The present study was designed to investigate the effects of microemulsion and aqueous solution containing transforming growth factor alpha (TGF-alpha) and/or aprotinin administered intragastrically (i.g.) on healing of acute gastric ulcers induced by acetylsalicylic acid (ASA). Many coronary patients on antiplatelet (PLT) drugs require coronary artery bypass grafting surgery under conditions that may not allow sufficient time for or warrant discontinuation of anti-PLT treatment. Recent clinical data show that intraoperative aprotinin significantly reduces postoperative bleeding and transfusion requirements in this patient population. If surgical bleeding persists, the administration of Factor VIIa should be considered.

A case of amniotic fluid embolism during the delivery of a twin pregnancy is reported. After immediate resuscitation with artificial ventilation the mother and the neonates survived the acute episode. As a result of hyperfibrinolysis with premature separation of the placenta a dramatic postpartum haemorrhage occurred, which could be controlled by Fritsch' manoeuvre. Meanwhile it was possible to restore the coagulation disorder with fresh whole blood, fibrinogen and an antifibrinolytic agent (Aprotinin).

A standard experimental shock model was used to investigate the nature and treatment of refractory shock. Peripheral vasodilatation secondary to hypoxia appeared to be the primary problem in pathogenesis, such factors as myocardial or reticuloendothelial depression being of a secondary nature. Protease inhibition using aprotinin (Trasylol) was ineffective in therapy, but glucocorticoids in pharmacological dosage produced a significant improvement in cardiodynamic and oxygen metabolic factors, provided that the agent was administered early.

Differences in heart stability of human pancreatic ribonuclease (cRNase) and serum ribonuclease were abolished by aprotinin, suggesting that the pancreatic enzyme was similar to the serum enzyme, but was being destroyed by proteases. Serum ribonuclease levels in normal subjects correlated with age but were unaffected by meal ingestion. Serum ribonuclease was not found to be useful in the detection of pancreatic cancer and was more frequently abnormal in patients with other solid tumours or renal failure.

A report of the results of a multicenter study including 4686 patients concerning the therapeutic value of Trasylol (Aprotinin) in traumatic shock. The group treated additionally with Trasylol showed, beside a reduced overall mortality, a statistically significant reduction of mortality in those treated within the first 30 min after the injury. Surgery performed in the posttraumatic stage showed a lower mortality in the Trasylol group. The importance of a time factor in shock treatment and the effects of Trasylol in traumatic shock are emphasized.

Numerous transfusion issues arise in the support of solid organ transplantation. ABO and HLA can cause graft rejection immunity or problems in transfusion support. Blood usage has been reduced by the use of erythropoietin in renal failure and aprotinin during liver transplants. Multifactorial coagulopathy during liver transplant surgery continues to be a challenge requiring close monitoring, however. The beneficial effect of transfusions on graft survival has lessened in recent years, but remains an area of active research.
