Cost-effectiveness analysis of sacral neuromodulation for faecal incontinence in The Netherlands.

AIM: Sacral neuromodulation (SNM) plays a major part in the algorithm of management of faecal incontinence, but there are limited data on its cost-effectiveness. This study aimed to analyse this and the quality-adjusted life-years (QALYs) associated with two different treatment algorithms. The first (SNM-) included use of an artificial sphincter [dynamic graciloplasty (DGP) (50%) and artificial bowel sphincter (ABS) (50%)]. The second (SNM+) included SNM (80% of cases) and artificial sphincter (DGP 10%; ABS 10%) The incidence of sphincteroplasty was assumed to be equal in both algorithms. MET

Sustained delivery of glial cell-derived neurotrophic factors in collagen conduits for facial nerve regeneration.

UNLABELLED: Facial nerve injury caused by traffic accidents or operations may reduce the quality of life in patients, and recovery following the injury presents unique clinical challenges. Glial cell-derived neurotrophic factor (GDNF) is important in nerve regeneration; however, soluble GDNF rapidly diffuses into body fluids, making it difficult to achieve therapeutic efficacy. In this work, we developed a rat tail derived collagen conduit to connect nerve defects in a simple and safe manner. GDNF was immobilized in the collagen conduits via chemical conjugation to enable controlled release of

Asphalt pavement emission behavior under solar radiation during in-service period.

The toxic fume emitted from asphalt pavement remains a health and environmental hazard towards public safety, especially the emission of volatile organic compounds (VOCs) and polycyclic aromatic hydrocarbons (PAHs). Despite extensive studies focused on characterizing asphalt fumes generated during construction stages (i.e. 150-180°C) have already been conducted, the long-term emissions induced by solar radiation during its service life are relatively unexplored (i.e. 60-70°C), especially the emission composition trends and their relationship with asphalt composition. In this study, three commo

A Comprehensive Review of Diagnostic Approaches to Vocal Fold Paralysis Using Artificial Intelligence.

Vocal fold paralysis, a condition caused by nerve damage, leads to impaired vocal fold movement, impacting speech, breathing, and swallowing. Diagnosing this condition is challenging as conventional methods are invasive. Even after requiring specialized equipment, the diagnosis often fails to confidently distinguish it from similar disorders. This results in treatment delay, affecting the patient's quality of life. Artificial intelligence can be trained on datasets to identify patterns in data that humans can miss, especially in cases where the disease incidence is not common. This narrative r

If Turing Played Piano With an Artificial Partner.

Music is an inherently social activity that allows people to share experiences and feel connected with one another. There has been progress … self-other integration. We discuss what principles may be needed, including open-ended evolution in Artificial Life, to arrive at artificial partners that enable genuinely interactive experience.

Bionic artificial skin with a fully implantable wireless tactile sensory system for wound healing and restoring skin tactile function.

Tactile function is essential for human life as it enables us to recognize texture and respond to external stimuli, including potential threats with sharp objects that may result in punctures or lacerations. Severe skin damage caused by severe burns, skin cancer, chemical accidents, and industrial accidents damage the structure of the skin tissue as well as the nerve system, resulting in permanent tactile sensory dysfunction, which significantly impacts an individual's daily life. Here, we introduce a fully-implantable wireless powered tactile sensory system embedded artificial skin (WTSA), wi

The role of endocrine mechanisms in ventilator-associated lung injury in critically ill patients.

The critically ill subjects are represented by a heterogeneous group of patients suffering from a life-threatening event of different origin, e.g. trauma, cardiopulmonary failure, surgery or sepsis. The majority of these patients are dependent on the artificial lung ventilation, which means a life-saving chance for them. However, the artificial lung ventilation may trigger ventilation-associated lung injury (VALI). The mechanical ventilation at higher volumes (volutrauma) and pressure (barotrauma) can cause histological changes in the lungs including impairments in the gap and adherens junctio

Artificial Intelligence in the Management of Hereditary and Acquired Hemophilia: From Genomics to Treatment Optimization.

