interface between maturation ameloblasts and the enamel by dual immunofluorescence staining of ODAPH and LAMC2. Odaph knockout mice were generated to explore the function of ODAPH in amelogenesis. Odaph-/- mice teeth showed severely attrition and reduced enamel mineraliz…

SLC4A9 deficiency is involved in Sjögren's syndrome and enamel defects through RPS27-triggered ribosomal stress.

Xerostomia, a salivary gland disorder affecting up to 30% of adults, compromises oral homeostasis by reducing salivary flow and buffering capacity. Amelogenesis imperfecta is a hereditary condition characterized by defective enamel formation and mineralization, with a prevalence ranging from 1 in 700 to 1 in 14,000 children worldwide. Although xerostomia has been associated with increased susceptibility to dental caries and subsequent enamel damage, its relationship with amelogenesis imperfecta remains unclear, and no shared molecular pathway has been causally linked to both hyposalivation and…

ODAPH (p.Arg77*) Phenotype and Onset of Ameloblast Pathology During Postsecretory Transition Demonstrated by FIB-SEM Analyses of OdaphC41*/C41* Mice.

Defects in Odontogenesis-Associated Phosphoprotein (ODAPH) cause autosomal recessive non-syndromic amelogenesis imperfecta (AI; OMIM #614832). We present a 14-year-old boy with delayed tooth eruption and generalized soft and yellow enamel. The proband's parents and two siblings were unaffected. Whole exome analyses identified the AI-causing homozygous ODAPH truncation mutation (p.Arg77*) in the proband. Both unaffected parents and one sibling were heterozygous for this change. The other sibling had the reference sequence. To better understand the role of ODAPH in normal amelogenesis and resolv…

The Absence of Claudin-10 in the Enamel Organ Alters Its Integrity.

Rare disorders related to tight junction (TJ) proteins have been associated with amelogenesis imperfecta. Pathogenic variants of CLDN10, encoding claudin-10b, a cation transport pore, cause the autosomal recessive HELIX syndrome (Hypohidrosis, Electrolyte imbalance, hypoLacrymia, Ichthyosis, Xerostomia). Patients exhibit salivary dysfunction and rapid enamel wear after tooth eruption. Since Cldn10 is expressed in the dental epithelium, this study explores the role of claudin-10b in amelogenesis. We analyzed amelogenesis in constitutive and conditional Cldn10 knockout (KO) murine models, compar…

Non lethal Raine syndrome and differential diagnosis.

Raine syndrome is a rare autosomal recessive bone dysplasia characterized by characteristic facial features with exophthalmos and generalized osteosclerosis. Amelogenesis imperfecta, hearing loss, seizures, and intracerebral calcification are apparent in some affected individuals. Originally, Raine syndrome was originally reported as a lethal syndrome. However, recently a milder phenotype, compatible with life, has been described. Biallelic variants inFAM20C, encoding aGolgi casein kinase involved in biomineralisation, have been identified in affected individuals. We report here a consanguineo…

[Natal and postnatal teeth: their enamel].

The enamel of a natal tooth in a healthy full-term child and the enamel of a postnatally erupted tooth in a premature child have been studied with scanning electron microscopy on longitudinal and cross sections. The first case shows that following a normal development, amelogenesis was arrested. This is evident by the presence of the external aprismatic layer in spite of the narrowness of the enamel which corresponds to about a third of the normal thickness. This premature arrest of amelogenesis can be dated to about the 6th month "in utero". In the second case we observed a reversible ameloge…

Bicarbonate Transport During Enamel Maturation.

Amelogenesis (tooth enamel formation) is a biomineralization process consisting primarily of two stages (secretory stage and maturation stage) with unique features. During the secretory stage, the inner epithelium of the enamel organ (i.e., the ameloblast cells) synthesizes and secretes enamel matrix proteins (EMPs) into the enamel space. The protein-rich enamel matrix forms a highly organized architecture in a pH-neutral microenvironment. As amelogenesis transitions to maturation stage, EMPs are degraded and internalized by ameloblasts through endosomal-lysosomal pathways. Enamel crystallite …

Orthodontic bonding in special circumstances.

This clinical paper provides an in-depth exploration of advanced techniques for bonding orthodontic attachments under special circumstances. Challenges arise when bonding brackets to non-enamel surfaces, such as dental restorations, and in conditions such as amelogenesis imperfecta, which affect enamel integrity. Distinct approaches required for bonding to different restorative materials, including glassy ceramics, zirconia, resin composites and metals, are outlined. Moreover, we describe strategies to manage bonding in conditions including amelogenesis imperfecta, hypodontia and microdontia i…
