Implant-associated infections (IAIs) represent one of the most challenging complications following orthopedic implantation. Current antibacterial strategies struggle to achieve complete pathogen eradication while maintaining sustained osteogenic activity at the implant-bone interface. Leveraging the spatial divergence in energy metabolism between prokaryotic and eukaryotic cells, we designed and synthesized guanidine-quaternary ammonium polymer-derived carbon dots (GQCDs) with confirmed structure. Under non-infected conditions, GQCDs enhance mitochondrial metabolism to promote bone regeneratio

In cementless total hip replacement surgery the conditions for micromotion initiation at the bone-stem interface and the role of stair climbing versus gait in promoting incipient slipping deserve attention. The goal of the present paper was to propose a finite element approach for analysing the structural behaviour of hip joint prostheses under physiological loadings and boundary conditions, which allows the prediction of micromotion initiation with low computational effort. In this paper, three-dimensional (3D) finite element analyses were performed of intact and implanted human femurs in ord

Bone-anchored limb prostheses allow for the direct transfer of external loads from the prosthesis to the skeleton, eliminating the need for a socket and the associated problems of poor fit, discomfort, and limited range of movement. A percutaneous implant system for direct skeletal attachment of an external limb must provide a long-term, mechanically stable interface to the bone, along with an infection barrier to the external environment. In addition, the mechanical integrity of the implant system and bone must be preserved despite constant stresses induced by the limb prosthesis. Three diffe

Revision surgery for particle-induced implant loosening in total joint replacement is expected to increase dramatically over the next few decades. This study was designed to investigate if local tissue and serum markers of bone remodeling reflect implant fixation following administration of lipopolysaccharide (LPS)-doped polyethylene (PE) particles in a rat model. Twenty-four rats received bilateral implantation of intramedullary titanium rods in the distal femur, followed by weekly bilateral intra-articular injection of either LPS-doped PE particles (n = 12) or vehicle that contained no parti

PURPOSE: To evaluate the biologic width dimensions around implants with nonmatching implant-abutment diameters.
MATERIALS AND METHODS: Five canines had their mandibular premolars and first molars removed bilaterally and replaced with 12 implants that had nonmatching implant-abutment diameters. On one side, six implants were placed in a submerged surgical approach, and the other side utilized a nonsubmerged approach. Two of the implants on each side were placed either 1 mm above, even with, or 1 mm below the alveolar crest. Two months later, gold crowns were attached, and the dogs were sacrifi

BACKGROUND: Improvement of implant healing in orthopedic and trauma surgery serves to improve the life expectancy of the implant. Good primary stability by clamping is a prerequisite for secondary stability and for the actual integration and healing of the implant.
RESULTS: Possible causes of implant loosening are abrasive particles, which arrive at non-integrated implants at the unsealed prosthesis-bone interface and provoke a macrophage-mediated foreign body reaction, resulting in periprosthetic osteolysis. Numerous animal studies have already described the use of bisphosphonates to inhibit

The objective of this work was to investigate the use of Biogran® functionalized with parathyroid hormone (PTH) 1-34 by sonochemistry for the local delivery of this anabolic agent to the implant site. The effects of Biogran® and topical administration of PTH 1-34 on peri-implant bone regeneration were evaluated from the microscale to ultrastructural levels in healthy (SHAM) and orchiectomized (ORQ). While some animals only received a titanium implant in their tibial metaphyses (CLOT group), in others the peri-implant defect was first filled with Biogran® either without or with PTH 1-34 functio

The success of hip arthroplasty is based on modern materials in addition to the continuous development of surgical techniques and clinical experience gained over six decades. The biocompatible implant materials used in hip arthroplasty can be textured or coated with biomimetic surfaces to ensure durable component ingrowth and moderate host response. Material integrity plays a critical role in the durability of the stable interface between implant components and periprosthetic tissues. Inflammation at the interfaces due to the release of degradation products from the implant materials is one of

PURPOSE: The purpose of this study is to analyze the impact of changes in the implant-abutment interface fit taper on loosening and fatigue performance.
MATERIALS AND METHODS: Three abutment structures with varying tapers (11.5°, 12°, and 13°) were self-designed and customized. Then, the impact of different taper fits between the abutment and implant on loosening and fatigue performance was investigated. The experiments measured screw preload, static and dynamic loosening torque, abutment settlement, pullout force, and the fatigue life of the implant system. The surface wear of the abutment a

