Factors associated with improvement in ankle dorsiflexion after transfibular total ankle arthroplasty.

BACKGROUND: While transfibular total ankle arthroplasty (TAA) demonstrates favorable short-term outcomes, factors influencing postoperative ankle dorsiflexion remain underexplored. This is the first study aimed to identify factors influencing dorsiflexion range of motion after transfibular TAA. METHODS: Forty-nine patients undergoing transfibular TAA were classified into dorsiflexion-improved (DF-improved) group (≥5°,n = 26) and dorsiflexion-non-improved (DF-non-improved) group (<5°, n = 23). Radiographic parameters, residual medial tibial osteophytes, and Achilles tendon lengthening were eva

Chronic Ankle Joint Instability Induces Ankle Sensorimotor Dysfunction: A Controlled Laboratory Study.

BACKGROUND: Chronic ankle instability (CAI) is a clinical sequela that causes the recurrence of ankle sprain by inducing ankle sensorimotor dysfunction. Animal models of CAI have recently shown that ankle ligament injuries mimicking an ankle sprain result in chronic loss of ankle sensorimotor function. However, the underlying mechanisms determining the pathogenesis of CAI remain unclear. HYPOTHESIS: Ankle instability after an ankle sprain leads to the degeneration of the mechanoreceptors, resulting in ankle sensorimotor dysfunction and the development of CAI. STUDY DESIGN: Controlled laborat

Surgical Management of Complex Ankle Bony Impingement Combined With Chronic Ankle Instability.

BACKGROUND: Anterior ankle bony impingement, which can cause pain and dysfunction of the ankle, is commonly seen in sports injuries. Its primary cause is repeated injury due to chronic ankle instability. An excellent clinical result has been reported by surgically removing osteophytes and ankle stabilization. However, reports of complex anterior impingement combined with lateral ankle instability are rare, which remains challenging in clinical practice. PURPOSE/HYPOTHESIS: The purpose of this study was to evaluate the clinical outcomes of anterior ankle bony impingement combined with lateral

Outcomes of Ankle Arthrodesis Conversion to Total Ankle Arthroplasty: A Systematic Review.

Ankle arthrodesis (AA) provides reliable pain relief, good patient satisfaction scores, and improved overall function. However, this procedure has been associated with numerous complications and sequelae, such as pseudoarthrosis, malunion, gait abnormalities, increased demand on surrounding joints, and a long period of convalescence. Conversion to total ankle arthroplasty (TAA) is a potential option in the management of these complex and challenging situations. The purpose of this study is to investigate the outcomes of AA conversion to TAA. A systematic review of electronic databases was perf

Ankle arthrodesis for end-stage haemophilic ankle arthropathy using a Ilizarov method.

PURPOSE: Only a few studies in the literature have reported on the outcome of ankle arthrodesis in haemophilia patients, and the optimal surgery technique for end-stage haemophilic ankle arthropathy remains controversial. The purpose of this study was to evaluate the results of ankle arthrodesis using the Ilizarov method in treating end-stage haemophilic ankle arthropathy. METHODS: Fourteen haemophilia patients with end-stage ankle arthropathy who underwent ankle arthrodesis using the Ilizarov method from December 2013 to May 2018 were enrolled in this study. The hospital day, fusion rate, co

Pitfalls in the ankle-brachial index and brachial-ankle pulse wave velocity.

BACKGROUND: The ankle-brachial index (ABI) and pulse wave velocity (PWV) are indices of atherosclerosis and arterial stiffness. The Japan-made measuring devices of those indices have spread widely because of their convenience and the significance of the parameters. However, studies that comprehensively discuss the various pitfalls in using these indices are not available. METHODS: This study presents several representative pitfalls in using the ABI and brachial-ankle PWV (baPWV) by showing the result sheets of the device, "the Vascular Profiler". Furthermore, some considerations when utilizin

Acute lateral ankle sprain to chronic ankle instability: a pathway of dysfunction.

