Management of acute atrial fibrillation.

Atrial fibrillation (AF) is the most common cardiac arrhythmia; despite many available guidelines the optimal management strategy remains elusive. The most common presentations of acute AF are palpitations, breathlessness, syncope, dizziness, chest pain, transient ischaemic attack (TIA) and stroke. Patients may need urgent electrical cardioversion (ECV) or pharmacological cardioversion (PCV) and long term anticoagulation therapy. This review aims to discuss some issues surrounding the early management of acute AF in the Acute Medical Unit.

Stroke prevention in atrial fibrillation.

The only major and potentially fatal risk for patients with atrial fibrillation is the development of systemic thromboembolism. Stroke occurs five times more frequently in patients with atrial fibrillation than in comparable patients in sinus rhythm. The yearly incidence of stroke in atrial fibrillation largely depends on the underlying heart disease: from 0.5% in "lone" atrial fibrillation up to 20% in rheumatic heart valve disease. Oral anticoagulation with vitamin K antagonists dramatically reduces the stroke risk by two-thirds, but is a laborious and patient-unfriendly therapy. Oral direct…

Atrial fibrillation after vardenafil therapy.

Vardenafil is a new oral phosphodiesterase inhibitor used for erectile dysfunction. We report a case admitted with a first-detected, symptomatic paroxysmal atrial fibrillation in a healthy patient after self-medication with vardenafil.

Antithrombotic treatment in atrial fibrillation.

Atrial fibrillation (AF) is the most common sustained arrhythmia, that substantially increases morbidity and mortality. AF is gaining in clinical and economic importance, with stroke and thromboembolism being major complications. In this article, the evidence for AF treatment trial of antithrombotic therapy is reviewed. Stroke risk stratification of patients with AF is discussed, and practical recommendations for thromboprophylaxis are presented.

Hybrid therapy of atrial fibrillation.

Antiarrhythmic drugs have shown a poor long-term efficacy in the management of atrial fibrillation. It has been suggested that the association of antiarrhythmic drugs and non-pharmacological treatments may be superior to the prescription of a single treatment only. Electrical cardioversion of atrial fibrillation can be ineffective in several cases (long-lasting atrial fibrillation, large atria, advanced age, underlying diseases, high transthoracic impedance): the prescription of antiarrhythmic drugs prior to electrical shock has been demonstrated to be able to increase the success rate and to …

[Atrial fibrillation in heart failure].

BACKGROUND: Atrial fibrillation is a common arrhythmia in patients with congestive heart failure caused by left ventricular dysfunction and is associated with significant morbidity and possibly increased mortality rates. It occurs with increasing frequency as the severity of heart failure increases. TREATMENT: As therapeutic options, two basic strategies are available: rhythm control with or without pharmacological manipulation to increase the chance of successful cardioversion and to maintain sinus rhythm, and rate control with anticoagulation. So far, a clear benefit of one of these two str…

[Antithrombotic drugs in atrial fibrillation].

Stroke and peripheral thrombo-embolism are common in all forms of atrial fibrillation which is responsible for a hypercoagulable state. Clinical and echocardiographic risk stratification allow therapeutic recommendations among the different antithrombotic strategies used in randomized studies (adjusted-dose oral anticoagulant, aspirin or a combination of aspirin and low-dose oral anticoagulant) in agreement with recent guidelines from the European and American associations of cardiology for the managMent of patients with atrial fibrillation.

Current treatments of atrial fibrillation.

Atrial fibrillation is the most common arrhythmia encountered in medical practice and is responsible for significant morbidity and mortality and consumes much of healthcare's finances. Controlling the patient's ventricular rate becomes the first priority, and digoxin, calcium channel blockers, and beta blockers all come under consideration for the individual patient. Evaluation and treatment of contributing factors such as thyrotoxicosis, acute infarct, or ethanol consumption must also always be addressed. Consideration can then be given to whether a patient should be anticoagulated and cardio…

Atrial fibrillation in elite athletes.

INTRODUCTION: Atrial fibrillation (AF) is a rare event in people younger than 25 years of age, but is probably more frequent in competitive athletes. We analyzed the presence of AF, paroxysmal or chronic, in a population of young elite athletes, including previous Olympic and World champions, who were studied for arrhythmias that endangered their athletic careers. METHODS AND RESULTS: From 1974 to June 1977, 1,772 athletes identified with arrhythmias (1,464 males and 308 females; mean age 21 years) underwent individualized work-ups. Among these, 146 (122 males and 24 females; mean age 24 year…

Preventing stroke in atrial fibrillation.

Atrial fibrillation, a common cardiac arrhythmia, is now recognized as a powerful risk factor for stroke. Previously, atrial fibrillation was thought to predispose persons to stroke only in the presence of rheumatic heart disease with mitral stenosis. The significant impact of nonvalvular atrial fibrillation on stroke incidence, recurrence and mortality was not fully appreciated. A series of clinical trials have confirmed that a five-fold increase in stroke incidence occurs in patients with atrial fibrillation, and that warfarin anticoagulation is efficacious in stroke prevention. This anticoa…

[Antithrombotic therapy in atrial fibrillation].

The known risk of embolic events in patients with recurrent or chronic atrial fibrillation makes it mandatory to recommend oral anticoagulation in patients with rheumatic mitral valve disease, specially mitral stenosis to maintain an INR between 2.0 and 3.0. If despite this treatment recurrent embolism occurs, the dose of oral anticoagulants should be increased (INR between 2.5 and 3.5) or dipyridamole (200 to 400 mg/day) or aspirin (160 to 320 mg/day) should be added to dicoumarinic drugs. In patients that must be cardioverted either electrically or pharmacologically and who have been on atri…

Uncommon etiologies of atrial fibrillation.

A newly diagnosed atrial fibrillation warrants a full investigation of the etiopathogenesis of this common arrhythmia. In the adult population, the most frequently associated conditions are systemic hypertension, coronary artery disease, mitral valvulopathy, congestive heart failure, and hyperthyroidism. Nevertheless, more infrequent and even rare, yet correctable, etiologies should not be overlooked. We describe three patients who presented to our hospital with a first episode of atrial fibrillation and who subsequently were demonstrated to have very unusual cardiovascular pathologies subtend…

[Antithrombotic therapy of atrial fibrillation].

In comparison with the incidence of cerebrovascular accident in the general population, atrial fibrillation increases the risk by a factor of five. Although age is without doubt the main risk factor for cerebrovascular accidents in patients with permanent of paroxysmal non-valvular atrial fibrillation, other independent risk factors have been identified: a previous history of hypertension, cerebrovascular accident, heart failure or diabetes. These factors enable identification of a population at risk in which oral anticoagulation may be recommended with an excellent efficacy/risk ratio. Six la…
