Gastric emptying and duodeno-gastro-oesophageal reflux in gastro-oesophageal reflux disease.

BACKGROUND: Previous studies present conflicting results regarding relationship between gastric emptying and gastro-oesophageal reflux disease. Reflux of duodenal content to oesophagus is generally considered to be associated with more severe disease. AIM: To assess presence of a gastric emptying disorder in persons with reflux of duodenal contents to oesophagus and to identify any correlation with gastric emptying and oesophageal motility. METHODOLOGY: A total of 15 subjects with (B+) and 15 subjects without (B-) bile reflux to oesophagus determined by 24-hour bilirubin monitoring were stud…

Oesophageal and gastric bile exposure after gastroduodenal surgery with Henley's interposition or a Roux-en-Y loop.

BACKGROUND: The degree which the various reconstruction techniques prevent bile reflux after gastroduodenal surgery has been poorly studied. METHODS: Bile exposure in the intestinal tract just proximal to the jejunal loop was measured with the Bilitec 2000 device for 24 h after gastroduodenal surgery in three groups of patients. Group 1 comprised 24 patients with a 60-cm Henley's loop after total gastrectomy. Group 2 included 31 patients with a 60-cm Roux-en-Y loop after total (22 patients) or subtotal (nine) gastrectomy. Group 3 contained 21 patients with a 60-cm Roux-en-Y loop anastomosed t…

Bile acids and Barrett's oesophagus: a sine qua non or coincidence?

BACKGROUND: Barrett's oesophagus (BO), a premalignant condition associated with the development of oesophageal adenocarcinoma (OAC), is thought to be a consequence of chronic duodeno-gastro-oesophageal reflux. Of the refluxates, bile acids, either alone or in combination with acid, are probably the most important. METHODS: Analysis of the literature on the role played by bile acids in inducing BO and/or progression to OAC. RESULTS: Combined pH and Bilitec 2000 (as a measure of bile reflux) monitoring and oesophageal aspiration studies in humans suggest a combined role for bile acids, particu…

[Spur valve jejunal interposition in choledochus cystectomy].

During 1983-1990, 113 patients with congenital choledochus cysts were operated upon with the procedure called the spur valve jejunal interposition following total cystectomy, and satisfactory results were recorded in two years follow-up. However, early complications happened in 26.5% patients e.g. postoperative bile leakage, blood vomiting, small intestinal intussusception, adhesive intestinal obstruction, chyliform ascites, and disruption of wound. Six months-2 years after operation, reflux into bile duct and rapid emptying were observed in some cases in barium examination, and abdominal pain

[Postoperative alkaline reflux gastritis (author's transl)].

Postoperative alkaline reflux gastritis is a distinct clinical entity occuring after operations enlarging, bypassing or resecting the pylorus. Reflux of alkaline duodenal content into the stomach is the causative factor. Primarily bile acids have an aggressive effect and lead to a destruction of the gastric mucosal barrier. Epigastric pain, fullness after meals and bile vomiting are the main symptoms. Gastroscopy with biopsy reveals a severe chronic atrophic gastritis and bile reflux. In most cases an achlorhydria that can be histamin-resistant is present. For adequate treatment surgical proce…

Comprehensive assessment of gastroscopic findings and quality of life using the GERD-HRQL questionnaire following single anastomosis sleeve ileal (SASI) bypass.

PURPOSE: Single Anastomosis Sleeve Ileal (SASI) bypass effectively alleviates GERD symptoms but may increase bile reflux risk. This study evaluates the effects of SASI bypass impact on endoscopic GERD features and GERD-related quality of life. METHODS: This single-center study is a retrospective analysis of prospectively collected data from patients who underwent SASI bypass between March 2020 and August 2022. RESULTS: Esophagitis prevalence significantly decreased from 51.4% preoperatively to 20.0% after SASI bypass, p = 0.010. Notably, esophagitis grade B, present in 17.1% of patients before…

An ambulatory bilirubin monitoring device for duodenogastroesophageal reflux.

Reflux of duodenal contents into the stomach is a normal physiological event occurring most commonly at night' but also in the fasting and postprandial daytime periods.' Previously, the terms "bile reflux" and "alkaline reflux" have been used to describe this process. However, duodenal contents contain more than just "bile" and studies have shown that the term "alkaline reflux" is a misnomer since pH > 7 does not correlate with reflux of duodenal contents.' Therefore, duodenogastroesophageal reflux (DGER) may be a more appropriate term to describe the pathological regurgitation of duodenal con…

Gastric ulcer after the Nissen fundoplication: a complication of alkaline reflux.

Four of a series of 40 patients who underwent the Nissen fundoplication to correct reflux oesophagitis developed gastric ulceration 24, 22, 15 and 13 months respectively after the procedure. The gastric ulceration is attributed to bile reflux, and the original oesophagitis to "alkaline" reflux. When severe oesophagitis results from "alkaline" reflux, a Roux-en-Y bile diversion operation with vagotomy and fundoplication should be considered.

Role of Helicobacter pylori infection and duodenogastric reflux in the pathogenesis of alkaline reflux gastritis after gastric operations.

