Current management of tarsometatarsal injuries in the athlete.

The frequency of foot injuries is increasing in certain athletes, particularly injuries to the tarsometatarsal joint complex. A high index of suspicion for this injury is required to make the diagnosis because the clinical signs often are subtle. A comprehensive examination along with bilateral weight-bearing plain radiographs of the foot should be obtained in any suspected midfoot injury. Further imaging studies and stress radiographs may assist in the diagnosis and direct management. Nonsurgical treatment can be considered in a stable sprain with less than 2 mm of diastasis between the first

Injury patterns and biomechanics of the athlete's shoulder.

Although shoulder injury patterns vary by sport and position, overhead sports, such as baseball, swimming, tennis, and volleyball, share a common dependence on the integrity of the kinetic chain. Disruption of this critical mechanism predisposes to injury and deserves attention as the most important of the biomechanical principles required to help evaluate and manage problems of the athlete's shoulder.

Management of cervical spine injuries in athletes.

OBJECTIVE: Although the incidence of catastrophic cervical spine injury in sport has been significantly reduced over the past 3 decades, the injury warrants continued attention because of the altered quality of life that often accompanies such an injury. The purpose of our literature review was to provide athletic trainers with an understanding of the mechanisms, anatomical structures, and complications often associated with sport-related cervical spine injury. We also present the most current recommendations for management and treatment of these potentially catastrophic injuries. DATA SOURCE

Musculoskeletal injuries in the pediatric and adolescent athlete.

There are approximately 35 million children participating in organized sports in the United States. With this increasing participation, we have seen an overall increase in sports-related injuries in young athletes over the past 20 years. Young athletes suffer both acute and chronic, or overuse, injuries. They are susceptible to many of the same injuries as their adult counterparts, but due to the effects of growth on the musculoskeletal system, they are at risk for injuries to the growth plate, apophysis, and joint surface. Common acute and overuse injuries seen in young athletes are discussed

Imaging of hip and groin injuries in athletes.

Proper function of the hip joint is imperative for athletes who participate in sporting activities that rely on utilization of the lower extremity; these primarily include kicking, running, and jumping activities. Sports-related injuries of the hip and groin are not frequent sources of disability in athletes; however, they may present a significant diagnostic dilemma, both from a clinical and radiological standpoint. Delay in diagnosis and treatment may result in undesired complications, such as premature degenerative disease, as well as time lost from athletic activities. Magnetic resonance (

Upper extremity arterial injuries in athletes.

Upper extremity arterial injury resulting in hand and digit ischemia can occur in athletes who perform repetitive, high-stress, overhead arm motions. The initial presentation of these injuries often mimics the more common musculoskeletal injuries found in these athletes, and therefore, a high index of suspicion is essential to establish the diagnosis in a timely fashion. There are several described mechanisms of injury, most of which involve extrinsic compression and injury to the axillary artery or its branches from the humeral head. The clinical diagnosis is confirmed with noninvasive vascul

Upper extremity injuries in the paediatric athlete.

Injuries to the upper extremity in paediatric and adolescent athletes are increasingly being seen with expanded participation and higher competitive levels of youth sports. Injury patterns are unique to the growing musculoskeletal system and specific to the demands of the involved sport. Shoulder injuries include sternoclavicular joint injury, clavicle fracture, acromioclavicular joint injury, osteolysis of the distal clavicle, little league shoulder, proximal humerus fracture, glenohumeral instability and rotator cuff injury. Elbow injuries include supracondylar fracture, lateral condyle frac

Elbow, forearm and wrist injuries in the athlete.

Competitive and recreational athletes sustain a wide variety of soft tissue, bone, ligament, tendon and nerve damage to their upper extremities. Most such injuries are related to direct trauma or repetitive stress, and account for a significant amount of 'down time' for athletes participating in a wide range of sports, particularly those in which the arm is utilised for throwing, catching or swinging. Overuse injuries to the elbow include musculotendinous injuries, ulnar nerve injuries and ligamentous injuries. Osteochondrol lesions of the capitellum and posterior impingement injuries in the j

Upper extremity injuries in young athletes.

With the knowledge base of normal anatomy, development, biomechanics, and differential diagnosis, the sports medicine professional can treat injured young athletes with greater efficiency. In addition, microtraumatic injuries may be prevented by emphasizing safe parameters of participation, proper throwing techniques, and careful monitoring of the amount of practice time and intensity. Gymnasts using apparatus should always have spotters. The height of towers and basket tosses by cheerleaders should be limited by age and ability. Proper pitching techniques, not the fastest pitch or youngest cu

Lower leg overuse injuries in athletes.

Achilles paratenonitis and medial tibial syndrome are the most common specific overuse injuries among athletes in Finland, and they are a problem especially in endurance sports, such as long-distance running and jogging. Conservative treatment is often successful, but if it fails operative treatment is necessary. The surgical methods developed in Finland, which are widely used in the treatment of Achilles paratenonitis and medial tibial syndrome, are presented with clinical follow-up results in this paper. The patients operated on for Achilles paratenonitis were some years older than the patie

Rehabilitation of athletes following spine injury.

We feel that the described system for staging cervical and lumbar spine injuries in the high-performance athlete affords a basic framework for the rehabilitation of those individuals. With spine injuries, one has to exercise care to rule out those injuries and additional pathologies presenting as neck or back pain that could result in catastrophic sequelae. We stress due diligence in the initial and continued follow-up of these patients with careful evaluation to rule out associated fractures, dislocations, and attendant disc pathology. The treatment of the "stinger" or "burner" is controversi

Overuse Knee Injuries in Young Athletes.

In brief Because of their skeletal immaturity, children and adolescents are subject to a somewhat different set of overuse knee injuries than are adults. Described here are the diagnosis and treatment for the most common growth-related causes of knee pain in active young people: Osgood-Schlatter disease, Sinding-Larsen-Johansson disease, osteochondritis dissecans, bipartite patella, medial plica syndrome, and discoid meniscus. In most cases these conditions respond to conservative treatment.

Peripheral nerve injuries of the shoulder in the athlete.

Athletic injuries about the shoulder resulting in lesions of the suprascapular, axillary, long thoracic, musculocutaneous, and spinal accessory nerves are uncommon. Yet these lesions are well described in the literature, and they may occur as isolated injuries. Rarely do they occur following surgical procedures about the shoulder of the athlete. Furthermore, these injuries may mimic other more commonly occurring athletic injuries thus giving rise to diagnostic confusion. The clinical syndromes that result from these isolated lesions have been discussed, along with their mechanisms of injury. I

Care of the Aging Athlete: Interventional Treatment Options for Musculoskeletal Injuries in the Masters Athlete.

As the population of aging athletes continues to grow, the number of older individuals presenting to sports medicine clinics seeking musculoskeletal care is expected to increase. Common conditions treated in the older athlete include osteoarthritis and overuse soft tissue injuries such as tendinopathy and fasciopathy. Initial treatment for such conditions is conservative. However, there is a significant knowledge gap on the next best steps in management when the older athlete becomes refractory to conservative treatments or if medications and surgery are contraindicated. In such circumstances,

Material properties: a factor in the selection and application of splinting materials for athletic wrist and hand injuries.

Athletic injuries to the wrist and hand are common in clinical practice. When indicated, the use of a protective playing splint enables athletes to continue sports participation. The purpose of this paper is to discuss the splinting material properties that determine the materials' suitability for use in fabrication of a protective playing splint. A second aim is to present currently available splinting materials. The clinical reasoning process for material application and selection is based on understanding material properties, the type and severity of the injury, phase of treatment, and type
