PRECICE intramedullary nail in the treatment of adult leg length discrepancy.

BACKGROUND: Limb length discrepancy in adults is not an uncommon occurrence following trauma and limited literature exists on limb lengthening in skeletally mature populations using modern techniques. This study aimed to evaluate outcomes of limb lengthening surgery using the PRECICE magnetic limb lengthening intramedullary nail in an adult population. METHOD: From 2013 to 2018, 21 adult patients were operated by a single surgeon, using the PRECICE nail, for lower limb length discrepancies. Low energy femoral osteotomies were performed via a limited approach. Tibial osteotomies were performed

On-top plasty using a free metacarpal head graft for lengthening of proximal phalanx in symbrachydactyly--a case report.

A three-year old patient with symbrachydactyly (didactyly type) presented with a little finger that was too short to allow pinching and consisting of a floppy soft-tissue envelope with hypoplastic phalanges, although the thumb was functional. As the proximal phalanx was too small to permit distraction lengthening or conventional bone grafting, on-top plasty using a 4th metacarpal graft with a cartilage head was undertaken for lengthening the proximal phalanx of the little finger for pinch reconstruction. At ten weeks after surgery, the patient achieved satisfactory pinch function due to the le

Limb lengthening for post-traumatic shortening over a pre-implanted femoral locking plate.

Attempts have been made to refine techniques involved in distraction osteogenesis to improve patient compliance and reduce complications. The prolonged use of external fixators for limb lengthening is associated with a number of problems, and in addition patient satisfaction with a cumbersome external fixator may be low. To reduce the complication rate and facilitate early removal of the external fixator by protecting the regenerate bone, lengthening over an intramedullary nail is being increasingly described. These techniques have limitations when used in skeletally immature patients and carr

Three-dimensional printing as a technology supporting the treatment of lower limb deformity and shortening with the Ilizarov method.

BACKGROUND: Treatment of multiplanar deformities, especially in younger children, requires construction of a complex Ilizarov fixator, frequently with small dimensions. The aim of this study is to verify clinical application of a3D-printed bone model in treatment with the Ilizarov method. METHODS: The study involved a 6-year-old child in whom clinical and radiological examination revealed multiplanar deformity of the right leg. Then, 3D models of individual bones were printed by means of additive manufacturing and were used as a scaffold to install the Ilizarov apparatus. To compare the expec

Modification of the Wagner lengthener to permit dynamic axial loading.

The Wagner lengthener is mechanically excellent, but it lacks a function allowing dynamic axial loading (dynamization), which stimulates bone consolidation at the lengthened section. The authors devised a simple metal insert with which they modified the Wagner lengthener to permit dynamization. The modified appliance was successfully used in two patients.

Trifocal Tibial Bone Transport Using a Magnetic Intramedullary Nail: A Case Report.

CASE: The patient sustained a pathologic fracture through chronic osteomyelitis of the tibia. Treatment involved resection of bone and soft tissue, leaving a 15-cm diaphyseal tibial defect. We designed a construct to allow for trifocal bone transport using a magnetic intramedullary lengthening nail connected to cables. This technique uses cables secured to the proximal tibia, the nail, and the transport segments. CONCLUSION: This case details the successful use of trifocal cable-assisted bone transport through a magnetic lengthening intramedullary nail. This technique can successfully be used

[Progressive lengthening of the upper limb in 2 patients].

In one case, a short humerus due to trauma in infancy was lengthened by 7 cm at the age of 15 years. The operation was done in two stages. At the first stage, through a lateral approach with mobilisation of the radial nerve, an osteotomy followed by a progressive lengthening of the humerus, stabilised by an external fixator was used. The second stage employed a medial approach with a bone graft and bone fixation. In the second case, lengthening of 7 cm of the ulna to correct shortening due to a congenital defect was performed. In both cases the functional and cosmetic results were satisfactory

Lengthening of the humerus for upper extremity limb length discrepancy.

We present a case of humeral lengthening for a limb length discrepancy of 7.7 cm due to proximal humeral growth arrest in a 12-year-old boy. Lengthening of 69 mm was achieved using the Wagner technique. Bone grafting was not required. Humeral lengthening may be performed for upper extremity limb length discrepancy with good results. Objectives of treatment must include protection of the radial nerve from traction injury and from entrapment in the osteotomy site. Elbow extension may be lost during lengthening, but in the adolescent age group, full return of elbow motion can be expected.

[Bone distraction osteogenesis].