Hemophilia, an X-linked bleeding disorder, is characterized by a deficiency in coagulation factors. It manifests as spontaneous bleeding, leading to severe complications if not properly managed. In contrast, acquired hemophilia is an autoimmune condition marked by the development of inhibitory antibodies against coagulation factors. Both forms present significant diagnostic and therapeutic challenges, highlighting the need for advanced genetic, molecular, laboratory, and clinical assessments. Recent advances in artificial intelligence have opened new avenues for the management of hemophilia. M

Freeze-Dried Platelet-Rich Plasma Accelerates Bone Union with Adequate Rigidity in Posterolateral Lumbar Fusion Surgery Model in Rats.

Fresh platelet-rich plasma (PRP) accelerates bone union in rat model. However, fresh PRP has a short half-life. We suggested freeze-dried PRP (FD-PRP) prepared in advance and investigated its efficacy in vivo. Spinal posterolateral fusion was performed on 8-week-old male Sprague-Dawley rats divided into six groups based on the graft materials (n = 10 per group): sham control, artificial bone (A hydroxyapatite-collagen composite) -alone, autologous bone, artificial bone + fresh-PRP, artificial bone + FD-PRP preserved 8 weeks, and artificial bone + human recombinant bone morphogenetic protein 2

Nutritional Interventions in Chronic Intestinal Pseudoobstruction.

Although chronic intestinal pseudo-obstruction (CIPO) is a rare disorder, it presents a wide spectrum of severity that ranges from abdominal bloating to severe gastrointestinal dysfunction. In the worst cases, patients may become dependent upon artificial nutrition via parenteral nutrition or choose to have an intestinal transplant. However, whatever the severity, a patient's quality of life can be seriously compromised. This article defines the disorder and discusses the spectrum of disease and challenges to providing adequate nutrition to help improve a patient's quality of life.

Prospective randomized study comparing permanent pacing with rate drop response and closed loop stimulation in patients with vasovagal syncope where permanent pacing is indicated and selected as the appropriate treatment option.

BACKGROUND: Pacing for vasovagal syncope is established. Two pacing algorithms are available. The rate-drop-response (RDR-Medtronic) is triggered by falling heart rate acting with modified rate-hysteresis. The closed loop stimulation or system (CLS-Biotronik) is triggered by impedance changes in the right ventricle reflecting falling volume and rising contractility. These are very different physiologically. Both algorithms carry favorable reports in clinical use. METHODS: A randomized-controlled superiority trial is proposed to compare the two algorithms for the control of vasovagal syncope i

Sustaining humans in space.

One goal of NASA's Ames Research Center is to extend the presence of humans in space. Biomedical research, human performance, and life-support systems form the core of the Ames program. Major questions highlight how the effects of microgravity on living systems are modified by exercise, artificial gravity, autogenic feedback training, and nutrition. Bioengineering and life science studies include humans, animals, and plants. Ames investigators collaborate extensively with outside university and commercial scientists, both within the United States and internationally.

A novel artificial neural network for the diagnosis of orofacial pain and temporomandibular disorders.

BACKGROUND: Temporomandibular disorders (TMD) and orofacial pain are highly prevalent. This prevalence can be compared to that of leading non-communicable diseases (NCDs). However, it is surprising to still find a high degree of controversy regarding its diagnosis and management. Patients usually experience treatment delays, missed diagnoses, and receive unnecessary therapies. New artificial intelligence algorithms have helped diagnose numerous diseases. Nevertheless, no studies have focused on the use of artificial intelligence to diagnose these conditions. OBJECTIVES: This study aimed to de

A Bedside-to-Bench-to-Bedside Journey to Advance Osteochondral Allograft Transplantation towards Biologic Joint Restoration.

More than 70 million adults in the United States are impacted by osteoarthritis (OA). Symptomatic articular cartilage loss that progresses to debilitating OA is being diagnosed more frequently and earlier in life, such that a growing number of active patients are faced with life-altering health care decisions at increasingly younger ages. Joint replacement surgeries, in the form of various artificial arthroplasties, are reliable operations, especially for older (≥65 years), more sedentary patients with end-stage OA, but have major limitations for younger, more active patients. For younger adul
