Pre-implantological bone formation in the floor of the maxillary sinus in a self-supporting space.

INTRODUCTION: In edentulous patients the form and size of the maxillary sinus vary greatly. Therefore sinus floor augmentation is a standard procedure for implantological purposes. As the sinus membrane cannot be characterized as periosteum, various augmentation materials are used. HYPOTHESIS: an artificially generated space underneath the sinus membrane in the floor of the sinus will lead to spontaneous callus forming and a stable bony consolidation without augmentation material. METHODS: Ten edentulous patients with highly atrophic maxillae were selected. Augmentation of the sinus floor wa

Fracture healing in osteoporotic bone.

As the world population rises, osteoporotic fracture is an emerging global threat to the well-being of elderly patients. The process of fracture healing by intramembranous ossification or/and endochondral ossification involve many well-orchestrated events including the signaling, recruitment and differentiation of mesenchymal stem cells (MSCs) during the early phase; formation of a hard callus and extracellular matrix, angiogenesis and revascularization during the mid-phase; and finally callus remodeling at the late phase of fracture healing. Through clinical and animal research, many of these

Thoracic outlet syndrome secondary to a mid-clavicle malunion.

A 22-year-old man presented with a painful 'clunking' sensation in the right mid-clavicle, and pain and dysaesthesia along the medial aspect of his right arm and hand. Three months earlier, he had been involved in a vehicle accident and sustained a right clavicle fracture. He had a large step off of the right clavicle with a medialisation of the right shoulder. At 90° abduction in external rotation of both shoulders he developed pain, paraesthesia and disappearance of the right radial artery pulsation. CT of the right shoulder in the neutral position demonstrated the clavicle-to-first rib dist

Comparison between upper and lower limb lengthening in patients with achondroplasia: a retrospective study.

Lengthening of the humerus is now an established technique. We compared the complications of humeral lengthening with those of femoral lengthening and investigated whether or not the callus formation in the humerus proceeds at a higher rate than that in the femur. A total of 24 humeral and 24 femoral lengthenings were performed on 12 patients with achondroplasia. We measured the pixel value ratio (PVR) of the lengthened area on radiographs and each radiograph was analysed for the shape, type and density of the callus. The quality of life (QOL) of the patients after humeral lengthening was comp

Rejuvenation of the inflammatory system stimulates fracture repair in aged mice.

Age significantly reduces the regenerative capacity of the skeleton, but the underlying causes are unknown. Here, we tested whether the functional status of inflammatory cells contributes to delayed healing in aged animals. We created chimeric mice by bone marrow transplantation after lethal irradiation. In this model, chondrocytes and osteoblasts in the regenerate are derived exclusively from host cells while inflammatory cells are derived from the donor. Using this model, the inflammatory system of middle-aged mice (12 month old) was replaced by transplanted bone marrow from juvenile mice (4

Prognostic significance of BsALP in healing of long bone fractures.

The aim of this work was to assess the relationship of both total alkaline phosphatase (ALP) and bone-specific alkaline phosphatase (BsALP) with the course and outcome of operatively treated long-bone fractures. The activity of total ALP and BsALP was measured in 41 patients with a long bone fracture, comprising 26 men and 15 women. All patients were treated operatively. Total ALP and BsALP levels were measured in sera on day 1, 7, 14 and 21 after sustaining injury. Patient monitoring included X-rays. According to the outcome, patients were divided into two groups: the fast healing group and t

Biomechanical comparison of callus over a locked intramedullary nail in various segmental bone defects in a sheep model.

Humans and food-producing animals are constantly exposed to and affected by stress. As a consequence of stress, the release of stress-related catecholamines, such as norepinephrine (NE) and dopamine (DA), from nerve terminals in the gastrointestinal tract potentiates both the growth and the virulence of pathogenic bacteria. This may lead to the enhancement of gastrointestinal infections in humans or food-producing animals. Compared with foodborne bacterial pathogens such as Escherichia coli and Salmonella spp., less is known about the effect of stress catecholamines on Campylobacter jejuni sub

Bone non-union after osteotomy in patients treated with methotrexate.

We describe 2 men with seronegative rheumatoid arthritis who presented non-union of bone after osteotomy of the tibia in one case and of a metatarsal bone in the second. Both patients were still being treated with oral low doses of methotrexate (MTX). After MTX was stopped, a prompt healing of the bone occurred. These observations suggest that MTX should be stopped temporarily in cases with delayed bone healing after surgery.

Two consecutive rib stress fractures in a female competitive swimmer.

We present a case of two consecutive stress fractures in a female swimmer. The diagnosis of the present stress fracture of the ninth rib was based on clinical history and examination and on a new fracture line and callus formation seen in consequent conventional radiographs. Based on the clinical history and radiography, the patient had suffered another rib stress fracture in the fifth rib 15 months earlier. No external trauma had preceded either of the fractures, and no secondary cause of stress fracture was found. Her anatomical and biomechanical characteristics and training errors seem to h

[Internal callus distraction in epiphyseal groove defect--a physiologic means of spontaneous correction. Results of a comparative histomorphologic study in a small and large animal experiment].