PURPOSE: Based on a self-controlled case, this study evaluated the finite element analysis (FEA) results of a single missing molar with wide mesiodistal length (MDL) restored by a single or double implant-supported crown.
METHODS: A case of a missing bilateral mandibular first molar with wide MDL was restored using a single or double implant-supported crown. The implant survival and peri-implant bone were compared. FEA was conducted in coordination with the case using eight models with different MDLs (12, 13, 14, and 15 mm). Von Mises stress was calculated in the FEA to evaluate the biomechan

Several implant materials used in dental and orthopedic surgery were placed in rat tibial bones to study their effects on mineralization. The implants consisted of bone bonding and non-bonding materials. Changes in mineralization were defined by morphometric analysis of matrix vesicle distribution at the implant interface and in normal bone healing following marrow injury. Bone-bonding materials induced an increase in matrix vesicle activity. This finding was supported by study of the biochemical changes in the same model that manifested high correlations to the morphometrical observations wit

Osseointegrated implants inserted in the temporal bone are a vital component of bone-anchored hearing systems (BAHS). Despite low implant fa … ppeared within the bone ~30-50 μm from the laser-modified surface, while separation occurred at the bone-implant interface for the machined surface. Strong correlations were found between RTQ and BIC and between RFA at retrieval and BA. A bovine-bone mineral block was used to treat a severe horizontal and vertical anterior ridge deficiency. Such a block can be shaped to conform to the defect, and it avoids the need for harvesting autogenous bone or fixation of the block with screws. After a 6-month integration period, an implant was placed. Six months later the implant was restored with a single crown. The case has been followed for 3 years. Slow bone resorption has been observed at the interface between the block and the surrounding bone, but the interproximal bone peaks, important for soft tissue support and esthetics, have

BACKGROUND: Trans- inferior alveolar nerve (IAN) implantation technique was wildly used while the potential appropriate angle range in which the residual alveolar bone can bear the stress without absorption are currently unclear. This study aimed to evaluate the stress distribution pattern of the interface between bone and implant by finite element analysis (FEA) to determine the appropriate range of the implant tilt angle.
METHODS: Cone beam computed tomography (CBCT) images of 120 patients with missing mandibular second molars and vertical bone height < 9 mm in the edentulous area were sele

Pedicle screw stability is poor in osteopenic vertebrae attributable, in part, to low screw-bone interface strength. The current authors examined cement augmentation using a low curing temperature hydroxyapatite and bis-phenol-A glycidol methacrylate-based composite resin. This cement may stiffen the screw-bone interface and reduce the harmful effects associated with polymethylmethacrylate regarding temperature and toxic monomer. Thirty-five lumbar vertebrae from human cadavers were instrumented with pedicle screws, with one pedicle previously injected with cement and the other as the control.

The primary stability of cementless acetabular components is a prerequisite for their clinical success. The target of the present study was to analyse possible effects of post-operative joint loading on the initial mechanical stability of a press-fitted acetabular prosthesis. For this purpose, a three-dimensional finite element model of the pelvic bone with acetabular reconstruction was set-up. The analysis included two steps: (1) simulation of the prosthesis press-fit implantation and (2) simulation of the instant of peak resultant hip loading during the one-legged stance. The difference betw

AIM: Radiotherapy is associated with cell depletion and loss of blood supply, which are linked to compromised bone healing. However, the molecular events underlying these effects at the tissue-implant interface have not been fully elucidated. This study aimed to determine the major molecular mediators associated with compromised osseointegration due to previous exposure to radiation.
MATERIALS AND METHODS: Titanium implants were placed in rat tibiae with or without pre-exposure to 20 Gy irradiation. Histomorphometric, biomechanical, quantitative polymerase chain reaction (qPCR) and enzyme-lin

Due to the increasing number of periprosthetic femoral fractures (PFF), the optimisation of implant design gains importance. For the presented research a validated, subject specific finite element model of a human femur with an inlying total hip stem was used to compare the influence of different geometrical implant parameters on the development of PFF. The heterogeneous bone tissue was modelled on the basis of computed tomography scans. A ductile damage model with element deletion was applied to simulate bone fracture in a load case re-enacting a stumbling scenario. The results were compared
