BACKGROUND AND PURPOSE: Juvenile rheumatoid arthritis (JRA) can result in disability, growth disturbance, and systemic complications. This study investigated the efficacy and adverse effects of azathioprine (AZA) therapy in children with JRA.
METHODS: Data from the medical records of 24 children with JRA treated with oral AZA during the period from 1988 to 1998 were retrospectively analyzed. The case of a man who presented with dyspnea and a dry cough six weeks after mediastinal radiotherapy for malignant thymoma is described. We report a case of azathioprine-induced haemorrhagic alveolitis, in a 14-year-old boy, after renal transplantation. On day 25 the patient developed haemoptysis, fever and hypoxaemia. Chest X-ray showed diffuse reticulo-nodular shadows in both lung fields. In 167 unselected patients with renal allografts, after a lag of 3.5 years the prevalence of dysplastic keratotic lesions increased linearly by 6.8%/yr and number of lesions also increased. There was no relationship to sun exposure or skin type. Malignancies occurred in 5 patients after 9 years. Viral warts occurred in 42% but prevalence and extent were not related to time after transplantation or keratoses. A 67-year-old woman with biopsy-proven autoimmune hepatitis (AIH) developed acute pancreatitis shortly after starting azathioprine (AZA) and budesonide. She presented with abdominal pain, fever, and elevated lipase levels. We describe the case of a pediatric patient on azathioprine therapy with previously undiagnosed homozygote thiopurine S-methyltransferase (TPMT) deficiency, resulting in myelotoxic thiopurine metabolite levels. The patient was successfully treated with a very low azathioprine dose of 50 mg once a week (4% of standard dose), guided by frequent thiopurine metabolite measurement and a close clinical surveillance. We demonstrate that azathioprine therapy still might be an effective and safe therapeutic option in pediatric thiopurine S-methyltransferase-deficient IBD patients.
