MRI of the sinus tarsi in acute ankle sprain injuries.

PURPOSE: Our goal was to evaluate the visibility and incidence of traumatic abnormalities of the sinus tarsi in patients with acute ankle sprain injuries and compare these findings with the extent of lateral ankle ligament injuries on MRI. METHOD: Sixty athletically active patients (aged 18-45 years) with recent inversion trauma (< or = 7 days) underwent MRI. Replacement of fat tissue in the sinus tarsi was recorded. Inter/intraobserver agreement was calculated (kappa-statistics). Injuries of the three lateral ligaments, evaluated by MRI, were graded according to partial (1 point) or complete

Primary care of foot and ankle injuries in the athlete.

A thorough knowledge of foot and ankle anatomy is required to allow an accurate and focused examination of the injured athlete. This short review has attempted to educate the treating physician on our approach to foot and ankle injuries commonly seen in athlete. We have tried to elucidate less common injuries that present in similar manner to the more common foot and ankle sprains and strains.

Decision rules for roentgenography of children with acute ankle injuries.

OBJECTIVE: The Ottawa Ankle Rules (OAR) assist emergency physicians in the appropriate use of roentgenography in adults with acute ankle injuries. The OAR state that ankle roentgenograms are needed only if there is pain near the malleoli and one or more of the following exists: (1) age 55 years or older; (2) inability to bear weight; or (3) bone tenderness at the posterior edge or tip of either malleolus. This study assessed the utility of the OAR on pediatric patients with acute ankle injuries. DESIGN: Prospective, consecutive survey of pediatric patients with acute ankle injuries. SETTING:…

The sprained ankle. Diagnosis and management of lateral ligament injuries.

Ankle sprains are common injuries. They are often associated with residual disability with the ankle tending to give way on uneven ground (functional instability). Good management and rehabilitation require accurate diagnosis of the nature and extent of the injury, attention to the physical treatment and follow up with ankle strengthening and proprioceptive exercises to minimise the risk of functional instability.

Historical perspectives on injuries of the ligaments of the ankle.

As is readily apparent, there is not a consensus regarding the treatment of lateral ligamentous ruptures of the ankle. Certainly, the elimination of functional instability is the goal of treatment. What actually causes this is still open to debate, but from Harrington's work, it appears that degenerative changes can result from mechanical instability. We are all aware that many athletes compete actively with "two-plane" instability of the ankle, but it remains to be seen if they will also suffer the late degenerative changes that Harrington describes. In the meantime, each physician must decid

Inversion injuries of the ankle: clinical assessment and radiographic review.

Inversion injuries of the ankle are a common cause of referral and presentation to accident units. They impose a load on radiographic services. A prospective trial was carried out to determine the accuracy of clinical examination. All patients were assessed clinically then examined radiographically, the clinical assessment missed 5% of the fractures. These, however, were all minor avulsion fractures or crush fractures and Tubigrip support was sufficient. Clinical examinations is, therefore, accurate and the need for most x-ray examination is questionable. X-ray examination should be reserved f

Rationalising requests for x-ray examination of acute ankle injuries.

Radiographs of injured ankles represent about 1 in 50 of all radiological examinations. The notes and radiographs of 100 patients with ankle injuries were reviewed, and the films of a further 93 patients who had ankle fractures treated by immobilisation were also scrutinised to assess the presence of absence of soft tissue swelling over the malleoli. Any accompanying radiographs of the foot requested at the same time were also studied. In 65 of the 100 cases of ankle injury there was no soft tissue swelling, and none of the patients had a major fracture, while 92 of the 93 patients with a majo…

Can American Orthopaedic Foot and Ankle Society (AOFAS) score prevent unnecessary MRI in isolated ankle ligament injuries?

INTRODUCTION AND OBJECTIVE: Ankle injuries are the most common musculoskeletal injuries. Its incidence is also high among sports injuries. Direct X-ray, ultrasound and MRI can be requested after the history and physical examination in the patient who presents with ankle ligament injury. Some classifications are used for requesting direct X-ray after ankle injury. Since clear limits are not specified in the literature for MRI, the rate of unnecessary MRI examinations is high. We argue that the decision can be made according to the AOFAS score to be checked before MR is requested, and thus unnec…

Achilles Allograft Reconstruction for Ankle Multiligament Injuries in a Patient Presenting With Rotational Ankle Instability: A Surgical Technique.

The ankle comprises two main ligamentous complexes: the medial and lateral complex. In cases of chronic ankle instability following a sprain involving both complexes, rotational ankle instability can develop. We introduce a 25-year-old male with rotational ankle instability who reported having 20 episodes of "giving away" over the 6 months following an ankle sprain. Due to persistent instability, the patient was deemed a candidate for surgical reconstruction. An Achilles tendon allograft was utilized to reconstruct four ligaments-two from the lateral complex (the anterior talofibular ligament

"Effects of ankle orthoses on postural balance and gait parameters in athletes with ankle injuries: A systematic review".

OBJECTIVES: Ankle injuries in athletes often impair balance and gait, affecting performance. Although ankle orthoses are used to improve stability, their effects on postural control and gait remain unclear. This review investigates the impact of ankle orthoses on balance and gait in injured athletes. METHODS: This systematic review, registered in PROSPERO (XXXXXX) and conducted following PRISMA 2020 guidelines, included interventional and observational studies up to April 30, 2025. Databases searched were PubMed, Scopus, Web of Science, and CINAHL. Two independent reviewers conducted screenin…

Incidence and Association of CT Findings of Ankle Tendon Injuries in Patients Presenting With Ankle and Hindfoot Fractures.

OBJECTIVE: Tendon injuries are a commonly encountered finding in ankle CT examinations performed for fractures. This study was designed to identify the incidence and associations of tendon injuries in ankle CT examinations performed for fractures. MATERIALS AND METHODS: A retrospective review was performed of 410 patients who underwent ankle CT during a 6-year period. RESULTS: Tendon injuries were common, seen in 25% of all ankle CT examinations. Tendon subluxation-dislocation accounted for most of the tendon injuries (77 of 196 total injuries). Pilon fractures carried 2.2 times increased ri

Development and validation of the Sports Athlete Foot and Ankle Score: an instrument for sports-related ankle injuries.

BACKGROUND: Many existing scoring systems assess ankle function, but there is no evidence that any of them has been validated in a group of patients with a higher demand on their ankle function. Problems include ceiling effects, not being able to detect change or they do not contain a sports-subscale. The aim of this study was to create a validated self-administered scoring system for ankle injuries in the higher performing athlete. METHODS: First, 26 patients were interviewed to solicit opinions needed to create the final score, which is modified from the Foot and Ankle Outcome Score (FAOS).…

Pathoanatomy and Associated Injuries of Posterior Malleolus Fracture of the Ankle.

BACKGROUND: We present a classification system that progresses in severity, indicates the pathomechanics that cause the fracture and therefore guides the surgeon to what fixation will be necessary by which approach. METHODS: The primary posterior malleolar fracture fragments were characterized into 3 groups. A type 1 fracture was described as a small extra-articular posterior malleolar primary fragment. Type 2 fractures consisted of a primary fragment of the posterolateral triangle of the tibia (Volkmann area). A type 3 primary fragment was characterized by a coronal plane fracture line invol