For this experimental study we removed with a trephine in small (rabbit) and larger (pigs) animals cylinders of 2 mm respectively 8 mm diameter from the central growth plate area of proximal tibia. The direction of burring is in the axial line of the femur proximo-distally; the insertion point of the trephine is chosen on the cheek of the medial femoral condyle. As common reaction to that kind of trauma the defects in growth plates of rabbits become filled with a bone bridge that joins the epiphyseal core to the metaphyseal trabeculae, leading to gross growth disturbances. In pigs a similar so

Capacitively coupled electrical field in the treatment of a leg fracture after total knee replacement.

Electrical stimulation has been used as treatment for nonunions of fractures since the early 1950s, with a reported success rate of 80-85%. We report a case of nonunion of a tibial fracture below a revised total knee prosthesis treated with a capacitively coupled electrical field. After 3 months of treatment, consolidation of this difficult fracture was evident with abundant callus formation.

Neutral proteases in regenerating bone.

Proteolytic enzymes acting at physiologic pH (neutral proteases) are involved in both the formation and modeling of new bone and the remodeling of mature bone. In endochondral ossification systems such as growth-plate calcification, fracture healing, osteophyte formation, and demineralized bone matrix-induced osteogenesis, neutral proteases are predominantly involved in modifying proteins and proteoglycans in the extracellular matrix in preparation for calcification. These enzymes are of low molecular weight (below 30,000 Mr), are poorly charged, metal ion dependent, and appear to become activ

X-ray microanalysis of fluoride distribution in microfracture calluses in cancellous iliac bone from osteoporotic patients treated with fluoride and untreated.

Fluoride is able to augment cancellous bone mass in vertebral osteoporosis but is responsible for osteoarticular side effects in which microfractures are thought to be involved. During healing of these microfractures, a callus is formed all around the cancellous fracture line. Our hypothesis is that in fluoride-treated osteoporotic patients, calluses are bone sites where fluoride is focally deposited at a high concentration, and this could induce a local defect of calcification with a poor healing of microfractures. Our aim was to validate this hypothesis on several calluses following microfra

Bone Scan in Detection of Biological Activity in Nonhypertrophic Fracture Nonunion.

Biological activity of the fracture site is very important factor in treatment planning of fracture nonunion. If no biological activity is detected, then an autologous bone graft can be supplemented or osteogenic supplementations, such as bone morphogenetic protein is given. If biological activity is present, then secure fixation is sufficient to achieve bony union. Biological activity of nonunions is usually assessed by conventional radiographs. The presence of callus formation is usually assessed as the presence of biological activity. However, high number of radiologically nonhypertrophic n

Early correction of paediatric malunited distal metaphyseal radius fractures using percutaneous callus osteoclasis ("Calloclasis").

INTRODUCTION: Closed reduction of secondary displacements of distal metaphyseal radius fractures is an easy procedure only when performed within the first ten to fifteen days post-trauma, prior to the occurrence of malunion. When a hard bony callus prevents proper reduction, an open osteotomy is generally advocated. HYPOTHESIS: We suggest the use of a less invasive technique which aims at correcting early malunion when closed reduction is made impossible: the percutaneous callus osteoclasis. MATERIALS AND METHODS: Callus osteoclasis consists in a series of multiple bone-drilling in a postage

Bone morphogenetic protein-7 accelerates fracture healing in osteoporotic rats.

BACKGROUND: Osteoporosis is characterized by low bone mass, bone fragility and increased susceptibility to fracture. Fracture healing in osteoporosis is delayed and rates of implant failure are high with few biological treatment options available. This study aimed to determine whether a single dose of bone morphogenetic protein-7 (BMP-7) in a collagen/carboxy-methyl cellulose (CMC) composite enhanced fracture healing in an osteoporotic rat model. MATERIALS AND METHODS: An open femoral midshaft osteotomy was performed in female rats 3 months post-ovarectomy. Rats were randomized to receive eit

Bioceramic implantation in the intermandibular space in bilateral rostral mandibulectomy of the dog.

Effect of bioceramic implantation on the intermandibular space along with plating in rostral mandibulectomy was investigated in the dog. In this group, bioceramic beads consisting of a complex of hydroxyapatite and tricalcium phosphate were implanted in the intermandibular space between the lingual surfaces of the fracture ends. In two other groups, the dogs received only plating on the ostectomized end, or they received only rostal mandibulectomy without plating. No difficulty in mastication was observed in the implanted group after surgery. In the dogs which received plating, however, diffic

Parathyroid hormone and trabectedin have differing effects on macrophages and stress fracture repair.

Stress fractures occur as a result of repeated mechanical stress on bone and are commonly found in the load-bearing lower extremities. Macrophages are key players in the immune system and play an important role in bone remodeling and fracture healing. However, the role of macrophages in stress fractures has not been adequately addressed. We hypothesize that macrophage infiltration into a stress fracture callus site promotes bone healing. To test this, a unilateral stress fracture induction model was employed in which the murine ulna of four-month-old, C57BL/6 J male mice was repeatedly loaded

Proteomic profiling links increased mast cell activity in regenerating callus to impaired scaffold-guided bone regeneration in diabetic rats.

Type 2 diabetes mellitus significantly impairs bone healing, with delayed regeneration observed in over 60% of patients with diabetes. Using time-resolved proteome profiling of critical-sized femoral defects in diabetic and non-diabetic rats treated with polycaprolactone scaffolds, we investigated the molecular mechanisms underlying diabetes-associated compromised regeneration. Explanted regenerated callus and contralateral bone tissue were analyzed at 21 and 42 days post-surgery using mass spectrometry-based quantitative proteomics and functional enrichment analysis, complemented by histologi