Lateral ankle sprains (LAS) have been reported as one of the most common musculoskeletal injuries observed in sports and in individuals who are recreationally active. Approximately 40% of individuals who sustain a LAS develop a condition known as chronic ankle instability (CAI). Years of research has identified numerous impairments associated with CAI such as decreases in range of motion (ROM), strength, postural control, and altered movement patterns during functional activities when compared to individuals with no LAS history. As a result, an impairment-based rehabilitation model was develop

Recalcitrant Lateral Premalleolar Bursitis of the Ankle Associated with Lateral Ankle Instability.

Lateral premalleolar bursitis of the ankle is a rarely reported disorder in the English literature although it is not uncommon in Asian countries where people commonly sit on their feet. Here, we present the case of a 66-year-old woman with recalcitrant lateral premalleolar bursitis associated with lateral ankle instability which was successfully treated with surgical resection of the bursa and repair of the anterior talofibular ligament. Operative findings revealed a communication between the bursa and articular cavity of the ankle joint via the sheath of the extensor digitorum longus tendon,

Biomechanics of ankle instability. Part 1: Reaction time to simulated ankle sprain.

PURPOSE: To test the hypothesis that ankles with functional instability will demonstrate slower muscular reaction times than their contralateral stable ankle (SA) and stable healthy controls to a simulated nonpathological ankle sprain mechanism. METHODS: Nineteen male volunteers with a history of unilateral ankle sprain and functional ankle instability (FAI) and 19 healthy male controls performed reaction time tests on a purpose-built platform that simulated a nonpathological combined inversion/plantarflexion ankle sprain mechanism. Participants provided informed written consent. Reaction tim

Use of bivalved ankle-foot orthosis in neuropathic foot and ankle lesions.

The neuropathic foot is a common complication of diabetes mellitus and is associated with development of chronic ulcers and Charcot joints. Treatment of these complications presents a complex management task. This report presents long-term follow-up data on 16 patients with neuropathic changes who were treated with a total contact, laminated, bivalved, rocker-bottom-soled ankle-foot orthosis (TCAFO). Of the 16 patients, 6 were treated for Charcot changes only, 10 were treated for ulcers, and 2 of the 10 had ulcers bilaterally. Eight of the 12 ulcers (67%) healed in an average of 10 months (ran

Consideration for Total Ankle Replacement in the Varus Ankle and Cavovarus Foot Type.

The varus ankle and cavus foot pose challenges in surgical correction with regard to total ankle replacement surgery. Etiology of cavus foot type and varus ankle must be evaluated and confirmed. Pes cavus is increased height of the arch with metatarsus adductus and increased calcaneal inclination angle. There often is intrinsic musculature irregularity leading to imbalance of the foot. Although not all cavus foot types and varus ankle deformities are sequelae of neuromuscular disorder, neurologic etiology must be considered. Attaining neutral alignment of ankle joint articular surface is param

Outcomes of Total Ankle Arthroplasty in Ankles with >20° of Coronal Plane Deformity.

BACKGROUND: A preoperative severe coronal plane deformity of >20° has been considered a contraindication for total ankle arthroplasty. We aimed to evaluate whether outcomes of total ankle arthroplasty in ankles with severe coronal plane deformity (20° to 35° of varus or valgus) are comparable with those with moderate deformity (5° to 15° of varus or valgus). METHODS: A total of 148 consecutive ankles (142 patients) that underwent primary total ankle arthroplasty using the HINTEGRA prosthesis were included. The overall mean follow-up duration was 74 months (range, 24 to 160 months). We divided

Validity of the Foot and Ankle Ability Measure in athletes with chronic ankle instability.

CONTEXT: The Foot and Ankle Ability Measure (FAAM) is a region-specific, non-disease-specific outcome instrument that possesses many of the clinimetric qualities recommended for an outcome instrument. Evidence of validity to support the use of the FAAM is available in individuals with a wide array of ankle and foot disorders. However, additional evidence to support the use of the FAAM for those with chronic ankle instability (CAI) is needed. OBJECTIVE: To provide evidence of construct validity for the FAAM based on hypothesis testing in athletes with CAI. DESIGN: Between-groups comparison.
