'The quicksand of forgetfulness': semantic dementia in One hundred years of solitude.

This multidisciplinary article compares the pattern of memory loss described in Gabriel García Márquez's One Hundred Years of Solitude to that exhibited by patients with semantic dementia (SD). In his renowned novel, García Márquez depicts the plight of Macondo, a town struck by the dreaded insomnia plague. The most devastating symptom of the plague is not the impossibility of sleep, but rather the loss of 'the name and notion of things'. In an effort to combat this insidious loss of knowledge, the protagonist, José Arcadio Buendía, 'marked everything with its name: table, chair, clock, door,

Anomic alexia of kanji in a patient with anomic aphasia.

The ability to read aloud kanji (logogram) words and to comprehend their meaning was systematically examined to clarify the underlying mechanism of kanji alexia in a patient with anomic aphasia. Confrontation naming, reading aloud and reading comprehension tasks were performed using 110 words from 11 semantic categories written in kanji or kana. Performance in oral reading of kanji words was significantly worse than oral reading of the same words transcribed into kana words. In addition, for kanji words reading aloud was much worse than reading comprehension. Oral reading of kanji words had a

Naming speed deficits in adults with reading disabilities: a test of the double-deficit hypothesis.

The present study investigated the persistent nature of naming speed deficits within the context of the double-deficit hypothesis in a university sample of adults with reading disabilities (RD). Twenty-five university students with RD were compared to 28 typically achieving readers on measures of reading skill, phonological processing, and naming speed. The results indicated that both naming speed and phonological processing deficits characterized the RD group. In a regression analysis, neither naming speed nor phonological processing were important variables in explaining comprehension when r

Alexia without agraphia and associated disorders: importance of recognition in the rehabilitation setting.

Alexia without agraphia is readily recognized in its pure (without other neurologic findings) but rare form. However, this deficit is more common when associated with other behavioral disturbances that result from trauma or cerebral infarction to the posterior cerebral hemispheres. Two cases presented with alexia without agraphia following infarction within the posterior cerebral artery territory. Because of initial confusion and disorientation the patients were diagnosed as demented and unsuitable for rehabilitation. Both subjects had a visual field deficit and color agnosia. Putative problems and pure progress in neuropsychological single-case studies.

We consider the logic of basing theories of normal and pathological cognitive functioning upon the selective deficits revealed by single-case studies. Objections to this strategy, concerned with the rarity of pertinent cases, the role of methodological artifacts, and the existence of normal variation in brain functions are refuted. We conclude that, in neuropsychology, a group has no significance over and above the individual members it contains.

Cortico-subcortical organization of language networks in the right hemisphere: an electrostimulation study in left-handers.

We have studied the configuration of the cortico-subcortical language networks within the right hemisphere (RH) in nine left-handers, being operated on while awake for a cerebral glioma. Intraoperatively, language was mapped using cortico-subcortical electrostimulation, to avoid permanent deficit. In frontal regions, cortical stimulation elicited articulatory disorders (ventral premotor cortex), anomia (dorsal premotor cortex), speech arrest (pars opercularis), and semantic paraphasia (dorsolateral prefrontal cortex). Insular stimulation generated dysarthria, parietal stimulation phonemic para…

[Creutzfeldt-Jakob disease with unilateral onset: clinical profile and neuroimaging].

The usual clinical profile of Creutzfeldt-Jakob disease (CJD) is that of subacute dementia and intractable myoclonus. Occasionally, some cases present peculiar clinical features. We report on a case of CJD with an unilateral onset showing remarkable neuroimaging features. The patient, aged 72 years, began to suffer from sudden anomia, initially restricted to persons; but in a few weeks it evolved into a global aphasia, right hemiparesis, severe gait disorder, and finally akinetic mutism and intractable myoclonus. He died 11 weeks after onset. Early in the course, an analysis of 14-3-3 protein …

Optic aphasia: a process of interaction between vision and language.

A neurological syndrome, called in the literature ether optic aphasia or visual anomia, is defined in principle as the inability to name visually presented objects, together with the preservation of both the ability to identify them by sight correctly and to name them when they are presented in another sensory modality. This syndrome was first described by Freund in 1889, but since then its existence has been continually questioned. When it is accepted, the most common interpretation of it is in terms of an anatomical visuo-verbal disconnection. However, the precise level of the psychological …

Acquired dysgraphia with selective damage to the graphemic buffer: a single case report.

We describe one patient with acquired dysgraphia who showed spelling errors (mainly deletions and substitutions), both for words and non-words, across all output modalities (oral and written spelling, and delayed copying). Spelling accuracy was not affected by lexical factors, but was a function of word length. The patient's performance in oral and written tasks suggests the hypothesis of selective damage to the Graphemic Buffer.

Naming speed in children with dyslexia.

A series of tests of naming speed in discrete reaction time format were undertaken by seven groups of children: three groups with dyslexia with mean ages 8, 13, and 17 years; three groups of normally achieving children matched for age and IQ with the dyslexic groups; and a group of 10-year-old children with mild learning difficulties (slow learners) matched for reading age with the youngest dyslexic group. The children with dyslexia were significantly slower than even their chronological age-matched controls, and equivalent to their reading age-matched controls, on naming colors, digits, and l

[Complete human Klüver-Bucy syndrome after encephalitis caused by herpes simplex type 2].

A case of complete Klüver-Bucy syndrome is reported. A 42 year-old man developed Herpes Simplex type II (H.S.V.II) encephalitis. Good sparing of language functions allowed thorough neuropsychological testing. The troubles usually described in Klüver-Bucy syndrome as psychic blindness, colour agnosia, prosopagnosia, auditive and tactile agnosia were present. The authors theorize that these symptoms are mainly correlated with the amnestic syndrome, which is constantly reported in human Klüver-Bucy syndrome. Unfortunately, after 8 weeks, the encephalitis recurred and the patient was left demented

[Group therapy of aphasia patients--a professional report].

Theoretical considerations in, and an exemplary instance of, group speech therapy with aphasic patients are submitted for discussion. Contrary to current practice, individual and group therapy are considered of equal rank, as all aspects of speech and language can be realized in the latter, with both approaches having similar goals. Group work contents is characterized as verbal coping with guided verbal tasks. Group composition is recommended to emphasize speech and language criteria, as opposed to social and psychosocial criteria. Also dealt with is the position of the speech therapist.

Simultaneous Normalization and Compensatory Changes in Right Hemisphere Connectivity during Aphasia Therapy.

Changes in brain connectivity during language therapy were examined among participants with aphasia (PWA), aiming to shed light on neural reorganization in the language network. Four PWA with anomia following left hemisphere stroke and eight healthy controls (HC) participated in the study. Two fMRI scans were administered to all participants with a 3.5-month interval. The fMRI scans included phonological and semantic tasks, each consisting of linguistic and perceptual matching conditions. Between the two fMRI scans, PWA underwent Phonological Components Analysis treatment. Changes in effective…

Update on frontotemporal dementia.

BACKGROUND: Frontotemporal dementia has recently been recognized as a common cause of young-onset dementia. OBJECTIVE: To review the current approach to the clinical evaluation, understanding of pathophysiology, and management of frontotemporal dementia. RESULTS: Two main clinical presentations are: (1) behavioral, with impulsive behaviors and disinhibition, change in personality such as apathy and indifference, and poor judgment, and (2) language, with a nonfluent aphasia with anomia (primary progressive aphasia), or a fluent aphasia with early loss of word meaning (semantic dementia).