In patients undergoing gastric operations, we studied the relationship between data suggesting alkaline reflux gastritis (symptoms, endoscopic alterations and histologic lesions) and two factors that produce chronic gastritis (helicobacter pylori and duodenogastric reflux). Of 225 patients who underwent operations for gastroduodenal gastric ulcer at our General Surgery Unit between 1980 and 1982, 63 agreed to undergo endoscopy and biopsies. Of these 63 patients, 38 agreed to a test to quantify duodenogastric reflux (24 hour gastric pH monitoring associated with the determination of bile acids

Roux-en-Y versus interposition biliary reconstruction.

We compared two types of biliary reconstruction for congenital dilatation of the bile duct--Roux-en-Y hepaticojejunostomy and jejunal interposition hepaticoduodenostomy after removal of the dilated bile duct. There were no significant differences in the occurrence of early and late complications except for the repeated attacks of abdominal pain that occurred only in the group that underwent jejunal interposition hepaticoduodenostomy. The results of further examinations for the presence of bile reflux with 99mTc-pyridoxyl methyl tryptophan scintigraphy and gastroscopy in this group revealed a h…

Gastro-oesophageal reflux: the disease of the millennium.

Gastro-oesophageal reflux disease (GORD) is attributable primarily to acid and bile reflux through the lower oesophageal sphincter. Bile reflux has been implicated in severe oesophagitis, Barrett's oesophagus and oesophageal adenocarcinoma. Developments in evaluating and managing GORD and Barrett's are discussed, with their aetiological role in junctional gastro-oesophageal malignancies.

Development of a novel reflux-free bilioenteric anastomosis procedure by using a bioabsorbable polymer tube.

BACKGROUND: Bilioenteric anastomosis allows reflux of bowel fluid into bile ducts, resulting in reflux cholangitis. We sought to develop a reflux-free procedure of bilioenteric anastomosis endowed using an artificial bile duct (ABD). METHODS: The study was performed in four groups of pigs. In Group A, end-to-side choledochoduodenostomy using the interposed ABD with its distal end wrapped by the duodenal seromuscular layer was performed. In Group B, conventional end-to-side choledochoduodenostomy using the ABD was performed. In Group C, conventional end-to-side choledochoduodenostomy was perfo

Risk Factors for Bile Aspiration and its Impact on Clinical Outcomes.

INTRODUCTION: Bile reflux may cause for lung allograft rejection, yet there are no studies that determine (i) the relationship between gastric and lung bile concentrations, (ii) whether bile is present in lungs of nontransplant patients, (iii) the relationship between gastric dysmotility and lung bile, (iv) the impact of reflux therapies on lung bile, and (v) whether lung bile worsens outcomes in nontransplant patients. This study will address these gaps in the literature. METHODS: We prospectively recruited lung transplant (LTX) patients and nontransplant patients with respiratory symptoms (…

Development of Barrett's esophagus after a total gastrectomy.

Bile reflux is a complication that could develop in patients with total gastrectomy. The role of biliary reflux in Barrett's esophagus after a total gastrectomy continues to be controversial. Therefore, we report a series of three cases that describe the development of BE after total gastrectomy and provides additional support on the role that bile reflux plays in patients with BE.

Pancreatobiliary reflux and the length of a common channel.

BACKGROUND/PURPOSE: Gallbladder cancer occurs frequently in patients with pancreaticobiliary maljunction due to pancreatobiliary reflux. Pancreatobiliary reflux is also detected in some patients with a relatively long common channel. This study aimed to clarify the correlation between pancreatobiliary reflux and the length of a common channel. METHODS: Two hundred and three patients, in whom both the length of a common channel and amylase level in the bile were measured, were enrolled from nine centers. RESULTS: Bile amylase level was correlated with the length of a common channel (P < 0.01)

A simple method for the quantification of biliary reflux.

99mTc-diethyl-IDA is completely excreted into the bile. When cholecystokinin is given after priming of the biliary tract with this tracer, gallbladder contraction leads to expulsion of bile into the duodenum. At the same time cholecystokinin causes contraction of the pylorus, which should normally prevent substantial reflux of tracer into the stomach. We have applied these physiological characteristics in a method to quantify biliary gastric reflux. Fourteen controls had a median reflux of 4.3% of the intravenous dose (93% of controls had values less than 9%). In 18 patients with Billroth II g

'Bile' in the oesophagus.

Acid gastro-oesophageal reflux occurs when the lower oesophageal sphincter is incompetent, but oesophagitis caused by reflux of duodenal content implies incompetence of both the pyloric and gastro-oesophageal sphincters. The term 'alkaline' reflux oesophagitis was coined long before objective analysis was made of bile in the stomach and oesophagus, and well before pH monitoring was introduced. Surgical procedures to divert bile from the stomach and oesophagus were developed on a clinical basis and gave encouraging results in the management of peptic oesophageal stricture. Alkaline oesophagitis

Composition of gastro-oesophageal refluxate.

Fifty two patients with abnormal acid gastro-oesophageal reflux were studied by simultaneous oesophageal pH monitoring and continuous aspiration for 16 hours. Aspirates (from discrete two hour periods) were analysed for volume, pH, bile acids (conjugated and unconjugated), trypsin, and pepsin. The results were compared with pH changes and degree of oesophagitis. Patients with oesophagitis had greater acid reflux than those without, but patients with stricture and Barrett's oesophagus had similar acid reflux to those with uncomplicated erosive oesophagitis. Pepsin concentrations were highest in