Bone osteogenesis has been used by many surgeons to compensate dissymmetry due to congenital malformations, diseases or traumas. Bone distraction is the process of generating new bone in a gap between two bone segments in response to the application of graduated tensile stress across the bone gap. Different types of distraction are actually proposed to be an alternative to facial osteotomies or bone grafting techniques. Distraction can be unilateral, bilateral and correct the deficiencies in one, two or three dimensions. The advantages of the distraction osteogenesis are the possibilities to o

Treatment of 18 cm lower limb length discrepancy using lengthening over nail technique in tibia and lengthening and plating in femur - A case report.

INTRODUCTION AND IMPORTANCE: The management of extreme limb-length discrepancy remains a challenge for surgeons. Limb lengthening using an external fixator is a popular method for managing limb-length discrepancy; however, it had many complications. Other techniques using external fixators have been described, such as lengthening over a nail (LON) technique and lengthening and then plating (LATP), which decrease external fixator duration, equinus contracture, pin-site infection, bone alignment, and bone fracture. Only a few cases of management of extreme limb-length discrepancy due to hip dysp

Bilateral humeral lengthening and deformity correction with hexapod external fixator in an achondroplastic patient: A case report.

INTRODUCTION AND IMPORTANCE: Achondroplasia, a prevalent skeletal dysplasia, often results in limb asymmetry and functional limitations, typically managed surgically with bone lengthening techniques. CASE PRESENTATION: We present the case of a 10-year-old girl with achondroplasia who underwent bilateral humeral lengthening using hexapod external fixators. The patient, previously treated for lower limb lengthening, exhibited significant upper limb shortening affecting daily activities. Surgical intervention involved two stages, employing two hexapod external fixators for precise correction and

Management of infected non-union of subtrochanteric fracture: Two cases.

Infected non-union of subtrochanteric fractures is challenging to treat. We experienced two cases and had good clinical results. Treatment strategy comprised debridement without hesitation after considering later limb lengthening; insertion of the proximal lateral bone edge spike into the distal bone marrow cavity until achieving medial-side bony contact and holding good alignment to compensate for the medial-side bone loss, according to the modified Dimon method; and internal fixation with an angled plate in the decubitus position. The angle of the angled plate should be directed toward the a

Staged reconstruction of unilateral neglected hip dislocation through total hip arthroplasty and subsequent intramedullary femoral lengthening.

BACKGROUND AND PURPOSE: Total hip arthroplasty (THA) is a successful approach to treat unilateral symptomatic neglected hip dislocation (NHD). However, the extensive leg length discrepancy (LLD) can hereby only be partially corrected. In case of residual LLD of more than 2 cm, subsequent femoral lengthening can be considered. PATIENTS/MATERIAL/METHODS: Retrospective analysis of clinical data and radiographs of five patients (age 38.1 (28-51) years) with unilateral NHD who underwent THA with (n  = 3) or without (n = 2) subtrochanteric shortening osteotomy (SSO) and secondary intramedullary fem

Simultaneous ipsilateral femoral and tibial lengthening with the Ilizarov method.

Eight pediatric patients who underwent nine simultaneous ipsilateral femoral and tibial lengthenings with the Ilizarov external fixator were reviewed. The patient's demographics, diagnoses, corticotomy levels, mechanical axes, healing indices, amounts of lengthening, and complications were recorded. The patients' average age was 8 years 10 months (5 years 4 months-15 years 10 months) with an average follow-up of 49 months (30-88 months). The percentage of femoral lengthening averaged 16.7% (8-23%) with an average healing index of 28 days/cm (20-38 days/cm). The percentage of tibial lengthening

The diameter of callus in leg lengthening: 28 tibial lengthenings in 14 patients with achondroplasia.

We investigated the relation between callus diameter during bone distraction and the occurrence of late fracture and deformity. We retrospectively reviewed 28 tibial lengthenings in 14 patients with achondroplasia. The minimal diameter of the lengthened zone was measured on radiographs, when the sliding mechanism of the lengthening device was released, and the callus diameter ratio in two planes (CDR; diameter of the callus/diameter of the tibia at the level of the osteotomy end) was calculated. The CDR correlated negatively with the distracted length. Late fracture or late angular deformity o

Distraction osteogenesis of the craniofacial skeleton.

In summary, distraction osteogenesis is a safe and effective means of achieving bone lengthening. These techniques were originally applied to the long bones of the extremities; over the past 10 years they have been effectively applied to the bones of the craniofacial skeleton. The new bone regenerate that is observed after distraction osteogenesis is stable, and relapse rates after skeletal advancement are believed to be lower than with conventional osteotomy and bone graft techniques. There is considerable variability in distraction protocols employed in clinical practice, including differenc
