Neoadjuvant chemoradiotherapy in rectal cancer and anorectal sphincter dysfunction: Review of the literature.

PURPOSE: The purpose of this article was to review the current medical literature regarding deterioration of anorectal function in patients receiving neoadjuvant chemoradiotherapy before surgery for locally advanced rectal cancer. METHODS: We reviewed the current literature including research studies, electronic database PUBMED-MEDLINE, published research results and metanalysis papers from high-volume institutes, collecting and comparing the different results. Pathophysiology as well as emerging solutions for treating anorectal sphincter dysfunction were researched in order to provide an ins

Dinitrochlorobenzene treatment of condylomata acuminata.

Fifteen patients with treatment-resistant condylomata acuminata have been treated by application of Dinitrochlorobenzene (DNCB). The agent was applied weekly as a 0.5 or 1% solution in acetone for a period of 6 to 8 weeks. Patients were sensitised 15 days before the onset of treatment by applying a 2% DNCB solution on the right arm in a quantity of 0.15 ml. In one patient treatment had to be stopped because of local irritation and one discontinued treatment. In the other 13 patients all lesions cleared completely. Patients were followed up for a year and no recurrence has appeared as yet. DNCB

The accuracy of anal self- and companion exams among sexual minority men and transgender women: The Prevent Anal Cancer Palpation Study.

BACKGROUND: Squamous cell carcinoma of the anus (SCCA) annual incidence among sexual minority men (SMM) with and without HIV is 85/100,000 and 19/100,000 persons, respectively, which is significantly higher than the overall incidence (2/100,000). Since SCCA tumours average ≥30 mm at diagnosis, we assessed the accuracy of individuals to self-detect anal abnormalities. METHODS: The study enrolled 714 SMM and transgender women (SMM/TW), aged 25 to 81 years, in Chicago, Illinois and Houston, Texas during 2020-2022. Individuals were taught the anal self- and companion examinations (ASE/ACE). Then,

Human papillomavirus-associated malignancies in HIV-positive men and women.

Human papillomavirus (HPV) infections of the anogenital tract and HPV-associated anogenital neoplasia are commonly found in HIV-positive men and women. Although there is little evidence suggesting that the incidence of invasive cervical cancer is increased in HIV-positive women, the incidence of invasive anal cancer may be increased in HIV-positive men. Among HIV-positive women, cervical intraepithelial neoplasia progresses more rapidly and recurs more often after primary therapy than in HIV-negative women. HPV infection and HPV-associated disease are often multifocal in HIV-positive women, an

[Clinical application and standardized implementation of intersphincteric resection].

Intersphincteric resection (ISR) is the ultimate sphincter-preserving surgical technique for low rectal cancer. To promote the standardized implementation of ISR, this review discusses the important issues regarding the clinical application of ISR with reference to the latest Chinese expert consensus on ISR. In terms of ISR-related pelvic anatomy of the rectum/anal canal, hiatal ligament is not identical with the anococcygeal ligament. At the level where the rectourethralis muscle continuously extends to the posteroinferior area of the membranous urethra from the rectum, the neurovascular bund

Emotional Distress Prior to Chemoradiation for Rectal or Anal Cancer.

BACKGROUND/AIM: Anticipation of chemoradiation has been reported to cause psychosocial distress in patients with rectal cancer. This study provides additional data regarding prevalence and risk factors of emotional distress in patients who received chemoradiation for rectal or anal cancer. PATIENTS AND METHODS: Sixty-four patients were analyzed for emotional distress utilizing 12 factors. When applying the Bonferroni correction, p-values <0.0042 were considered significant. RESULTS: Worry, fears, sadness, depression, nervousness, and loss of interest in usual activities were reported by 31%,

Radical abdomino-pelvic surgery in the management of uterine carcinosarcoma with concomitant para-aortic lymphadenopathy metastasising from anal carcinoma.

Uterine carcinosarcomas are aggressive gynaecological cancers comprising less than 5% of uterine malignancies. We present the case of a woman in her 70s with a complicated history of advanced anal carcinoma treated with pelvic radiotherapy and multiple laparotomies, who was referred to gynae-oncology following MRI surveillance imaging showing evidence of endometrial carcinoma and para-aortic lymphadenopathy. Successful surgical excision required multidisciplinary teamwork between gynae-oncology, colorectal and urology surgeons. The patient underwent midline laparotomy, with adhesiolysis, ileum

Optimization of pre-, intra- and postoperative hyperthermic treatment in inoperable lower bowel and liver tumors.

Traditional therapies in malignant tumors are often inadequate, either because the tumors aren not responsive or because the tolerance dosage has little effect. With this in mind, our study examined advanced tumors of the lower bowel and liver. We evaluated the results of 10 cases of anorectal epidermoid cancer. We treated these patients with radiofrequency hyperthermia, since previous radiotherapy was unsuccessful. The patients had significant tumor remission, along with a 'free-of-disease' follow-up of 10 months. In this paper, we also report the positive response in 10 cases of multifocal l

Goblet cell adenocarcinoma of the anal canal with perianal Paget disease: A rare case report with literature review.

INTRODUCTION: AS an uncommon neoplasm, goblet cell adenocarcinoma (GCA) is characterized by mixed endocrine-exocrine features. It is almost exclusively found in the appendix. Primary GCA of the anal canal is extremely rare. CASE PRESENTATION: Herein we describe a novel rare case of 74-year-old Chinese female who is diagnosed with GCA in the anal canal with perianal Paget disease, including a brief review of the literature. In the lesion of anal canal, the tumor was composed of signet-ring-like cells on confluent growth model and copious mucin was produced as well. Simultaneously, the results
