Endoscopic Management of Crayfish (Procambarus clarkii) Aspiration: A Case Report.

INTRODUCTION: Airway foreign bodies are time-sensitive otolaryngologic emergencies that can occur in persons of any age. We describe a unique case of complete aphonia caused by the impaction of a crayfish (Procambarus clarkii) claw at the level of the vocal cords. CASE PRESENTATION: A 65-year-old man developed abrupt throat discomfort and loss of voice while eating crayfish. Indirect laryngoscopy showed a foreign body resting across the vocal cords with absent vibration. Cervical CT demonstrated a tubular hyperdense structure within the glottic region, and flexible transoral bronchoscopy conf…

Using sign language with tracheotomized infants and children.

Children's ability to communicate with others is important for cognitive development. However, the number of infants and young children deprived of vocalization (aphonia) is increasing in the pediatric patient population due to tracheotomy. The Communication Program for Infants and Parents (CPIP) uses sign language as an alternative system to verbal communication.

Retrospective study of 116 patients with non-organic voice disorders: efficacy of mental imagery and laryngeal shaking.

OBJECTIVE: To report the short- and long-term results of two techniques (mental imagery and manual shaking of the larynx) in patients with non-organic dysphonia or aphonia. DESIGN: Retrospective review of patient records, plus follow-up survey (questionnaire). SETTING: Academic teaching hospital. PATIENTS: One hundred and sixteen patients with moderate to severe non-organic dysphonia or aphonia. OUTCOME: Cure (i.e. normal voice) and improved voice quality, judged by clinicians and patients. RESULTS: One hundred (86 per cent) of the 116 patients were cured. Ninety-four (81 per cent) patien…

[The lombard reflex as a test of vocal function (author's transl)].

Any impairment of audio-phonatory control by background noise is followed by an increase in both the intensity and pitch of the speaking voice (Lombard reflex, 1911), thus increasing vocal strain. As a consequence, it might be anticipated that persons reacting to noise with marked changes in voice might be more liable to develop dysphonia. 22 singers, 34 normal controls, and 22 patients with hyperfunctional dysphonia where studied. In all patients, both ears were gradually masked with white noise. The change of the mean intensity level and of the mean pitch level of the speaking voice were the

[Pharmacotherapy of voice and articulation disorders in aphasia].

The phonation and articulation disorders in a group of aphasics with aphonia/dysarthrophonia could be considerably reduced by the use of medication. The group consisted of 10 apoplectic patients whose resulting aphasia could not be classified because of the vocal impairment. Extrapyramidal motion disorders were proved by laryngoscopy and stroboscopy. A definite improvement of phonation and articulation was observed after L-dopa medication.

[Typhoid encefalopathy followed by permanent memory defects (author's transl)].

An 18-years-old girl suddenly became comatose and afterwards presented stlpor, complex hyperkinesias and aphonia lasting 15-20 days. The clinical picture was that of an acute encephalopathy which was found to be due to typhoid fever. Follow-up showed severe permanent memory impairment and some degree of mental deterioration, but no longer evidence of basal ganglia signs.

[Atypical acute dystonia].

Acute dystonia is a common adverse effect following anti-psychotic medication, which mainly appears shortly after beginning treatment or increasing the dosage. Laryngeal dysfunction may carely occur as part of the picture of acute dystonia and, if so, usually with dyspnoea. We describe a case of acute dystonia with atypical onset without relation to changes in dosage and with laryngeal involvement with aphonia, but without dyspnoea.

[The arthritis of the cricoarytenoid joint, a rare postintubation complication and its novel treatment (author's transl)].

A case of aphonia following a long-time endotracheal intubation is presented. The paramedian position of the left and the incomplete adduction of the right vocal cord were caused by a complete, respectively partial ankylosis of the cricoarytenoid joints. The ankylosis was the result of an arthritis during the intubation period. A nearly normal mobility of the vocal cords and a normal voice was achieved by translaryngeal injections of Fludrocortisone and Hyaluronidase into the cricoarytenoid joints and by their passive mobilisation.

Myasthenia gravis focal to the larynx. Report of a case.

Myasthenia gravis, focal to the larynx, should be considered in the differential diagnosis of intermittent breathy dysphonia and aponia. A 20-year-old woman had intermittent breathy dysphonia and aphonia as the sole manifestation of myasthenia gravis. The vocal cords were flaccid, and adducted to the paramedian position for phonation. Improvement of laryngeal function was demonstrated with edrophonium and neostigmine bromide. Therapy with pyridostigmine bromide (Mestinon) maintained normal phonation.

Convergence to shortened and lengthened voice onset time in an imitation task.

Convergence to variation in voice onset time (VOT) of English voiceless stops is reported to be selective, with speakers adjusting their VOTs after exposure to stimuli with lengthened, but not shortened, VOT. The current study re-examined this proposed selectivity with an explicit imitation paradigm designed to maximize convergence, using stimuli with more extreme differences, and explored the perceptual salience of the differences in a discrimination task. Participants showed phonetic convergence to both shortened and lengthened VOT, and better discrimination of shortened than lengthened VOT.

Spastic dysphonia.

130 cases of spastic dysphonia are reviewed with special emphasis on the etiology of the disorder. An analysis of the circumstances surrounding the onset of the disease points strongly to a psychogenic origin of the disease in, at least, the vast majority of the cases.

Dynamic Study of Oesophageal Function during Phonation: Simple but Effective.

Secondary aphonia significantly affects the quality of life of the laryngectomy patient despite the excellent success of the operation. Vocal rehabilitation often does not achieve the desired results, given the innumerable prognostic variables. Tracheo-oesophageal speech is considered the standard gold method of voice restoration, but a possible spasm of pharyngo-oesophageal segment is the prominent cause of rehabilitation failure. However, the phenomenon is difficult to identify and underestimated, and diagnostic methods are inquisitive or expensive. To propose a phonatory and swallowing eval…

[Voice quality following CO2 laser cordectomy].

The voice of patients after CO2 laser cordectomy was evaluated by subjective assessment, registration of voice parameters and sonegraphic classification. The results proved to be closely concordant, the main result being a slight or medium degree of dysphonia. Severe dysphonia or aphonia occurred in about one fifth of patients. This result is somewhat inferior to radiotherapy, but superior to standard translaryngeal cordectomy. Yanagihara's sonegraphic classification of dysphonia is recommendable for future comparative studies.
